Summary: Effective antiviral therapy can improve the long-term prognosis of HBV-related decompensated patients and achieve re-compensation in some cases. This study analyzed the clinical outcomes of 196 patients with HBV-related first decompensated cirrhosis of ascites treated with nucleos(t)ide analogue and found that ascites regression and re-compensation were observed after NUC treatment. The level of serum ALT and HBV DNA at baseline were identified as risk factors for ascites re-compensation.

Summary: Hepatitis B virus (HBV) RNA levels showed significant variation between different tissue samples, with up to a 1000-fold difference. When replication was high, core RNA and S RNA levels were similar, indicating transcription from covalently closed circular DNA (cccDNA). In patients with medium-high or low replication, enhanced expression of S RNA relative to cccDNA and core RNA suggested that HBsAg can be primarily expressed from integrated HBV DNA.

Summary: Hepatocellular carcinoma (HCC) is a major cause of cancer-related death, with chronic viral hepatitis being a significant contributing factor. The pathophysiology of viral-related HCC involves liver inflammation, oxidative stress, and deregulation of cell signaling pathways. Regular ultrasound surveillance is recommended for patients with cirrhosis and high-risk individuals with chronic HBV infection. Antiviral therapy reduces the risks of HCC development and recurrence. Various scoring systems have been developed to predict the risk of HCC development in patients with chronic hepatitis B.

Summary: Alcohol-associated liver disease (ALD), including alcohol-associated hepatitis (AH), is the most common etiology among cirrhosis-related hospitalizations in the USA among American Indian/Alaska Native (AI/AN) individuals. In-hospital mortality rate was around 6% for AI/AN individuals, higher compared to Blacks but similar to other races. Public health policies should be implemented to reduce the burden of advanced ALD among AI/AN individuals.

Summary: Alcohol-associated liver disease (ALD) is a common liver disease and a reason for liver transplantation (LT). Patients with ALD often have alcohol use disorder (AUD) and psychiatric comorbidities. Early identification of ALD and treatment of AUD can prevent progression to advanced stages of the disease. However, there are barriers to screening and treatment for AUD in ALD patients. This review discusses the multidisciplinary care model for managing the dual pathology of ALD and AUD, with a focus on the roles of hepatologists, psychiatrists, addiction counselors, and social workers.

Summary: Alcohol-associated liver disease (ALD) is a complex and rapidly increasing disease without approved therapeutic targets. Challenges in study designs and clinical trials need to be overcome to drive drug development in this field. ALD management should focus on achieving and maintaining alcohol abstinence, while early liver transplantation protocols and noninvasive biomarkers require improvement. Integrated multidisciplinary care models are urgently needed to improve long-term outcomes for ALD patients with alcohol use disorder and liver disease.
