Innovative approaches to optimizing the delivery of vancomycin in individual patients

The delivery of personalized antimicrobial therapy is a critical component in the treatment of patients with invasive infections. Vancomycin, the drug of choice for infections due to methicillin-resistant Staphylococcus aureus, requires the use of therapeutic drug monitoring (TDM) for delivery of optimal therapy. Current guidance on vancomycin TDM includes the measurement of a trough concentration as a surrogate for achieving an AUC to minimum inhibitory concentration (MIC) by broth microdilution (AUC/MICBMD) ratio >= 400. Although trough-only monitoring has been widely integrated into clinical practice, there is a high degree of inter-individual variability between a measured trough concentration and the actual AUC value. The therapeutic discordance between AUC and trough may lead to suboptimal outcomes among patients with infections due to less susceptible pathogens or unnecessarily increase the probability of acute kidney injury (AKI) in others. Given the potentially narrow vancomycin AUC range for optimal effect and minimal AKI, clinicians need a real-time system to predict accurately the AUC with limited pharmacokinetic (PK) sampling. This article reviews two innovative approaches for calculating the vancomycin AUC in clinical practice based on one or two drug concentrations. One such approach involves the use of Bayesian computer software programs to estimate the true vancomycin AUC value with minimal PK sampling and provide AUC-guided dosing recommendations at the bedside. An alternative involves use of two concentrations (peak and trough) and simple analytic equations to estimate AUC values. Both approaches provide considerable improvements over the current trough-only concentration monitoring method. (C) 2014 The Authors. Published by Elsevier B.V.

High-Dose Micafungin in Neonates and Young Infants with Invasive Candidiasis: Results of a Phase 2 Study

Summary: This study evaluated the efficacy and safety of micafungin at a dose of 8 mg/kg daily in 35 neonates and young infants with invasive candidiasis, showing that the treatment was effective in resolving the infection in 86.7% of patients treated for >= 14 days, with some patients experiencing transient hypertransaminasemia. Micafungin at this dosage was found to be well tolerated in this population.

Estimated glomerular filtration rate with and without race for drug dosing: Cystatin C vs. serum creatinine

Summary: The study aimed to explore the precision in dosing with different equations for estimating glomerular filtration rate (eGFR) by using a model drug (vancomycin) dependent on kidney function clearance. The findings suggest that the nonrace-based serum cystatin C (S-cys) eGFR model outperformed other clearance models and improved the probability of target attainment. When S-cys data is not available, the new 2021 CKD-EPI eGFR(cr) equation without race factor performs equally well as the conventional approach. This study highlights the importance of removing race as a factor for kidney function dose adjustment.

Linezolid Population Pharmacokinetics to Improve Dosing in Cardiosurgical Patients: Factoring a New Drug-Drug Interaction Pathway

Summary: Linezolid-induced myelosuppression is likely to occur in cardiosurgical patients due to reduced kidney function and potential drug interactions. Population modeling and simulation demonstrate that lower doses coupled with therapeutic drug monitoring are necessary in this vulnerable population. Thus, reducing the empiric linezolid doses in cardiosurgical patients may help avoid toxicities, and further studies are needed to verify potential drug interactions.

Removing race and body surface area indexation for estimated kidney function based drug dosing: Aminoglycosides as justification of these principles

Summary: This study examined the precision gain or loss in drug dosing with estimated creatinine clearance (eCLcr) and estimated glomerular filtration rate (eGFR) using serum creatinine (eGFRcr) with and without race and body surface area (BSA) indexation. The results showed that the eGFR equation without race and BSA indexation provided a better method for drug dosing in terms of precision.

Optimizing anidulafungin exposure across a wide adult body size range

Summary: Echinocandins like anidulafungin are first-line therapies for candidemia and invasive candidiasis, but their dosing may be suboptimal in obese patients. This study aimed to evaluate the impact of body size on anidulafungin pharmacokinetics (PK) and found that a higher maintenance dose of anidulafungin is needed in larger adults to achieve the desired effect.

Reconsideration of the current models of estimated kidney function-based drug dose adjustment in older adults: The role of biological age

Summary: As human lifespan increases, there is a decline in kidney function that affects the pharmacokinetics and pharmacodynamics of drugs. Various equations have been developed to estimate glomerular filtration rate (GFR), but the trajectory of changes differs with age. Biological age is recognized as an important parameter in kidney function equations, and efforts to quantify it have grown in recent years. This narrative review examines how different equations model the changes in GFR across chronological age and discusses the challenges and potential of incorporating biological age in drug dosing for older adults.

Kidney function as a key driver of the pharmacokinetic response to high-dose L-carnitine in septic shock

Summary: The study aimed to describe the population pharmacokinetics of high-dose L-carnitine in patients with septic shock, test various estimates of kidney function, and evaluate the correlation of PK parameters with pre-treatment metabolites. The results indicated that kidney function estimates and baseline metabolic status are important factors influencing the therapeutic effect of L-carnitine supplementation in septic shock patients.
