Relation Between C-Reactive Protein, Homocysteine Levels, Fibrinogen, and Lipoprotein Levels and Leukocyte and Platelet Counts, and 10-Year Risk for Cardiovascular Disease Among Healthy Adults in the USA

The association between systemic inflammation and the estimated 10-year risk for coronary artery disease (CAD) according to the Framingham risk score is largely unknown. In this study, 6,371 participants in the Third National Health and Nutrition Examination Survey (NHANES III) aged 40 to 79 years, who had no histories of heart attack, stroke, peripheral artery disease, or diabetes mellitus, were categorized into groups at low (<10%), intermediate (10% to 20%), and high (>20%) risk according to 10-year risk for CAD, calculated using the Framingham risk score modified by the National Cholesterol Education Program Adult Treatment Panel III. After adjustments for age, gender, race, body mass index, and co-morbidities, participants at high risk were more likely to have elevated circulating C-reactive protein levels (>= 2.2 mg/L: adjusted odds ratio [OR] 1.61, 95% confidence interval [CI] 1.30 to 2.01, p < 0.0001; >10.0 mg/L: OR 1.41, 95% CI 1.03 to 1.93, p = 0.034). The high-risk group had circulating fibrinogen, homocysteine, leukocyte, and platelet levels that were 20.98 mg/dl (95% CI 12.53 to 29.43, p < 0.0001), 1.54 mu mol/L (95% CI 0.76 to 2.32, p = 0.002), 0.90 mu mol/L (95% CI 0.36 to 1.43, p = 0.001), 910/mu l (95% CI 670 to 1,160, p < 0.0001), and 10,220/mu l (95% CI 2,830 to 17,610, p < 0.0001) higher, respectively, than in those in the low-risk group. There was also a dose-dependent increase in circulating levels of inflammatory markers across the categories of CAD risk. In conclusion, these findings indicate that low-grade systemic inflammation and hyperhomocysteinemia were present in participants with high 10-year risk for CAD. (C) 2010 Elsevier Inc. Central blood pressure lowering effect of telmisartan-rosuvastatin single-pill combination in hypertensive patients combined with dyslipidemia: A pilot study

Summary: This study aimed to evaluate the safety and efficacy of telmisartan/rosuvastatin single-pill combination therapy in reducing central blood pressure compared to telmisartan monotherapy. The results showed that the combination therapy was more effective in lowering central blood pressure, indicating the potential benefit of adding statin therapy for hypertensive patients with dyslipidemia. Both groups showed high compliance and no significant difference in adverse events.

Effect of dipeptidyl peptidase-4 inhibitor on the progression of coronary artery disease evaluated by computed tomography in patients receiving insulin therapy for type 2 diabetes mellitus

Summary: This study evaluated the effect of a dipeptidyl peptidase-4 inhibitor (DPP-4i) on obstructive coronary artery disease (OCAD) progression in patients with type 2 diabetes mellitus (T2DM) receiving insulin therapy. The results showed that the use of DPP-4i was associated with a lower incidence of newly developed OCAD compared to no DPP-4i use. Therefore, adding DPP-4i therapy to insulin treatment in T2DM patients can effectively prevent coronary artery disease progression.

Thus, improving adherence to antihypertensive medication is crucial in cancer patients.

Screening for secondary causes and emphasizing drug adherence through methods such as home blood pressure monitoring are important in managing resistant hypertension.

Standardized protocol of blood pressure measurement and quality control program for the Korea National Health and Nutrition Examination Survey

Summary: This article describes the blood pressure measurement protocol and quality control program in the Korea National Health and Nutrition Examination Survey (KNHANES). KNHANES transitioned to mercury-free auscultatory devices for blood pressure measurement in 2020 and will adopt Microlife as the unified device from 2023. Under the new protocol, blood pressure measurements are conducted three times with specific conditions, and the average of the second and third readings is used as the representative value. The quality control program ensures the competence of examiners and involves weekly calibration and on-site evaluations.

Can GRACE Risk Score Predict Mortality and the Need for Thrombolytic Treatment in Acute Pulmonary Embolism?

Summary: This study aimed to evaluate the predictive value of the GRACE risk score in acute pulmonary embolism (PE) patients for 30-day mortality and the need for thrombolytic treatment. The study found that the GRACE risk score, along with the PESI score, were independent risk factors associated with 30-day mortality and the need for thrombolytic treatment. The GRACE risk score showed high sensitivity and specificity in predicting mortality, and its combination with the PESI score helped define high-risk PE patients and predict poor prognosis.

Summary: Transthoracic echocardiography (TTE) is commonly used to evaluate isolated tricuspid regurgitation (TR), but there is limited research comparing its quantification with magnetic resonance imaging (MRI). In this study, a novel TTE algorithm was developed to identify severe TR based on TTE parameters. The algorithm had a higher accuracy than the current guidelines' criteria in detecting severe TR by MRI.

Coronary Angiography and Chronic Total Occlusion Angioplasty in a Patient With Situs Inversus from the Right Radial Approach

Pericardial Adipose Tissue Thrombospondin-1 Associates With Antiangiogenesis in Ischemic Heart Disease

Summary: This study evaluated the angiogenic capacity of adipose tissue in patients undergoing cardiac surgeries and found that patients with coronary artery disease (CAD) had reduced angiogenic capacity in their pericardial adipose tissue. Gene expression analysis revealed upregulated expression of the anti-angiogenic gene TSP-1 in CAD patients, with no significant differences in other angiogenic factors. Additionally, inhibiting TSP-1 expression significantly improved angiogenic deficiency in CAD patients.

Evaluation of MADIT-II Risk Stratification Score Among Nationwide Registry of Heart Failure Patients With Primary Prevention Implantable Cardiac Defibrillators or Resynchronization Therapy Devices

Summary: The current guidelines recommend prophylactic implantable cardioverter-defibrillator (ICD) for patients with symptomatic heart failure (HF) and low left ventricular ejection fraction. However, not all patients will benefit from ICD treatment. This study evaluated the feasibility of using the MADIT-II-based Risk Stratification Score (MRSS) to predict the survival benefit of prophylactic ICDs in patients with HF. The results showed that different risk subgroups had varying levels of ICD survival benefit.
