Systematic review: are lymphocytic colitis and collagenous colitis two subtypes of the same disease - microscopic colitis?

Background Despite similar clinical symptoms, collagenous colitis (CC) and lymphocytic colitis (LC) are considered two distinct disease entities. Aim To compare pathoanatomical findings, clinical presentations, risk factors, course of diseases and response to treatment in CC and LC to establish whether they could be subtypes of the same disease, microscopic colitis (MC). Methods The MEDLINE was searched for CC, LC and MC, and clinical studies of >20 patients were included. Pooled results with 95% confidence intervals were calculated based on the number of patients. Results An abnormal number of intraepithelial lymphocytes are found in 45% (4050%) with CC, and an abnormal subepithelial collagen band in 16% (1320%) with LC suggesting a histological overlap. The incidence of CC and LC has increased in parallel. Mean age (CC 63 similar to years; LC 60 similar to years) and clinical presentation are indistinguishable, and females are predominant in CC (77%; 7579%) as well as LC (68%; 6670%). Risk factors such as nonsteroid anti-inflammatory drugs consumption CC 39% (3642%); LC 32% (2935%) are similar and prevalence of concomitant autoimmune diseases such as coeliac disease (CC 5%; CI: 46% and LC 7%; CI: 69%) do not differ. Bile acid diarrhoea is highly prevalent in CC (41%; 3745%) and LC (29%; 2434%). The effect of budesonide is identical. Conclusions CC and LC could be considered histological subtypes of the same disease, MC. To facilitate recruitment to clinical trials, all MC patients could be included in future trials and stratified for subtypes.

Summary: The study evaluated the distribution of histopathological features of microscopic colitis throughout the colon. Both collagenous colitis and lymphocytic colitis patients exhibited more pronounced inflammation in the proximal colon compared to the distal colon. The minimum criteria required for diagnosis were present in the majority of segments, suggesting microscopic colitis as a pancolitis with the rectum excluded.

Summary: Microscopic colitis (MC) encompasses lymphocytic colitis (LC) and collagenous colitis (CC). LC often presents with thickening of the subepithelial collagen band, while CC is characterized by an increased number of intraepithelial T-lymphocytes (IELs). The essential histopathological feature of MC is considered to be the lymphocytic inflammatory infiltration of the mucosa, although there is a non-linear correlation between the number of IELs and thickness of the collagenous band, indicating differences in their pathogenesis.

Summary: This study aimed to evaluate the efficacy of budesonide as induction therapy for MCi, but was prematurely discontinued due to insufficient patient recruitment. Although the primary endpoint of clinical remission was not statistically significant, budesonide showed advantages in improving quality of life and reducing the frequency of soft stools in patients with MCi.

Autoimmune diseases in microscopic colitis: A Danish nationwide case-control study

Summary: Using a matched case-control design based on nationwide registry data, it was found that there is a strong association between microscopic colitis and a wide range of autoimmune diseases, particularly those of gastrointestinal origin. The results suggest an autoimmune predisposition to microscopic colitis.

Summary: Microscopic colitis is considered a chronic but benign disease, with most patients experiencing a chronic active or relapsing disease course during the first year after diagnosis. Symptoms and quality of life tend to improve after follow-up, but a relapsing or chronic active disease course is associated with significantly more symptoms and impaired quality of life.
