Summary: Approximately one-third of patients with Crohn's disease develop fibrostenotic intestinal disease, which is difficult to treat even with new biologic agents. Medical treatment, especially anti-TNF agents, provides the most significant benefit for inflammatory strictures. However, surgery may be required for patients with fibrotic strictures or those refractory to medical treatment. Endoscopic balloon dilatation and endoscopic stricturoplasty are minimally invasive and safe options, while surgery is indicated for patients who cannot undergo endoscopic therapy. This review presents and analyzes the available medical, endoscopic, and surgical management approaches for stricturing Crohn's disease.

Summary: Fibrostenosis is a major feature of Crohn's disease (CD) and poses a challenge in the clinical management of inflammatory bowel disease patients. While there are no specific therapies to prevent or treat fibrosis in CD currently, efforts are being made to standardize definitions and propose endpoints for better management, along with research to develop future anti-fibrotic therapies.

Summary: This study aimed to analyze disease progression and surgery rates in population-based cohort over different therapeutic eras. The study found that the probability of disease progression from inflammatory to stenosing or penetrating phenotype significantly decreased and the probability of first resective surgery also decreased. Overall, the rates of surgery and disease progression decreased over time.

Summary: This study aimed to predict surgical risks for patients with symptomatic stricturing Crohn's disease (CD) using computed tomography enterography (CTE), and to assess the association between CTE findings and pathological changes. The results showed that CTE variables including bowel wall thickness, luminal stenosis, and upstream lumen were predictors of surgical-free survival for symptomatic stricturing CD patients. CTE was highly predictive of disease course and surgical-free survival, suggesting its important role in treatment decision-making.

Summary: In this study, a large database was used to explore the association between celiac disease and inflammatory bowel disease (IBD) and the impact of IBD treatment. The results showed a bidirectional association between celiac disease and IBD, with a stronger association observed in Crohn's disease (CD). Patients with IBD who received corticosteroids, 5-aminosalicylates, immunomodulators, or anti-tumor necrosis factor drugs had a lower association with celiac disease. Additional research is needed to understand the underlying mechanisms for the modification of celiac disease incidence by IBD therapy.
