Summary: This study aimed to assess whether patients with Medicaid or Medicare-Medicaid dual eligibility had lower return to use of ambulatory care rates compared to other insurance types during the pandemic. The results showed that overall utilization of 6 ambulatory care services decreased between March 2020 and February 2021, and participants with Medicaid or Medicare-Medicaid dual eligibility had a significantly lower rate of increase in utilization compared to other insurance types.

Summary: A study with a test-negative design analyzed 41,552 admissions to 187 hospitals and 21,522 visits to 221 EDs or urgent care clinics. The mRNA-based vaccines (>= 14 days after the second dose) were highly effective against SARS-CoV-2 infection leading to hospitalization (89%), ICU admission (90%), or an urgent care visit (91%).

Summary: This study highlighted the demographic disparities in proximity to certified stroke care, with greater disparities observed in nonurban areas compared to urban areas. Higher proportions of elderly, American Indian, uninsured, or low-income populations in nonurban census tracts were found to be more distant from certified stroke care facilities.

Summary: In this study of a nationally representative data set in the United States, the prevalence, symptom severity, and medication use in individuals with Rome IV chronic idiopathic constipation (CIC), opioid-induced constipation (OIC), and opioid-exacerbated constipation (OEC) were assessed. The results showed that OIC and OEC were less prevalent than CIC, but individuals with OIC and OEC had more severe constipation symptoms and were more likely to use prescription medication.

Summary: In the survey, only 36% of respondents were willing to take the vaccine as soon as it became available, while 56% were undecided. Only 8% of healthcare workers do not plan to get vaccinated. Vaccine acceptance increased with age, education, and income level, with direct medical care providers showing higher acceptance rates.

Summary: The prevalence of non-HIV sexually transmitted infections (STIs) has increased over the years in U.S. ambulatory-care settings, mainly driven by HPV-related infections, syphilis, and gonorrhea. Younger age, Black race, and HIV diagnosis were associated with higher odds of having STI-related visits.

Summary: The management of chronic symptomatic functional bowel disorders (FBDs) in the United States incurs considerable healthcare resource use and cost, with pharmacologic therapies being more commonly prescribed than nonpharmacologic interventions. Combination treatment strategies are more likely to be implemented by primary care physicians, highlighting the need to improve comprehensive FBD management.

Summary: Health insurance coverage in the United States is uncertain and unstable. Even with the Affordable Care Act (ACA), a significant number of people remain uninsured at some point over a 2-year period. The risk of losing insurance is higher for those with Medicaid or private exchange coverage compared to those with employer-provided coverage. It is important to focus on both the percentage of uninsured individuals at a specific time and the stability and certainty of insurance coverage.

The results suggest that regional environments may require interventions to negotiate historically marginalizing relationships.

Summary: This study assessed whether the use and outcomes of medical crowdfunding align with health financing needs in the United States. The findings showed that medical crowdfunding raised unequal amounts of money and had low success rates, particularly in areas with higher medical debt, uninsured populations, and poverty. Therefore, medical crowdfunding is misaligned with key indicators of health financing needs in the United States.

Summary: The study found that the Affordable Care Act's increase in Medicaid fees did not lead to more primary care visits for dual-eligible Medicare and Medicaid beneficiaries. Instead, there were decreases in visits with primary care physicians, partially offset by increases in visits with nurse practitioners and physician assistants.

Summary: This study conducted a systematic review and meta-analysis to investigate the placebo response in pharmacological trials for functional dyspepsia (FD). The study found that the pooled placebo response rate ranged from 35.5% to 39.6% depending on the responder definition used. Lower baseline symptom scores were significantly associated with higher placebo response rates. The study suggests considering entry criteria based on symptom severity and separate reporting of core FD symptoms in future trials.

Summary: This review summarizes the direct comparisons between subcutaneous semaglutide and other GLP-1 RAs in individuals with type 2 diabetes, focusing on their efficacy for inducing weight loss and improving metabolic health markers. Five studies which fulfilled the inclusion criteria were identified. Randomized trials support the superior efficacy of semaglutide over other GLP-1 RAs in terms of weight loss, but tirzepatide is more effective than semaglutide.

Summary: This systematic review assessed the association between the microbiota and metabolome in diverticulosis, symptomatic uncomplicated diverticular disease (SUDD), and diverticulitis pathophysiology. The findings suggest that there is no convincing evidence of microbial dysbiosis in colonic diverticula to suggest that the microbiota contributes to the pathogenesis of diverticulosis, SUDD, or diverticular disease.

Summary: This study aimed to compare the circulating concentrations of amino acids and neurotransmitters between healthy participants and those with functional gastrointestinal disorders (FGIDs). The results showed significant differences in branched-chain amino acids, ornithine, and alpha-aminobutyric acid between healthy controls and FGID groups. No other significant differences were observed for the neurotransmitters or any other amino acids analyzed.

Summary: This study examined the changes of eosinophils and other immune cells in the mucosa of colonic diverticula through quantitative histology. The results showed a significant increase in eosinophil counts at the base and neck of the diverticula. These findings provide new clues for understanding the role of eosinophils and chronic inflammation in the pathophysiology of colonic diverticulosis.
