Adolescents' Understanding of Their Congenital Heart Disease on Transfer to Adult-Focused Care

Adolescents with congenital heart disease (CHD) must take responsibility for their life and care. This requires that they have sufficient knowledge about their heart disease, treatment, and preventive measures. Thus, CHD-related education should be directed to adolescents. Research on adolescents' understanding and knowledge of CHD is limited. It is unknown what adolescents with CHD know about their heart defect, treatment, and preventive measures necessary to avoid complications. We addressed these questions in a descriptive cross-sectional study of 91 adolescents with CHD (53% males; median age 17 years). In the present study, we assessed the subjects' knowledge of CHD using the Leuven Knowledge Questionnaire for Congenital Heart Disease. The results showed that the patients had adequate knowledge (>80% correct answers) about the need for regular follow-up, their required diet, past treatment, and dental practices. They had moderate knowledge (50% to 80% correct answers) about the frequency of follow-up, occupational choices, medication regimen, and sexual activities. However, the patients had poor knowledge (<50% correct answers) of the name of their heart defect; the reasons for follow-up; the effects of competitive sports; the symptoms that reflect deterioration of their heart disease; the definition, characteristics, and risk factors of endocarditis; the possibility of recurrent episodes of endocarditis during their lifetime; the effect of smoking and alcohol on their heart disease; the hereditary nature of their condition; the suitability of intrauterine devices as contraceptives; the appropriateness of oral contraceptives; and the risks of pregnancy. In conclusion, the results of the present study have showed that the level of knowledge of adolescents with CHD has significant gaps. (C) 2010 Elsevier Inc. All rights reserved. Summary: The epidemiology of congenital heart disease (CHD) has changed in the past 50 years due to the increasing prevalence and survival rate of CHD. The mortality in CHD patients has dramatically changed since the latter half of the twentieth century as a result of timely diagnosis and intervention development. As CHD patients age, the burden of the disease shifts from the heart to acquired cardiovascular and systemic complications. The societal costs of CHD are high, both in terms of healthcare utilization and quality of life.

Short-term Prognostic Value of Heart Failure Diagnosis in a Contemporary Cohort of Patients With Adult Congenital Heart Disease

HF diagnosis appears to be a valuable prognostic marker in addition to the ACHD-AP classification.

Preterm Birth Is Associated With Adverse Cardiac Remodeling and Worse Outcomes in Patients With a Functional Single Right Ventricle

Summary: This study aimed to evaluate the effects of preterm birth on cardiac structure and function as well as transplant-free survival in infants with hypoplastic left heart syndrome and associated anomalies. The results, obtained from the Single Ventricle Reconstruction trial, showed that preterm-born infants had lower echocardiographic measures at birth but higher measures at 14 months compared to term-born infants. Preterm-born infants had an increased risk of death or heart transplantation from birth to age 6, with a higher risk as gestational age decreased below 37 weeks. Approximately 27.3% of the total effect of preterm birth on transplant-free survival was mediated through birth weight.

Reversal of Right Ventricular Remodeling After Correction of Pulmonary Regurgitation in Tetralogy of Fallot

Summary: In a sheep model with pathophysiologic changes similar to repaired TOF patients, severe pulmonary regurgitation (PR) leads to fibrotic changes in the right ventricle (RV). Pulmonary valve replacement reverses these fibrotic changes. Early valve replacement results in quick RV recovery, and timing of the surgery does not affect the outcome. The fibrotic changes correlate well with the circulating biomarker PICP, which can be useful in clinical follow-up.

Summary: This study examines the longitudinal associations between identity formation and general and cancer-specific functioning in adolescent and emerging adult childhood cancer survivors. The results reveal clinically meaningful pathways linking identity formation to psychosocial functioning over time. Bidirectional effects were found for general functioning, while mainly unidirectional effects were found for cancer-specific functioning.

Summary: The rates of successful surgical repair and life expectancy for patients with congenital heart disease have significantly increased in recent decades. However, exposure to cardiovascular risk factors has led to a shift in the clinical trajectory of adult congenital heart disease, with an increasing burden of acquired cardiovascular disease, primarily atherosclerotic cardiovascular disease (ASCVD). Currently, comprehensive guidance for the prevention and management of acquired heart disease in adult congenital heart disease patients is lacking. This document provides an overview of acquired heart disease and offers guidance on ASCVD prevention for both specialists and non-specialists in adult congenital heart disease, aiming to reduce the cardiovascular burden in this growing population.

Summary: Clinical exome sequencing (ES) was found to have a diagnostic efficacy rate of 16.3% in individuals with non-isolated anomalous pulmonary venous return (APVR). Three novel genes (EFTUD2, NAA15, and NKX2-1) and three recurrent copy number variants (1q21.1 microdeletions, 15q11.2 deletions, and 22q11.2 deletions) were identified as being associated with APVR. Genetic testing through ES and chromosomal microarray analysis (or genome sequencing) should be considered for individuals with non-isolated APVR without an identified genetic cause, but not for individuals with EFTUD2-, NAA15-, and NKX2-1-related disorders.

Comparing youth with and without type 1 diabetes on perceived parenting and peer functioning: a propensity weighting approach

Summary: The study aimed to investigate the impact of type 1 diabetes (T1D) on how youth perceive parents and peers. Propensity weighting was used to minimize limitations of previous observational studies. Questionnaires were completed by 558 youth with T1D and 426 control youth, revealing that youth with T1D perceived more overprotective mothers, less friend support, and less extreme peer orientation. However, there were no significant group differences in paternal overprotection, and parental responsiveness and psychological control.

Preferences for disease-related information and transitional skills among adolescents with congenital heart disease in the early transitional stage

Can GRACE Risk Score Predict Mortality and the Need for Thrombolytic Treatment in Acute Pulmonary Embolism?

Summary: This study aimed to evaluate the predictive value of the GRACE risk score in acute pulmonary embolism (PE) patients for 30-day mortality and the need for thrombolytic treatment. The study found that the GRACE risk score, along with the PESI score, were independent risk factors associated with 30-day mortality and the need for thrombolytic treatment. The GRACE risk score showed high sensitivity and specificity in predicting mortality, and its combination with the PESI score helped define high-risk PE patients and predict poor prognosis.

Pericardial Adipose Tissue Thrombospondin-1 Associates With Antiangiogenesis in Ischemic Heart Disease

Summary: This study evaluated the angiogenic capacity of adipose tissue in patients undergoing cardiac surgeries and found that patients with coronary artery disease (CAD) had reduced angiogenic capacity in their pericardial adipose tissue. Gene expression analysis revealed upregulated expression of the anti-angiogenic gene TSP-1 in CAD patients, with no significant differences in other angiogenic factors. Additionally, inhibiting TSP-1 expression significantly improved angiogenic deficiency in CAD patients.

Summary: The current guidelines recommend prophylactic implantable cardioverter-defibrillator (ICD) for patients with symptomatic heart failure (HF) and low left ventricular ejection fraction. However, not all patients will benefit from ICD treatment. This study evaluated the feasibility of using the MADIT-II-based Risk Stratification Score (MRSS) to predict the survival benefit of prophylactic ICDs in patients with HF. The results showed that different risk subgroups had varying levels of ICD survival benefit.
