Markers of Inflammation and Cardiovascular Disease Clinical Applications of C-Reactive Protein Determination

C-reactive protein (CRP) is produced by the macrophages in the liver and adipocytes and is integrated in the acute-phase response pathway. Being a nonspecific marker of inflammation, it increases in response to inflammation. The results of recent studies that have analyzed the role of CRP have not yet influenced current clinical practice. When used in combination with other established biomarkers for the prediction of the first major cardiovascular event or death, CRP does not improve the risk stratification obtained with the current guidelines. The reduction of CRY levels itself or as a statin-related pleiotropic effect has been assessed in different scenarios, including the acute phase of myocardial infarction; secondary prevention of cardiovascular diseases; special patient populations, such as diabetic patients; and finally in a primary prevention study (JUPITER [Justification for the Use of statins in primary Prevention: an Intervention Trial Evaluating Rosuvastatin]). Risk stratification in all the examined scenarios was related to serum LDL-C levels; in other words, the degree of cardiovascular risk was always lipid dependent.

Summary: This study assessed the cost-effectiveness and cost-utility ratios of restrictive vs. liberal transfusion strategy in acute myocardial infarction patients. The results showed that the restrictive strategy was dominant at 30 days, saving costs and improving outcomes, but it no longer maintained non-inferiority on MACE at 1 year.

Alpha-cardiac Actin Serum Expression Levels Detect Acute Cellular Rejection in Heart Transplant Patients

Summary: In this study, we found differential expression of 17 sarcomeric genes associated with acute cellular rejection (ACR) using RNA sequencing analysis of serum samples from transplant recipients. Among them, the gene encoding alpha-cardiac actin (ACTC1) showed the most significant results with high diagnostic capacity. We further confirmed the overexpression of ACTC1 protein and its significant diagnostic potential for clinically relevant rejection. Therefore, we suggest incorporating ACTC1 serum expression levels into the development of a molecular panel test for noninvasive ACR detection.

Effects of guideline-directed medical therapy in patients with left bundle branch block-induced cardiomyopathy

Summary: This study aimed to evaluate the impact of guideline-directed medical therapy on left ventricular ejection fraction (LVEF) and functional class in patients with LBBB-induced cardiomyopathy. The results showed that medical therapy did not significantly improve LVEF and functional class, while cardiac resynchronization therapy device implantation led to a significant improvement in LVEF.

Summary: There are similarities and dissimilarities in AF management and clinical outcomes between DACH and ORW regions. The increased use of NOACs in DACH is associated with a mismatch in risk-adapted anticoagulation. Suboptimal control of INR is observed in both regions. DACH region has higher rates of cardiovascular death and lower rates of non-haemorrhagic stroke.

Summary: This study aimed to select potential patients for bempedoic acid treatment and evaluate its cost-effectiveness. Further research is needed to assess the cost-effectiveness of bempedoic acid in different scenarios.

The impact of inter-clinician electronic consultation in patients diagnosed with atrial fibrillation in primary care

Summary: This study found that an inter-clinician electronic consultation program based on shared electronic patient dossiers significantly reduced the time for cardiology consultations and initiation of oral anticoagulants. The implementation of this program was also associated with a lower risk of stroke and cardiovascular/all-cause mortality.

Coronary Artery Disease and Prognosis of Heart Failure with Reduced Ejection Fraction

Summary: The aim of this study was to determine the prognostic impact of coronary artery disease (CAD) on heart failure with reduced ejection fraction (HFrEF) mortality and readmissions. A total of 583 patients with left ventricular ejection fraction <40% were included in the study, among which 266 had CAD and 137 had idiopathic dilated cardiomyopathy (DCM). The study found that one-year mortality and readmissions were similar between patients with CAD and idiopathic DCM, but patients with idiopathic DCM were more likely to receive a heart transplant.

Summary: The study aimed to investigate the main predictors for post-operative atrial fibrillation (POAF) in patients undergoing cardiac surgery. Clinical variables, plasma, and biological tissues were analyzed before surgery. The main predictors for POAF were cardiopulmonary bypass time and plasma pre-operative orosomucoid levels. Orosomucoid was found to be the best predictor for POAF in women but not in men, supporting the role of pre-operative inflammation pathway in the risk of POAF, mainly in women.

Summary: This study aims to determine the predictive value of the sST2 biomarker in atrial fibrillation (AF) recurrence. The results showed that sST2 values at 3 months may provide utility in predicting AF recurrence at 6 months in patients who underwent electrical cardioversion (ECV), but not in patients who underwent pulmonary vein isolation (PVI).

Enhanced Levels of Adiposity, Stretch and Fibrosis Markers in Patients with Coexistent Heart Failure and Atrial Fibrillation

Summary: The coexistence of heart failure (HF) and atrial fibrillation (AF) worsens patients' prognosis. In this study, we investigated the levels of inflammation, metabolism, adiposity, and fibrosis markers in epicardial and subcutaneous fat and blood, and their relationship with HF and AF. Our findings showed that higher levels of adiposity markers were associated with AF, while higher levels of stretch markers were correlated with HF. Patients with both AF and HF had higher levels of ANP and GDF-15. After excluding AF patients, plasma FABP4 was identified as the main predictor of HF. Fat biopsies from AF patients demonstrated an enhanced inflammatory profile. These results suggest the involvement of the adiposity-fibrosis pathway in the coexistence of HF and AF.

Cost Analysis of Magnetic Resonance Imaging and Computed Tomography in Cardiology: A Case Study of a University Hospital Complex in the Euro Region

Summary: In recent years, hospitals have started directly managing MRI equipment in their cardiology departments. The aim of this study was to determine the total cost per test for CT and MRI in a tertiary hospital's Cardiology Department. The process involved three phases: identifying testing process phases, identifying resources consumed, and quantifying and assessing inputs. The results showed that MRI had higher personnel and equipment costs, while CT had higher consumables and overhead costs. The total cost per test was higher for MRI. In conclusion, the unit cost for each CT and MRI test was EUR 177.73 and EUR 180.60, respectively, due to consumables for CT and equipment and staff time for MRI.

Finerenone: Towards a holistic therapeutic approach to patients with diabetic kidney disease

Finerenone, a nonsteroidal highly selective mineralocorticoid antagonist, may directly reduce inflammation and fibrosis, and has shown promising results in reducing albuminuria and slowing CKD progression in persons with diabetes.
