Summary: This study compared the long-term outcomes of multiarterial graft (MAG) coronary artery bypass grafting (CABG) versus percutaneous coronary intervention (PCI) with second-generation drug-eluting stents (DES) for treating stable multivessel coronary artery disease. The findings showed that MAG had better survival rates and fewer major cardiac adverse events compared to DES over a 5-year period. Therefore, CABG with MAG may be considered the treatment of choice for patients with stable multivessel coronary artery disease.

Summary: The 2-year results of the SENIOR trial in elderly PCI patients demonstrate that using a strategy combining DES with short-term DAPT reduces the rate of revascularization compared to BMS, with very few late events, and without increasing bleeding complications or stent thrombosis.

Summary: A comparison of long-term outcomes between PCI with drug-eluting stents and CABG in patients with left main coronary artery disease showed no significant difference in 5-year all-cause mortality. However, differences were observed in rates of myocardial infarction, stroke, and repeat revascularisation, with higher risks associated with PCI. A Bayesian approach suggested a slight advantage favoring CABG in terms of mortality.

Summary: The use of double arterial conduits during coronary artery bypass grafting has different outcomes based on sex. In men, there is no difference in mortality and major adverse cardiac and cerebrovascular events between double arterial conduits and single arterial conduits. However, in women, the use of double arterial conduits is associated with an increased risk of 30-day death and major adverse cardiac and cerebrovascular events. Additionally, double arterial conduits have beneficial effects on medium-term survival and cardiovascular outcomes in both sexes.

Summary: The best revascularization strategy for patients with mild to moderate ischaemic heart failure (ejection fraction [EF] 35-50%) remains uncertain. This study found that coronary artery bypass grafting (CABG) may be superior to drug-eluting stent (DES) implantation in terms of long-term survival rate and rate of repeat coronary revascularization for these patients.

Summary: In a randomized controlled trial in Spain, researchers compared the Cre8 EVO stent with the Resolute Onyx stent in patients with diabetes undergoing percutaneous coronary intervention. The study found that the Cre8 EVO stent was non-inferior to the Resolute Onyx stent in terms of target-lesion failure composite outcome, with a suggestion of potential superiority for the Cre8 EVO stent at 1 year follow-up.

Summary: This study evaluated whether interhospital variation in mortality rates for coronary artery bypass grafting was driven by complications and failure to rescue. The study found a positive correlation between hospital failure to rescue rates and complications, suggesting that early recognition and management of complications are important factors in improving the success of coronary artery bypass grafting.

Summary: A systematic review and pooled analysis of high-quality individual patient data from CABG trials found that women have worse outcomes than men in the first 5 years after surgery, with higher rates of cardiac events and repeat revascularization. However, this difference is not significant in patients aged over 75 years and is not affected by the surgical technique.

Summary: This study investigated how information about innovative surgical procedures is communicated to patients. Through qualitative research on seven case studies in hospitals across the United Kingdom, interviews were conducted with clinician innovators, preoperative consultations were recorded, and postoperative interviews were conducted with patients. The study found inconsistencies between clinicians' intentions to inform patients about uncertain outcomes and their actual discussions with patients. Many patients were unaware of the unknown risks associated with the procedure.

Summary: This study quantified the bleeding risk among patients undergoing different antiplatelet and triple therapy regimens for percutaneous coronary intervention or coronary artery bypass grafting, as well as conservatively managed acute coronary syndrome. The results suggest that more potent dual antiplatelet therapy may increase the risk of bleeding without reducing the incidence of major adverse cardiovascular events.

Summary: This study systematically compared the in-hospital mortality risk prediction post-cardiac surgery using different approaches, including the EuroSCORE II, logistic regression, and various machine learning techniques. The results showed that machine learning techniques, such as XGBoost and random forest, had some statistical improvements over traditional models. However, the clinical impact of these improvements is currently modest.
