However, it is concerning that even in a high resource setting, almost half of ACHD patients are not receiving regular cardiac care. Summary: This study aimed to describe the experiences and challenges faced by adolescents with moderate and severe congenital heart defects (CHD) or Chronic Rheumatic Heart Disease (CRHD) in order to develop an Adolescent Transition Psychoeducational Program. Qualitative data analysis identified five main themes and eleven subcategories related to the experiences and challenges of these adolescents, with suggestions for improving the program focused on self-management of illness and social support. Participants expressed support for the program and willingness to participate if their schedule allowed.

Summary: The epidemiology of congenital heart disease (CHD) has changed in the past 50 years due to the increasing prevalence and survival rate of CHD. The mortality in CHD patients has dramatically changed since the latter half of the twentieth century as a result of timely diagnosis and intervention development. As CHD patients age, the burden of the disease shifts from the heart to acquired cardiovascular and systemic complications. The societal costs of CHD are high, both in terms of healthcare utilization and quality of life.

Summary: This study aimed to evaluate the effects of preterm birth on cardiac structure and function as well as transplant-free survival in infants with hypoplastic left heart syndrome and associated anomalies. The results, obtained from the Single Ventricle Reconstruction trial, showed that preterm-born infants had lower echocardiographic measures at birth but higher measures at 14 months compared to term-born infants. Preterm-born infants had an increased risk of death or heart transplantation from birth to age 6, with a higher risk as gestational age decreased below 37 weeks. Approximately 27.3% of the total effect of preterm birth on transplant-free survival was mediated through birth weight.

Summary: In a sheep model with pathophysiologic changes similar to repaired TOF patients, severe pulmonary regurgitation (PR) leads to fibrotic changes in the right ventricle (RV). Pulmonary valve replacement reverses these fibrotic changes. Early valve replacement results in quick RV recovery, and timing of the surgery does not affect the outcome. The fibrotic changes correlate well with the circulating biomarker PICP, which can be useful in clinical follow-up.

The results reveal clinically meaningful pathways linking identity formation to psychosocial functioning over time. Bidirectional effects were found for general functioning, while mainly unidirectional effects were found for cancer-specific functioning.

Summary: The rates of successful surgical repair and life expectancy for patients with congenital heart disease have significantly increased in recent decades. However, exposure to cardiovascular risk factors has led to a shift in the clinical trajectory of adult congenital heart disease, with an increasing burden of acquired cardiovascular disease, primarily atherosclerotic cardiovascular disease (ASCVD). Currently, comprehensive guidance for the prevention and management of acquired heart disease in adult congenital heart disease patients is lacking. This document provides an overview of acquired heart disease and offers guidance on ASCVD prevention for both specialists and non-specialists in adult congenital heart disease, aiming to reduce the cardiovascular burden in this growing population.

Summary: Clinical exome sequencing (ES) was found to have a diagnostic efficacy rate of 16.3% in individuals with non-isolated anomalous pulmonary venous return (APVR). Three novel genes (EFTUD2, NAA15, and NKX2-1) and three recurrent copy number variants (1q21.1 microdeletions, 15q11.2 deletions, and 22q11.2 deletions) were identified as being associated with APVR. Genetic testing through ES and chromosomal microarray analysis (or genome sequencing) should be considered for individuals with non-isolated APVR without an identified genetic cause, but not for individuals with EFTUD2-, NAA15-, and NKX2-1-related disorders.

Summary: The study aimed to investigate the impact of type 1 diabetes (T1D) on how youth perceive parents and peers. Propensity weighting was used to minimize limitations of previous observational studies. Questionnaires were completed by 558 youth with T1D and 426 control youth, revealing that youth with T1D perceived more overprotective mothers, less friend support, and less extreme peer orientation. However, there were no significant group differences in paternal overprotection, and parental responsiveness and psychological control.

Summary: This study aimed to evaluate the predictive value of the GRACE risk score in acute pulmonary embolism (PE) patients for 30-day mortality and the need for thrombolytic treatment. The study found that the GRACE risk score, along with the PESI score, were independent risk factors associated with 30-day mortality and the need for thrombolytic treatment. The GRACE risk score showed high sensitivity and specificity in predicting mortality, and its combination with the PESI score helped define high-risk PE patients and predict poor prognosis.

Summary: Transthoracic echocardiography (TTE) is commonly used to evaluate isolated tricuspid regurgitation (TR), but there is limited research comparing its quantification with magnetic resonance imaging (MRI). In this study, a novel TTE algorithm was developed to identify severe TR based on TTE parameters. The algorithm had a higher accuracy than the current guidelines' criteria in detecting severe TR by MRI.

Situs inversus is a rare congenital abnormality characterized by the inverted position of chest and abdominal organs. Cardiac catheterization is uncommon in patients with this particular abnormality, highlighting the importance of customizing techniques to engage coronary arteries and optimize guide support for percutaneous coronary intervention if needed.

Summary: This study evaluated the angiogenic capacity of adipose tissue in patients undergoing cardiac surgeries and found that patients with coronary artery disease (CAD) had reduced angiogenic capacity in their pericardial adipose tissue. Gene expression analysis revealed upregulated expression of the anti-angiogenic gene TSP-1 in CAD patients, with no significant differences in other angiogenic factors. Additionally, inhibiting TSP-1 expression significantly improved angiogenic deficiency in CAD patients.

Summary: The current guidelines recommend prophylactic implantable cardioverter-defibrillator (ICD) for patients with symptomatic heart failure (HF) and low left ventricular ejection fraction. However, not all patients will benefit from ICD treatment. This study evaluated the feasibility of using the MADIT-II-based Risk Stratification Score (MRSS) to predict the survival benefit of prophylactic ICDs in patients with HF. The results showed that different risk subgroups had varying levels of ICD survival benefit.

The results showed that the younger, female, non-Hispanic Black or Hispanic individuals with a high burden of co-morbidities and unfavorable social determinants of health had the highest risk of mortality in the identified latent classes.

Summary: This study aimed to examine the relation between reverse septal curvature (RSC) and nonsustained ventricular tachycardia (NSVT) in patients with hypertrophic cardiomyopathy (HCM). The study found that patients with RSC were younger and had a higher incidence of NSVT, and RSC was identified as a strong independent risk factor for NSVT.
