Relation of Serum 25-Hydroxyvitamin D to Heart Rate and Cardiac Work (from the National Health and Nutrition Examination Surveys)

Vitamin D may protect against cardiovascular disease, but its association with cardiac function is unclear. The aim of this study was to examine the associations of serum 25-hydroxyvitamin D (25[OH]D) with heart rate, systolic blood pressure, and the rate pressure product (RPP). Data analyses were carried out on 27,153 participants aged >= 20 years, with measurements of serum 25(OH)D, heart rate (from radial pulse), and systolic blood pressure, in the National Health and Nutrition, Examination Surveys (NHANES) carried out from 1988 to 1994 and from 2001 to 2006. RPP was calculated as heart rate times systolic blood pressure. Results were adjusted for age, gender, race or ethnicity, body mass index, physical activity, tobacco smoking, co-morbidities, and blood pressure treatment. Compared to participants with 25(OH)D >= 35 ng/ml, the adjusted mean +/- SE heart rate was significantly (p <0.001) higher, by 2.1 +/- 0.6 beats/min, in participants with 25(OH)D <10.0 ng/ml, while mean systolic blood pressure was 1.9 +/- 0.8 mm Hg higher (p <0.05) for participants with 25(OH)D <10.0 ng/ml and 1.7 +/- 0.6 mm Hg higher (p <0.01) for those with 25(OH)D of 10.0 to 14.9 ng/ml. As a consequence, adjusted mean RPP was 408 +/- 110 beats/min . mm Hg higher (p <0.001) for participants with 25(OH)D <10.0 ng/ml and 245 +/- 80 beats/min . mm Hg higher (p <0.01) for participants with 25(OH)D of 10.0 to 14.9 ng/ml, compared to those with 25(OH)D 35 ng/ml. In conclusion, these results show that low serum 25(OH)D levels are associated with increased heart rate, systolic blood pressure, and RPP and suggest that low vitamin D status may increase cardiac work. The survey results showed that among the EDs receiving pediatric telehealth, there was a lack of pediatric emergency medicine physicians and pediatricians available, and most EDs used pediatric telehealth services infrequently.

Characteristics of Pediatric Patient Transfers From General Emergency Departments in California From 2005 to 2018

Summary: During the COVID-19 pandemic, pregnant women experienced similar levels of prenatal depression and perceived stress compared to before the pandemic. However, gestational age at birth was shorter and depression was associated with shortened gestation. Birth weights did not differ significantly, but infants born during the pandemic had slightly larger birth weights for gestational age. More women who gave birth early in the pandemic reported being moderately or extremely distressed about changes in prenatal care and delivery. The majority reported negative views of the impact of COVID-19 on their life.

Summary: This study aimed to investigate the association between exclusive breastfeeding during ages 0-2.9 months and bronchiolitis hospitalization in infants. The results showed a significant negative correlation between exclusive breastfeeding and bronchiolitis hospitalization. Exclusive breastfeeding can significantly reduce the risk of bronchiolitis hospitalization in infants.

Summary: Using tIgE and virus data, we identified phenotypes of infant bronchiolitis and studied their association with asthma development and biological characteristics. We found 4 phenotypes, and infants with the tIgE(high)virus(RSV-low/RV) phenotype had a significantly higher risk for developing asthma. These phenotypes also exhibited different biological characteristics, such as depleted type I interferon and enriched antigen presentation pathways.

Epinephrine treatment of food-induced and other cause anaphylaxis in United States and Canadian Emergency Departments: a systematic review and meta-analysis

Summary: This study investigated whether epinephrine use for food-induced and other cause anaphylaxis in United States and Canadian emergency departments has changed over time. The results showed that there has been an increase in epinephrine treatment for food-induced anaphylaxis, but no clear change for anaphylaxis of any cause. Approximately 45% of emergency department patients with anaphylaxis received epinephrine in the last 10 years. A limitation of the study is the heterogeneity in anaphylaxis definitions.

Can GRACE Risk Score Predict Mortality and the Need for Thrombolytic Treatment in Acute Pulmonary Embolism?

Summary: This study aimed to evaluate the predictive value of the GRACE risk score in acute pulmonary embolism (PE) patients for 30-day mortality and the need for thrombolytic treatment. The study found that the GRACE risk score, along with the PESI score, were independent risk factors associated with 30-day mortality and the need for thrombolytic treatment. The GRACE risk score showed high sensitivity and specificity in predicting mortality, and its combination with the PESI score helped define high-risk PE patients and predict poor prognosis.

Summary: Transthoracic echocardiography (TTE) is commonly used to evaluate isolated tricuspid regurgitation (TR), but there is limited research comparing its quantification with magnetic resonance imaging (MRI). In this study, a novel TTE algorithm was developed to identify severe TR based on TTE parameters. The algorithm had a higher accuracy than the current guidelines' criteria in detecting severe TR by MRI.

Coronary Angiography and Chronic Total Occlusion Angioplasty in a Patient With Situs Inversus from the Right Radial Approach

Summary: This article presents a case of a patient with known situs inversus who underwent cardiac catheterization and was found to have chronic total occlusion in the right coronary artery. Situs inversus is a rare congenital abnormality characterized by the inverted position of chest and abdominal organs. Cardiac catheterization is uncommon in patients with this particular abnormality, highlighting the importance of customizing techniques to engage coronary arteries and optimize guide support for percutaneous coronary intervention if needed.

Pericardial Adipose Tissue Thrombospondin-1 Associates With Antiangiogenesis in Ischemic Heart Disease

Summary: This study evaluated the angiogenic capacity of adipose tissue in patients undergoing cardiac surgeries and found that patients with coronary artery disease (CAD) had reduced angiogenic capacity in their pericardial adipose tissue. Gene expression analysis revealed upregulated expression of the anti-angiogenic gene TSP-1 in CAD patients, with no significant differences in other angiogenic factors. Additionally, inhibiting TSP-1 expression significantly improved angiogenic deficiency in CAD patients.

Evaluation of MADIT-II Risk Stratification Score Among Nationwide Registry of Heart Failure Patients With Primary Prevention Implantable Cardiac Defibrillators or Resynchronization Therapy Devices

Summary: The current guidelines recommend prophylactic implantable cardioverter-defibrillator (ICD) for patients with symptomatic heart failure (HF) and low left ventricular ejection fraction. However, not all patients will benefit from ICD treatment. This study evaluated the feasibility of using the MADIT-II-based Risk Stratification Score (MRSS) to predict the survival benefit of prophylactic ICDs in patients with HF. The results showed that different risk subgroups had varying levels of ICD survival benefit.

The results showed that the younger, female, non-Hispanic Black or Hispanic individuals with a high burden of co-morbidities and unfavorable social determinants of health had the highest risk of mortality in the identified latent classes.
