Sensitive cardiac troponins and N-terminal pro-B-type natriuretic peptide in stable coronary artery disease: correlation with left ventricular function as assessed by myocardial strain

N-terminal pro-B-type natriuretic peptide (NT-proBNP) and cardiac troponins (cTns) measured with sensitive assays provide strong prognostic information in patients with stable coronary artery disease. However, the relationship between these biomarkers and myocardial contractile function, as well as infarct size, in this patient group, remains to be defined. The study population consisted of 160 patients referred to a follow-up echocardiography scheduled 1 year after coronary revascularization. Concentrations of NT-proBNP, high-sensitive cTnT (hs-cTnT) and sensitive cTnI assays were assessed. Left ventricular function was measured as global peak systolic longitudinal strain by speckle tracking echocardiography and infarct size was assessed by late-enhancement MRI. NT-proBNP and sensitive cTnI levels were significantly associated with left ventricular function by peak systolic strain (R-values 0.243 and 0.228, p = 0.002 and 0.004) as well as infarct size (R-values 0.343 and 0.366, p = 0.014 and p = 0.008). In contrast, hs-cTnT did not correlate with left ventricular function (R = 0.095, p = 0.231) and only marginally with infarct size (R = 0.237, p = 0.094). NT-proBNP and sensitive cTnI levels correlate with left ventricular function and infarct size in patients with stable coronary artery disease after revascularization. As opposed to hs-cTnT, NT-proBNP and cTnI seem to be indicators of incipient myocardial dysfunction and the extent of myocardial necrosis.

EACVI survey on burnout amongst cardiac imaging specialists during the 2019 coronavirus disease pandemic

The survey results showed that more than half of the specialists experienced anxiety, one-third felt melancholic, and a quarter increased their alcohol intake. More than half reported difficulties in sleeping and 44% considered quitting their job.

Electrical markers and arrhythmic risk associated with myocardial fibrosis in mitral valve prolapse

Summary: This study aimed to characterize the substrate of T-wave inversion (TWI) and investigate the association between diffuse fibrosis and ventricular arrhythmias (VA) in patients with mitral valve prolapse (MVP). The results showed that an increasing number of ECG leads with TWI reflected a higher risk of severe VA and a higher degree of lateral diffuse fibrosis. Furthermore, lateral diffuse fibrosis was associated with VA even in the absence of late gadolinium enhancement (LGE).

Summary: This prospective study aimed to describe the incidence and morphology of ventricular arrhythmia in patients with arrhythmic mitral valve syndrome and propose risk stratification. The yearly incidence of ventricular arrhythmia was high in these patients, especially in those with secondary preventive implantable cardioverter-defibrillators (ICD). Predictors of first severe arrhythmic event included frequent premature ventricular complexes, non-sustained ventricular tachycardias, greater left ventricular diameter, and greater posterolateral mitral annular disjunction distance.

Summary: The study aimed to assess the level of transesophageal echocardiography (TOE) knowledge and skills among young cardiologists. Results from an online evaluation showed a relatively low level of TOE skills and knowledge among young cardiologists worldwide. Prior participation in TOE simulation-based training, a higher number of TOE exams performed per week, and EACVI certification for TOE were independently associated with higher scores.

Summary: The European Heart Journal-Cardiovascular Imaging, launched in 2012, has become one of the leading multimodality cardiovascular imaging journals over the years. It currently ranks 19th among all cardiovascular journals, with an impressive impact factor of 9.130. The most important studies published in the journal in 2021 will be highlighted in two reports, with Part II focusing on valvular heart disease, heart failure, cardiomyopathies, and congenital heart disease, while Part I has focused on studies related to myocardial function, risk prediction, myocardial ischemia, and emerging techniques in cardiovascular imaging.

Myocardial inefficiency is an early indicator of exercise-induced myocardial fatigue

Summary: This study aimed to determine the effect of prolonged, high-intensity endurance exercise on myocardial function. The results showed that increased exercise duration and intensity resulted in increased myocardial wasted work without alterations in left ventricular ejection fraction and global longitudinal strain. These findings suggest that markers of myocardial inefficiency may precede reduction in global left ventricular function.

Young athlete's growing heart: sex differences in cardiac adaptation to exercise training during adolescence

Summary: This study aimed to investigate the effect of sex on cardiac remodelling during adolescence in athletes. The results showed that male adolescent athletes displayed greater cardiac remodelling compared to females, and this difference increased throughout adolescence. The left ventricular end-diastolic volumes and left ventricular mass index of males showed a progressive increase from age 12 to 18.

Distinguishing left ventricular hypertrophy from hypertrophic cardiomyopathy in adolescents: a longitudinal observation study

Summary: This study compared left ventricular (LV) hypertrophy in early adolescent athletes and hypertrophic cardiomyopathy (HCM) genotype positive patients. The results showed that LV hypertrophy was found in a similar proportion in both groups, but only HCM genotype positive patients showed progression of hypertrophy. The study also identified a potential grey zone of LV hypertrophy with septum thickness Z-scores ranging from 2.0 to 3.3. LV volumes remained larger in athletes. These findings can help clinicians distinguish physiological LV hypertrophy from early HCM.
