Summary: The increasing incidence of esophageal adenocarcinoma (EAC) has prompted a reevaluation of screening methods. Recent research has identified two phenotypes of EAC, with the non-barrett esophagus/non-intestinal metaplasia phenotype associated with more aggressive behavior and worse survival. Additionally, the rising incidence of EAC cannot be attributed to new risk factors but rather a higher prevalence of known risk factors. Differences in genomic methylation and immune response may explain the two EAC phenotypes.

Summary: This study retrospectively evaluated 511 patients with esophageal cancer who underwent esophagectomy and found that increasing age, preoperative hypertension, respiratory complications, and Ivor Lewis approach were independent risk factors for postoperative atrial fibrillation (POAF). POAF was associated with prolonged length of stay. The study suggests that older patients, patients with hypertension, or patients who underwent Ivor Lewis approach should be closely monitored during the postoperative period.

Summary: The systematic review focused on the rising incidence of esophageal adenocarcinoma (EAC) in developed countries and its associated risk factors, finding that body mass index and waist circumference were linked to increased EAC risk, while physical activity did not have a significant direct role in EAC risk. A diet rich in fruit, vegetables, and whole grains appeared to be more protective, while alcohol did not seem to be related to EAC, and smokers, especially heavy smokers, had an increased risk. Providing clear recommendations may help in preventing EAC.

Summary: Based on the Wuwei Cohort, this study found that different risk factors are associated with different stages of gastric adenocarcinoma (GAC) progression. Factors such as age, occupation, low income, Helicobacter pylori infection, drinking, and eating hot food are associated with the development of chronic atrophic gastritis (CAG). Factors such as age, illiteracy, H. pylori infection, smoking, and eating habits are associated with the progression from CAG to intestinal metaplasia (IM). Male gender, occupation, and history of peptic ulcer are associated with the development of low-grade dysplasia (LGD) from IM. Age, male gender, and polyp history are risk factors for the development of GAC/high-grade dysplasia (HGD) from LGD. The study highlights the importance of H. pylori eradication and control of other risk factors to reduce the incidence of GAC.

Summary: This study summarizes the known modifiable risk factors for pancreatic ductal adenocarcinoma (PDAC) as well as recent reports. PDAC has a late diagnosis and poor prognosis, making accurate analysis of risk factors crucial. The incidence of PDAC is increasing, with the lowest five-year survival rate among all tumors. The impact of cigarette smoking, alcohol consumption, and chronic pancreatitis in increasing the risk of PDAC is continuously confirmed, while emerging reports focus on the influence of lifestyle factors and hepatotropic viruses.

Summary: High-definition endoscopy is crucial in the initial diagnosis of inflammatory bowel disease (IBD), helping to characterize inflammation and differentiate between ulcerative colitis and Crohn's disease. In addition to videoendoscopy, the use of dyes sprayed via a catheter can improve detection rates of intraepithelial neoplasia. Virtual chromoendoscopy techniques should not be the sole surveillance strategy in IBD, while the results of oral methylene blue formulation show promise in increasing acceptance rates of classic chromoendoscopy.

Summary: This study investigated the associations between clinical characteristics of refractory celiac disease patients and aberrant intraepithelial lymphocytes, and evaluated the accuracy of immunophenotyping for identifying high-risk refractory celiac disease. The results showed that flow cytometry analysis is a good predictor for high-risk refractory celiac disease, and adding the evaluation of malabsorption to the diagnostic assessment improves accuracy.
