Review article: enzyme supplementation in cystic fibrosis, chronic pancreatitis, pancreatic and periampullary cancer

Background Over 11 000 UK patients each year develop pancreatic exocrine insufficiency - the major causes are not rare: cystic fibrosis (>300 new cases/year), pancreatic cancer (>7000 new cases/year) and chronic pancreatitis (>4000 new cases/year). Affected patients present in diverse ways, and for chronic pancreatitis, diagnosis is frequently made rather late in the course of the disease. Aim To raise awareness of key clinical issues specific to patients with pancreatic exocrine insufficiency through experience from UK clinicians, and to offer advice regarding appropriate treatment with pancreatic enzymes. Methods Three case studies describe clinical issues relating to pancreatic enzyme supplementation that may lead to underuse in patients with cystic fibrosis, pancreatic and periampullary cancer or chronic pancreatitis. Results The efficacy of the treatment of exocrine pancreatic insufficiency is dependent on adequate meal-time enzyme replacement therapy. Improvements in patients' weight and nutritional status are what is aimed for - an important reason for all doctors, nurses and dieticians to give this therapy close attention. Conclusions Pancreatic exocrine insufficiency may result in malnutrition, but enzyme supplementation can greatly improve quality of life in these patients.

Summary: This study projects the population-level impact of the new CF drug ELEX/TEZ/IVA on IV antibiotic treatment days by combining observational UK CF Registry data with RCT results. The results indicate significant reductions in IV antibiotic requirements at the population level with the use of ELEX/TEZ/IVA.

Elexacaftor-Tezacaftor-Ivacaftor improves exercise capacity in adolescents with cystic fibrosis

Summary: This study found that treatment with Elexacaftor/Tezacaftor/Ivacaftor for 6 weeks improved exercise capacity in three adolescents with cystic fibrosis, especially in those with more severe lung disease and lower aerobic fitness at baseline. However, the mechanisms behind these improvements require further investigation in larger trials.

Hospital discharge using salbutamol as required after acute attacks of wheeze in children: a service evaluation

Summary: This study introduces a new management approach for acute wheezing in children, which is to use bronchodilators as needed. By comparing with previous years, it is found that this approach can safely and effectively reduce the dosage of salbutamol.

Asthma, classical conditioning, and the autonomic nervous system - a hypothesis for why children wheeze

Summary: This review explores the possible role of abnormal triggering of the autonomic diving reflex in paediatric asthma. It suggests that stress during pregnancy could affect the developing nervous system, leading to increased parasympathetic responsiveness and excessive triggering of the diving reflex. Misfiring of this reflex could result in the pathological airway narrowing that occurs in asthma. The hypothesis also proposes that classically conditioned reflex responses become ongoing triggers of recurrent wheeze, regardless of allergen exposure.

The tracheal system of the Common Wasp (Vespula vulgaris) - A micro-CT study

Summary: X-ray micro-CT was used to study the tracheal system of Pre and Post hibernation Queen wasps and their workers. The findings showed that the tracheal system is more complex than previously thought, and one wasp had a previously undescribed tracheal system. Comparison was made with a study from the American Museum of Natural History, and recommendations for future studies were given.

Structured transition is associated with improved outcomes in diabetes

Summary: This study aimed to assess the effectiveness of a structured transition tool for young people with type 1 diabetes transitioning between pediatric and adult diabetic services. The study found that participation in the 'Ready Steady Go' transition program was associated with improved glycemic control, lower rates of non-elective hospital admissions, and lower non-high density lipoprotein cholesterol concentrations.
