Summary: The Lyon Consensus 2.0 provides conclusive criteria for diagnosing gastro-oesophageal reflux disease (GERD) and offers additional metrics to support or refute the diagnosis when primary criteria are inconclusive. A global team evaluated research since the original Lyon Consensus and collaborated on developing statements to update criteria. The updated consensus includes a modern definition of actionable GERD, describes the likelihood of symptoms being related to reflux episodes, and provides diagnostic strategies and testing options based on proven or unproven GERD.

Summary: This study aimed to evaluate the feasibility, effectiveness, and safety of anti-reflux mucosal ablation (ARMA) in the treatment of gastroesophageal reflux disease (GERD). The results showed that ARMA is a feasible and safe procedure that effectively reduces GERD symptoms in the short term, with clinical success achieved in approximately two-thirds of patients. Both reflux hypersensitivity and confirmed GERD patients, regardless of their response to acid suppression medication, may be suitable candidates for ARMA treatment.

Summary: Gastroesophageal reflux disease affects a significant portion of the population worldwide. Minimally invasive laparoscopic fundoplication is an effective treatment for selected patients. However, a small percentage of patients may require revisional surgery. Endoscopy plays an important role in evaluating the outcomes of fundoplication surgery.

Summary: The study highlights the incidence of postoperative GERD and endoscopic lesions following sleeve gastrectomy. While SG is not contraindicated in cases of reflux, patients may require close endoscopic surveillance post-surgery. Some patients also reported a lack of regular medical follow-up after 5 years.

Summary: Gastroesophageal reflux disease (GERD) is a common gastrointestinal disease that can be diagnosed based on clinical presentation and risk factors, although atypical symptoms may make diagnosis challenging. Advanced endoscopic techniques have been developed to enhance the diagnosis of GERD and related complications, providing improved detection of subtle irregularities in the mucosa and vascular structures.

Summary: This study validated 4 tools for predicting incident EAC and EGJAC in a large community-based population, with the Kunzmann tool showing the best calibration. Tools such as the Nord-Trondelag Health Study tool and Michigan Barrett's Esophagus pREdiction Tool were more accurate than using GERD alone for risk stratification. Future studies should focus on implementing these tools into clinical practice.

Summary: The study evaluated the association between reflux esophagitis and fatigue, finding no statistically significant association between reflux esophagitis and daytime sleepiness, fatigue, anxiety, or depression. However, fatigue was associated with GERD symptoms, being women, anxiety, and depression. Further research is needed to clarify the relationship between fatigue and reflux esophagitis.

Summary: Esophageal granular cell tumors are rare tumors that are more common in men and predominantly located in the distal esophagus. Some patients with these tumors present with regurgitation and/or heartburn symptoms and are confirmed to have reflux esophagitis through endoscopy. Endoscopic resection is an effective method for diagnosis and treatment of esophageal GCTs.

Summary: GERD is a prevalent medical condition that can cause chronic cough. The underlying molecular mechanism behind GER-related chronic cough (GERC) is still unclear. Different theories, such as reflux theory, reflex theory, airway allergies, and esophageal motility disorders, are linked to GERC. Multichannel intraluminal impedance combined with pH monitoring is the gold standard for diagnosing GERC, but it is invasive and not well tolerated by patients. Recent discoveries in impedance markers and techniques show promise for the diagnosis of GERD, but further investigation is needed for GERC. Advances in pharmacological and non-pharmacological treatments provide more options for managing GERC.

Summary: This study retrospectively investigated the prevalence of hiatal hernia (HH) and its association with age, gender, and body mass index (BMI) in patients who underwent esophagogastroduodenoscopy (EGD). The results showed that the prevalence of HH was 29.8% among all patients, and there was no significant association between HH and gender, age, or BMI.

Summary: In this multicenter study, women with BE were found to have a significantly lower risk of disease progression to cancer and HGD compared to men. The extremely low annual incidence rate of EAC in women with BE suggests that surveillance endoscopy may not be necessary for this subgroup of patients.

Summary: This study aimed to demonstrate the feasibility, safety, and effectiveness of endoscopic suturing (ES) for the closure of mucosal defects after endoscopic mucosal resection (EMR) of duodenal adenomas. ES was found to be technically successful in all cases and no complications such as bleeding or duodenal stricture were encountered. ES should be considered as an option for preventing or treating perforations associated with EMR of large duodenal adenomas.

Summary: We assessed the post-ERCP 30-day readmission rates, their etiologies, and impact on the health care system in the United States using national data. A total of 130,145 patients underwent ERCP, with a 12.5% readmission rate within 30 days, costing $268 million. The majority of readmissions occurred within 7 days and were related to gastrointestinal issues. Male gender, comorbidities, cirrhosis, Medicare insurance, and pancreatitis or pancreatitis-related indications were risk factors for readmission. Performing cholecystectomy during the index hospitalization reduced the odds of readmission by 50%.

Summary: Although the global incidence of non-cardia gastric cancer (NCGC) is decreasing, there are limited data on sex-specific incidence in the U.S. Our study found that NCGC incidence has been increasing at a greater rate in younger women compared to counterpart men, and this rise may be explained by a disproportionate increase in young non-Hispanic White women. These findings highlight the need for dedicated research to determine the etiologies of these alarming trends.
