Diagnostic Performance of Computed Tomography Coronary Angiography (from the Prospective National Multicenter Multivendor EVASCAN Study)

Computed tomographic coronary angiography (CTCA) has been proposed as a noninvasive test for significant coronary artery disease (CAD), but only limited data are available from prospective multicenter trials. The goal of this study was to establish the diagnostic accuracy of CTCA compared to coronary angiography (CA) in a large population of symptomatic patients with clinical indications for coronary imaging. This national, multicenter study was designed to prospectively evaluate stable patients able to undergo CTCA followed by conventional CA. Data from CTCA and CA were analyzed in a blinded fashion at central core laboratories. The main outcome was the evaluation of patient-, vessel-, and segment-based diagnostic performance of CTCA to detect or rule out significant CAD (>= 50% luminal diameter reduction). Of 757 patients enrolled, 746 (mean age 61 12 years, 71% men) were analyzed. They underwent CTCA followed by CA 1.7 +/- 0.8 days later using a 64-detector scanner. The prevalence of significant CAD in native coronary vessels by CA was 54%. The rate of nonassessable segments by CTCA was 6%. In a patient-based analysis, sensitivity, specificity, positive and negative predictive values, and positive and negative likelihood ratios of CTCA were 91%, 50%, 68%, 83%, 1.82, and 0.18, respectively. The strongest predictors of false-negative results on CTCA were high estimated pretest probability of CAD (odds ratio [OR] 1.97, p <0.001), male gender (OR 1.5, p <0.002), diabetes (OR 1.5, p <0.0001), and age (OR 1.2, p <0.0001). In conclusion, in this large multicenter study, CTCA identified significant CAD with high sensitivity. However, in routine clinical practice, each patient should be individually evaluated, and the pretest probability of obstructive CAD should be taken into account when deciding which method, CTCA or CA, to use to diagnose its presence and severity. Stent Size Optimization in the Femoral Bifurcation Using a Fractal Model: A Morphological Analysis

Comparison of the damage to aorta wall in aortitis versus noninflammatory degenerative aortic diseases

Summary: Transcatheter aortic valve implantation (TAVI) is an effective treatment for older patients with symptomatic severe aortic stenosis, and its indication has expanded to younger patients at lower risk. Despite a decrease in thromboembolic and bleeding complications after TAVI, these adverse events are still common. Recommendations for antithrombotic therapy in patients undergoing TAVI are mainly based on expert opinion, while recent studies and trials provide updated insights on antithrombotic treatment during and after the procedure.

Summary: The study aimed to compare characteristics of patients with renal infarction related to nontrauma renal artery dissection with or without underlying vascular disease, and report long-term renal and vascular outcomes. The results showed a similar frequency of new-onset RAD in patients with aneurysmal multisite arterial disease and fibromuscular dysplasia, but a higher rate of de novo extrarenal dissection in the former group.

Summary: The aim of this study was to assess the ability of phase-contrast (PC)-MRI to detect subclinical left and right ventricular diastolic dysfunction in systemic sclerosis (SSc) patients. The results showed that MRI was able to accurately detect diastolic dysfunction in SSc patients without left ventricular systolic dysfunction at echocardiography.

Left ventricular diastolic early and late filling quantified from 4D flow magnetic resonance imaging

Summary: This study investigated the value of 4D flow MRI in measuring mitral filling and its correlation with TTE and 2D flow MRI. The results showed good agreement between 4D flow MRI and the reference methods in measuring flow rates and E/A ratios. The measurements using 4D flow MRI were reproducible and correlated with age and LV remodeling.

Can GRACE Risk Score Predict Mortality and the Need for Thrombolytic Treatment in Acute Pulmonary Embolism?

Summary: This study aimed to evaluate the predictive value of the GRACE risk score in acute pulmonary embolism (PE) patients for 30-day mortality and the need for thrombolytic treatment. The study found that the GRACE risk score, along with the PESI score, were independent risk factors associated with 30-day mortality and the need for thrombolytic treatment. The GRACE risk score showed high sensitivity and specificity in predicting mortality, and its combination with the PESI score helped define high-risk PE patients and predict poor prognosis.

Summary: Transthoracic echocardiography (TTE) is commonly used to evaluate isolated tricuspid regurgitation (TR), but there is limited research comparing its quantification with magnetic resonance imaging (MRI). In this study, a novel TTE algorithm was developed to identify severe TR based on TTE parameters. The algorithm had a higher accuracy than the current guidelines' criteria in detecting severe TR by MRI.

Coronary Angiography and Chronic Total Occlusion Angioplasty in a Patient With Situs Inversus from the Right Radial Approach

Pericardial Adipose Tissue Thrombospondin-1 Associates With Antiangiogenesis in Ischemic Heart Disease

Summary: This study evaluated the angiogenic capacity of adipose tissue in patients undergoing cardiac surgeries and found that patients with coronary artery disease (CAD) had reduced angiogenic capacity in their pericardial adipose tissue. Gene expression analysis revealed upregulated expression of the anti-angiogenic gene TSP-1 in CAD patients, with no significant differences in other angiogenic factors. Additionally, inhibiting TSP-1 expression significantly improved angiogenic deficiency in CAD patients.

Evaluation of MADIT-II Risk Stratification Score Among Nationwide Registry of Heart Failure Patients With Primary Prevention Implantable Cardiac Defibrillators or Resynchronization Therapy Devices

Summary: The current guidelines recommend prophylactic implantable cardioverter-defibrillator (ICD) for patients with symptomatic heart failure (HF) and low left ventricular ejection fraction. However, not all patients will benefit from ICD treatment. This study evaluated the feasibility of using the MADIT-II-based Risk Stratification Score (MRSS) to predict the survival benefit of prophylactic ICDs in patients with HF. The results showed that different risk subgroups had varying levels of ICD survival benefit.
