Summary: This observational cohort study developed a novel test for diagnosing fecal incontinence. By stimulating nerve plexuses and recording muscle motor potentials, the study found that lumbar or sacral plexus neuropathy was detected in 40% to 75% of patients with fecal incontinence, with a 2-fold greater prevalence at the anal region than the rectum.

Summary: This study investigated the use of 3-dimensional high-resolution anorectal manometry (3D-HRAM) in diagnosing rectoperineal congenital anorectal malformations (CARMs) and evaluating its diagnostic value. The results showed that 3D-HRAM had a specificity and sensitivity of 100% in diagnosing rectoperineal CARM, providing a reliable tool for diagnosis and avoiding unnecessary anesthesia procedures.

Summary: Anorectal melanoma is a subtype of mucosal melanoma with a poor prognosis. The optimal treatment approach for anorectal melanoma is still evolving, despite recent advancements in cutaneous melanoma. This review focuses on the differences in pathogenesis between mucosal and cutaneous melanoma, new staging concepts for mucosal melanoma, updates in surgical management, and current data on adjuvant radiation and systemic therapy for this unique patient population.

Summary: Complications related to the gastrointestinal tract commonly occur in patients with diabetes mellitus. This study found that DM patients often report lower gastrointestinal symptoms, with those with long disease duration experiencing more anal discomfort and a sensation of incomplete evacuation. Overall, DM patients exhibited impaired anorectal function compared to control subjects, showing features of dyssynergic defecation and impaired visceral sensation.

Summary: Despite surgical correction, long-term bowel dysfunction can occur in children with anorectal malformations, including fecal incontinence and evacuation disorders. Anorectal manometry is commonly used to assess anorectal function in this population, but there is a lack of standardized protocols and interpretation. This systematic review evaluated existing data on anorectal manometry results in children following anorectal malformation repair and highlighted the need for standardized protocols to enhance comparability and clinical relevance.

Summary: This study compared clinical characteristics and anorectal pressure profile in patients with HSD/hEDS to those of age and sex matched controls. The results showed that the anorectal pressure profile in HSD/hEDS patients was compromised, while connective tissue pathologies may not have an impact. Further investigation is needed.

Summary: This study developed a deep learning algorithm to evaluate dyssynergic defecation using 3-dimensional high definition anal manometry (3D-HDAM) and compared it with traditional machine learning and hybrid methods. The results showed that the deep learning model had comparable diagnostic accuracy in the training cohort and handled ambiguous tests more cautiously.

Summary: The study investigated the clinical characteristics, psychological status, sleep quality, and quality of life in patients with functional anorectal pain (FAP). It also assessed the efficacy of paroxetine in alleviating FAP symptoms. The results showed that FAP patients commonly experience symptoms of anxiety, depression, and sleep disturbances. Paroxetine was found to improve depression, anxiety, and pain symptoms in FAP patients.

Summary: The study aimed to generate reference values for anorectal pressures, examining the effects of gender and age. Results showed that anal and rectal pressures measured using diagnostic and biofeedback catheters were correlated and not significantly different, with pressures found to be influenced by age and sex. Specific pressures during different activities varied among different gender and age groups, providing valuable reference values for clinicians.

Summary: Anorectal manometry measurements show significant interrater variability. New techniques like 3D high-resolution anorectal manometry (3D-HRAM) have the potential to improve diagnostic accuracy and understanding of defecation disorders. However, the extent of interrater variability in 3D-HRAM is still unknown.

Summary: This study evaluated constipated children with 3DHRAM and determined a new pediatric cut-off for DD variables. The analysis showed that the percent anal relaxation during straining significantly differed between patients and controls, leading to a new cut-off value of 31% for this variable.

Summary: Robot-assisted anorectal pull-through (RAARP) is a safe and effective alternative for the treatment of anorectal malformations, minimizing the injury to perirectal nerves and sphincters. The application of nerve- and sphincter-sparing technique in RAARP facilitates favorable evolution of continence and defecation functions in patients post-operatively. Overall, RAARP showed promising feasibility and early outcomes in the treatment of anorectal malformations.
