Merkel cell carcinoma (MCC) is a rare aggressive cutaneous malignancy of the elderly and immunocompromised populations. The clinical presentation of MCC is nonspecific, with the majority of cases presenting as localized skin involvement. Histologically and immunophenotypically, MCC is defined by both neuroendocrine and epithelial differentiation. Recently, the Merkel cell polyomavirus has been implicated in the pathogenesis of MCC. In addition, there have been numerous studies evaluating the histologic and immunohistochemical characteristics of MCC as they relate to diagnosis and prognosis. The purpose of this paper is to review the most salient and clinically relevant updates in the pathogenesis and histologic features of MCC. Specific attention is given to the clinical and histologic predictors of prognosis, staging, and the controversies concerning sentinel lymph node biopsy and therapy.

Evaluation of Melanocyte Loss in Mycosis Fungoides Using SOX10 Immunohistochemistry

Summary: Mycosis fungoides (MF) is a subtype of primary cutaneous T-cell lymphoma (CTCL) with indolent course. A study assessed melanocyte loss in MF through immunohistochemistry, finding a decrease or focal loss of melanocytes in 50% of cases. Factors such as predominant neoplastic cell type, age, race, gender, and histologic features were not predictive of melanocyte loss.

A Previously Unrecognized Granulomatous Variant of Gamma-Delta T-Cell Lymphoma

Summary: Primary cutaneous gamma delta T-cell lymphoma (PCGD-TCL) is a rare and aggressive T-cell neoplasm. The cases presented in this study showed adnexotropic infiltrate with atypical lymphocytes expressing CD3, CD8 and CD56. One patient underwent CT and PET scans revealing multi-organ involvement.

GLUT1 Expression in Cutaneous Sebaceous Lesions Determined by Immunohistochemical Staining Patterns

Summary: GLUT1, a membrane associated carrier protein involved in glucose transport, has shown increased expression in various tumor types. This study aimed to determine GLUT1 expression in cutaneous sebaceous lesions. Strong cytoplasmic staining of GLUT1 was observed in sebaceous carcinoma, while a gradient of staining intensity was seen in sebaceomas and sebaceous adenomas, and minimal staining in sebaceous hyperplasia.

Lack of PRAME Expression in Cutaneous T-Cell Lymphomas

Summary: PRAME is not expressed in cutaneous T-cell lymphomas (CTCLs), suggesting that the pathogenesis of CTCL is not mediated by PRAME. Further study is required to identify biomarkers that might aid in the diagnosis and prognostication of CTCLs.

Absence of TFE3 Immunoexpression in a Spectrum of Cutaneous Mixed Tumors: A Retrospective Pilot Study

Summary: Our study failed to reveal possible TFE3 gene fusion by IHC staining in benign, atypical, and malignant chondroid syringomas. Although the negative staining in MCS suggests a genetic heterogeneity in this entity, further studies with larger case groups are needed for a more definitive conclusion.

FLI-1/Melan-A dual stain is an alternative to PRAME in differentiating metastatic melanoma from nodal nevus: A monocentric retrospective study

Summary: Melanocytic nevi in lymph nodes pose a diagnostic challenge and can be mistaken for metastases. The use of PRAME immunohistochemical stain can help differentiate between the two. FLI-1 immunohistochemical expression has shown variable sensitivity in malignant melanoma, while melanocytic nevi are reported to be negative. In this study, we examined lymph nodes with metastatic melanoma and benign deposits, and found that FLI-1/Melan-A dual staining is equally sensitive and specific as PRAME in distinguishing between the two. Larger studies are needed to confirm these significant results.

TRPS1 Is Differentially Expressed in a Variety of Malignant and Benign Cutaneous Sweat Gland Neoplasms

Summary: A recent study investigated the expression of TRPS1 in various sweat gland tumors, revealing a high expression in both malignant and benign tumors. However, there was differential staining among different types of sweat gland tumors, suggesting potential differences in cellular origin or differentiation.

Two distinct pathogenic pathways of digital papillary adenocarcinoma - BRAF mutation or low-risk HPV infection

Summary: This study presents a series of eight cases of digital papillary adenocarcinoma (DPA) with two distinct clinical presentations, morphologies, immunophenotypes, and molecular features. Four cases showed painless, slow-growing nodules with glandular structures, while the other four cases showed painful, rapidly growing masses with solid, tightly packed papillary architecture.

Histopathologic Aspects of Malignancy-Associated Granuloma Annulare: A Single Institution Experience

Summary: Granuloma annulare (GA) is a benign inflammatory disease with different histologic patterns, while malignancy-associated granuloma annulare (MGA) is rarely associated with cancer. This study aimed to compare the clinical and histopathological differences between GA and MGA. It was found that MGA is more common in older individuals and affects the extremities, with chronic lymphocytic leukemia being the most common associated malignancy. The histopathological pattern of interstitial was more commonly seen in MGA, while necrobiotic pattern was more common in GA.

TRPS1: A Marker of Follicular Differentiation

Summary: This study evaluates the immunohistochemical expression of TRPS1 in cutaneous neoplasms with follicular differentiation, including trichoblastoma, trichoepithelioma, and basal cell carcinoma. The study found that TRPS1 showed intermediate-to-high positivity in trichoblastoma and trichoepithelioma, while it was negative or sporadically positive in basal cell carcinoma.
