Summary: Basophils, despite their small proportion in circulating blood leukocytes, play significant roles as potent immune effector cells. They express high-affinity receptors for IgE and store powerful inflammatory mediators ready to be secreted. They are implicated in allergic diseases, hypersensitivity reactions, chronic inflammation, and responses to infections. Despite their activation by Th2-biased signals, which are also observed in certain tumors, basophils have been neglected in cancer research. This article discusses the presence and functional significance of basophils in cancer patients and the tumor microenvironment, exploring gene expression analyses and the potential use of basophil activation test (BAT) to predict hypersensitivity to cancer treatments and monitor patient desensitization.

Summary: This study aimed to characterize patients reporting cefazolin allergic reactions and analyze the diagnostic role of in vivo and in vitro tests. Severe immediate-type reactions were often reported by patients allergic to cefazolin, with skin tests enabling a diagnosis in half of patients, but DPT could not be performed in all patients due to reaction severity. BAT may be a promising diagnostic tool for further research to clarify the underlying mechanisms, especially in severe reactions.

Summary: Drug hypersensitivity is increasing due to increased drug consumption, and it often presents in complex clinical settings with co-existing illnesses and polypharmacotherapy. Recent studies have improved the understanding of immediate drug hypersensitivity reactions by categorizing them based on endotypes, biomarkers, and phenotypes. This review focuses on the diagnosis and management of anaphylaxis, discussing both in vivo and in vitro diagnostic methods and highlighting the importance of clinical management, hidden allergens, and delabeling. Endophenotyping plays a crucial role in correctly identifying and treating patients with immediate drug hypersensitivity reactions, with drug desensitization being an effective approach to prevent future episodes.
