Combined High-dose Radiation Therapy and Systemic Chemotherapy Improves Survival in Patients With Newly Diagnosed Metastatic Nasopharyngeal Cancer

Objectives: To investigate the efficacy of high-dose radiation therapy (RT) to the primary and regional disease in combination with systemic chemotherapy and local treatment to metastatic foci in patients with newly diagnosed metastatic nasopharyngeal carcinoma (NPC). Methods: One hundred and five consecutive patients with pathologically confirmed NPC with distant metastasis at diagnosis seen between 1995 and 2002 were reviewed. All were offered cisplatin-based chemotherapy, high-dose RT (>30 Gy) to the head and neck region, and active treatment to the metastatic foci. Results: Patients' median age was 46 years, and all had a Karnofsky Performance Score of >= 70. Eighty-nine patients (85%) had metastases confined to 1 organ. Ninety-six patients (91%) received at least 1 cycle of chemotherapy and 71 (68%) received greater than 65 Gy of radiation to the head and neck region. With a median follow-up time of 22 months (range: 2 to 142 mo), 90 patients had deceased, and the median survival time of the entire group was 25 months. The 2 and 5-year estimated overall survival rates were 50% and 17%, respectively. Radiation dose of greater than 65 Gy to the primary region (P = 0.05) and number of organs with metastases (single vs. multiple) (P = 0.002) were independent predictive factors for overall survival on log-rank tests. Only moderately severe acute toxicities, such as Radiation Therapy Oncology Group grade 3 mucositis, skin desquamation, and leukocytopenia were observed. No patient experienced grade 4 acute toxicities. Conclusions: High-dose RT is indicated for local disease control in patients with metastatic NPC, and may improve survival when actively used with systemic chemotherapy and local treatment for metastatic foci. Patients with single-organ metastases have a better prognosis as compared with those with more widespread metastases.

Regional collaboration to improve quality of radiation therapy in Asia

Summary: The delivery of high-quality cancer treatment in Asia, especially in terms of radiation therapy, faces challenges such as shortage of treatment centres and outdated technology. The Federation of Asian Organizations for Radiation Oncology (FARO) was established to address these issues through regional collaboration, aiming to improve treatment quality collectively. This organization has already achieved some early successes in fact-finding and educational activities, and looks towards future possibilities for strategic proposals that could benefit the radiation therapy community and patients in Asia.

Pretreatment 18F-FDG uptake heterogeneity can predict treatment outcome of carbon ion radiotherapy in patients with locally recurrent nasopharyngeal carcinoma

Results showed that F-18-FDG uptake heterogeneity, particularly with a HI>=0.81, was significantly associated with poorer LRFS and PFS outcomes, indicating its potential as an independent predictor for LRNPC patients undergoing CIRT.

Association of IDH mutation and 1p19q co-deletion with tumor immune microenvironment in lower-grade glioma

Summary: While successful clinical trials of immunotherapy have shown promising strategies for many cancers, its application in glioma has lagged behind compared to other cancers. This study focused on the molecular alterations affecting therapeutic response in lower-grade glioma (LGG) and characterized the immunological phenotypes with different molecular subtypes to improve our understanding and application of immunotherapies in LGG. The research collected data from 1,052 patients, stratified them into three genetic subgroups, and discovered distinct immunological tumor microenvironments associated with IDH mutations and 1p/19q codeletions in LGG, ultimately leading to the construction of an immune-related prognostic signature that predicts overall survival time in LGG.

Recognizing the importance of molecular networks in overcoming drug resistance can provide a rational basis for improving cancer treatment with safer and more effective combined therapeutic strategies.

Definitive carbon ion radiotherapy for tracheobronchial adenoid cystic carcinoma: a preliminary report

Summary: The study evaluated the effects and toxicities of carbon ion radiotherapy in the management of tracheobronchial adenoid cystic carcinoma. Among 18 eligible patients, the 2-year overall survival, local control, and progression-free survival rates were 100%, 100%, and 61.4%, respectively, with an overall response rate of 88.2%. Only one patient experienced severe tracheal stenosis after CIRT, which was relieved after stent implantation.

VMP1, a novel prognostic biomarker, contributes to glioma development by regulating autophagy

Mixed Photon and Carbon-Ion Beam Radiotherapy in the Management of Non-Metastatic Nasopharyngeal Carcinoma

Adaptive carbon ion radiotherapy for locally advanced non-small cell lung cancer: Organ-sparing potential and target coverage

Summary: This study demonstrates the dosimetric benefits of adaptive carbon ion radiotherapy (CIRT) for locally advanced non-small cell lung cancer (LANSCLC) and compares the differences between patients with and without adaptive plans. The results show that adaptive plans can ensure target coverage and reduce spinal cord dose. Patients with adaptive plans have larger tumor volumes and greater volume changes. iGTV volume changes and iGTV center movements are the main factors influencing adaptive planning.

Mesenchymal Stem Cells Inhibits Migration and Vasculogenic Mimicry in Nasopharyngeal Carcinoma Via Exosomal MiR-125a

Additionally, artificial engineering of mesenchymal stem cells (MSCs) allowed the generation of miR-125a-overexpressing exosomes which attenuated VM formation and migration in nasopharyngeal carcinoma, providing a potential therapeutic approach.

The Majority of United States Citizens With Cancer do not Have Access to Carbon Ion Radiotherapy

The absence of CIRT facilities in the U.S. limits access to this treatment for American cancer patients and prevents their inclusion in phase III clinical trials, which are essential for FDA approval and insurance coverage. Overcoming this barrier would require establishing a limited number of CIRT facilities in the U.S.

A Systematic Review of Proton Therapy for the Management of Nasopharyngeal Cancer
