Summary: Chronic kidney disease-mineral and bone disorder (CKD-MBD) is common in older non-dialysis patients with advanced CKD. PTH and phosphate are independently associated with all-cause mortality, while only PTH is associated with cardiovascular mortality. Calcium does not have an independent association with mortality but modifies the effect of phosphate.

Summary: Chronic kidney disease is a global public health issue, with several recently published clinical practice guidelines regarding nutritional management. A study evaluating 11 guidelines found that those developed by KDOQI, ESPEN, and DAA were of moderate quality, while others were low-quality, suggesting a need for improved methodological approaches to enhance guideline quality.

Summary: Chronic kidney disease (CKD) associated with type 2 diabetes (T2D) is a serious complication with high morbidity and mortality. Current guidelines recommend annual screening for CKD, blood pressure and glucose control, as well as the use of renin-angiotensin system (RAS) inhibition. Recent additions to the guidelines include the use of SGLT2 inhibitors and finerenone, based on robust clinical trial data, to improve CKD outcomes in T2D patients.

Summary: This article discusses the importance of play in city planning, arguing that play is not just for children but has physical and mental benefits for people of all ages. The author suggests that play can combat the challenges associated with aging and should be considered in the planning of age-friendly communities. The article calls for further research and practice in the relationship between play, public space, and older adult well-being.

Summary: This study conducted exome sequencing (ES) on adult nephropathy patients and developed a clinical score to predict the possibility of obtaining a positive molecular diagnosis. The results showed that 24% of the patients were diagnosed using ES, and 47 distinct monogenic disorders were identified. This study highlights the role of ES in adult chronic kidney disease patients and suggests that the clinical score could be a useful tool for implementing nephrogenomics in adults.

Summary: This study investigated healthcare provider awareness of injury prevention resources for falls and motor-vehicle crashes in older adults. The results showed that only 20% of providers reported awareness of any of the resources. Providers were more likely to screen for fall risk or unsafe driving when an older adult presented with a fall or driving concern, rather than conducting annual screenings.

Summary: Cognitive impairment is a common complication of aging and is associated with chronic kidney disease. Vascular dysfunction has been linked to cognitive impairment in older adults, particularly those with CKD. Assessing cognitive and vascular function in older adults with preclinical cognitive impairment, with and without CKD, may be beneficial.

Summary: Older patients with advanced CKD are at high risk for complications and death, yet discussions on advance care planning (ACP) with kidney clinicians are rare. Study found discordant views between patients and clinicians on ACP, with cultural and religious barriers. Four themes influencing ACP discussions emerged, leading to insufficient ACP discussions with older adults with advanced CKD.
