Gait disorders in CKD patients: muscle wasting or cognitive impairment? A cross-sectional pilot study to investigate gait signatures in Stage 1-5 CKD patients

Summary: There is a correlation between gait and the severity of chronic kidney disease (CKD), with significant linear relationships between gait parameters and CKD severity. The findings are consistent with the results of clinical assessments, suggesting that gait disturbance in CKD patients is not only caused by metabolic factors, but also by brain changes that affect motor control. The treatment of renal disease should take cognitive aspects into consideration.

Summary: Patients with end-stage renal disease are known to be frail, with frailty potentially existing in the predialysis phase, and not worsening with the beginning of replacement therapy. The study found a stable situation in the first year of replacement therapy for both frail and fit participants, with a deterioration in the second year that could lead to death in frail patients.

Summary: This study analyzed data from COVID-19 survivors in Lombardy to reveal the increased rates of rehospitalization, emergency room visits, and outpatient visits in the 6 months following recovery. Various medical tests and drug usage also significantly increased, indicating an increased burden on healthcare resources in the post-COVID-19 condition.

Insulin treatment in patients with diabetes mellitus and heart failure in the era of new antidiabetic medications

Summary: This study compared the clinical effects of insulin with sodium-glucose cotransporter-2 inhibitor (SGLT-2i), glucagon-like peptide-1 receptor agonist (GLP-1RA), and other antihyperglycemic agents (other-AHAs) in patients with heart failure (HF) and diabetes mellitus (DM). The results showed that SGLT-2i and GLP-1RA significantly reduced major adverse cardiac events (MACE) compared with insulin, particularly death from any cause and first hospitalization for HF.

Summary: Despite limited knowledge on the long-term consequences of COVID-19, this study found that the majority of patients reported symptoms and sequelae even 12 months after hospitalization. Exertional dyspnea and fatigue were the most commonly reported symptoms.

Effectiveness and safety of GLP-1 receptor agonists versus SGLT-2 inhibitors in type 2 diabetes: an Italian cohort study

Summary: This study compared the effectiveness and safety of GLP-1 RA and SGLT-2i in a large cohort of patients with type 2 diabetes. The results showed that GLP-1 RA was equally safe but more effective than SGLT-2i in reducing the risk of major adverse cardiovascular events and non-fatal myocardial infarction. The incidence of serious adverse events was low in both treatment groups.

Summary: This study found that the longer the duration of diabetes mellitus in patients with acute myocardial infarction, the higher the risk of in-hospital mortality. The duration of diabetes should be considered as an important early prognostic risk factor in patients with AMI.

Role of Sex and Age in Fatal Outcomes of COVID-19: Women and Older Centenarians Are More Resilient

Summary: In this paper, the role of age and sex in the fatal outcome of COVID-19 was analyzed, and conflicting results in the literature were discussed. The findings showed that women are more resilient than men, centenarians overall did not have lower mortality rates compared to other older individuals, and centenarians over the age of 101 showed greater resilience to COVID-19 compared to younger centenarians in the first wave of the pandemic in 2020. The potential relationship with the 1918 Spanish flu epidemic is still unclear, and further research is needed to understand the underlying mechanisms.

Summary: Cognitive-motor dual-task (CMDT) is essential in daily life and can be challenging for older adults with chronic age-related conditions. Technology-assisted CMDT rehabilitation has shown to be effective in improving motor-cognitive functions, particularly for dual-task performances, falls, gait, and cognition. However, further research is needed to explore its application in different chronic conditions and frailty status.

Summary: This study aimed to describe the polytherapy and multimorbidity pattern of patients receiving anti-VEGF and dexamethasone drugs for the treatment of retinal conditions. The results revealed that patients receiving intravitreal injections have a high prevalence of multimorbidity and polytherapy, leading to a significant burden of care.
