Summary: Acute pancreatitis causes changes in the structure and function of the gut microbiome, with significant community succession observed within the initial 72 hours. The intestinal F/B ratio and the relative abundance of Lactobacillus may serve as potential markers for early diagnosis of acute pancreatitis. The genus Clostridium sensu stricto 1 was found to be the most enriched genus in acute pancreatitis and may be an important marker for its diagnosis.

Summary: This study investigated the common causes, routine diagnostic and therapeutic methods of acute pancreatitis in children in a pediatric gastrointestinal referral center. There were variations in causes and methods among different centers. It is important to consider the association between acute pancreatitis and systemic and metabolic diseases as well as the use of anticonvulsant drugs. The risks of unnecessary CT scans and inappropriate antibiotic prescription should be emphasized.

Summary: Acute pancreatitis is a common disease in gastroenterology, with gallstones and alcohol as the main causes. Diagnostic criteria and imaging techniques have been well defined, with ultrasound being useful for diagnosing the cause and computed tomography being better for assessing risk and complications. Fluid therapy, early oral feeding, and pain management are the mainstay of early treatment for acute pancreatitis. Antibiotics are useful when infected necrosis or extra pancreatic infections are present or suspected, but not for prophylaxis in sterile necrotizing pancreatitis. Minimally invasive approaches have emerged for managing walled off necrosis, improving complications rates, quality of life, and hospital stay compared to open surgery.

Summary: In the management of pediatric acute pancreatitis, early enteral feeding has been shown to improve outcomes and reduce hospital length of stay. Through interventions such as the development of a clinical care pathway and physician education, the proportion of patients receiving early enteral nutrition increased from 40% to 84%, while hospital length of stay significantly decreased. There was no increase in hospital readmission rates.

Summary: The study aimed to establish a risk score, DORM65, based on known variables to predict the likelihood of a positive endoscopic ultrasound (EUS) result in patients with idiopathic acute pancreatitis. The DORM65 score includes five predictors: delayed EUS, obesity, no repeated transabdominal ultrasound, male sex, and age >= 65 years. The score system accurately predicts the positive EUS result in patients with idiopathic acute pancreatitis.
