Alcohol policy in South Africa: a review of policy development processes between 1994 and 2009

Background Implementation of effective policies to reduce harmful alcohol consumption requires both a good understanding of the policy development process and which strategies are likely to work. Aims To contribute to this understanding by reviewing four specific policy development initiatives that have taken place in South Africa between 1994 and 2009: restrictions on alcohol advertising and counter-advertising, regulation of retail sales of alcohol, alcohol taxation and controls on alcohol packaging. Methods Material was drawn from a record of meetings and conferences held between 1994 and 2009 and a database of reports, newspaper clippings and policy documentation. Findings When the policy process resulted in a concrete outcome there was always a clear recognition of the problem and policy alternatives, but success was more likely if there was an alignment of 'political' forces and/or when there was a determined bureaucracy. The impact of the other factors such as the media, community mobilization, non-governmental organizations (NGOs), the liquor industry and research are also discussed. Future avenues for policy research are identified, including the need for more systematic studies that give greater consideration to economic factors. Conclusions Alcohol policy development in South Africa takes place in a piecemeal fashion and is the product of various competing influences. Having a comprehensive national alcohol strategy cutting across different sectors may be a better way for other developing countries to proceed.

Trauma trends during COVID-19 alcohol prohibition at a South African regional hospital

Summary: The study found that trauma volume significantly decreased during periods of complete alcohol prohibition in South Africa during the COVID-19 lockdown, increased when alcohol sales were partially reinstated, and decreased again with complete ban. When alcohol sales were partially allowed again, trauma volume increased to pre-COVID-19 levels.

Did COVID-19-Related Alcohol Sales Restrictions Reduce Alcohol Consumption? Findings from a National Online Survey in South Africa

Summary: South Africa has a high prevalence of heavy episodic drinking (HED), and during the COVID-19 pandemic, the public hospitals were overwhelmed due to alcohol misuse and violence. This study found that during lockdown restrictions, including alcohol sales bans, many participants bought alcohol illegally. Those who consumed more alcohol than usual during restrictions reported feeling stressed, needing to relax, and being bored.

Summary: This study is the ninth cross-sectional community study on fetal alcohol spectrum disorders (FASD) conducted in the Western Cape Province of South Africa. The results showed that FASD remains a significant problem in this region, but the severity of physical traits and anomalies within the FASD continuum is trending downwards.

Summary: Alcohol is widely used as a psychoactive drug. Hazardous drinking is associated with various health issues. Alcohol use disorders are psychiatric syndromes characterized by impaired control over drinking. The causes of alcohol use disorders include genetic, neurobiological, psychological, and social factors. Effective approaches for reducing alcohol harms exist but are underused. The field should focus on translating research into clinical applications and promoting evidence-based approaches.

The IAC policy index may serve as a useful tool for evaluating alcohol policies' support for abstinence in high and middle-income countries.

Summary: This study tested the efficacy of a brief intervention for reducing alcohol consumption among patients on HIV antiretroviral therapy. The intervention was found to be effective in reducing alcohol consumption.

Summary: Accurately quantifying alcohol use among persons with HIV (PWH) is crucial for assessing the efficacy of alcohol reduction interventions. In a randomized controlled trial in South Africa, we compared self-reported hazardous alcohol use with a biomarker test and found low agreement between AUDIT-C scores and heavy drinking. Underreporting of hazardous drinking was more common among females at the 6-month follow-up.

Summary: The study found that drinking patterns among different countries are associated with the International Alcohol Control (IAC) Policy Index. Increasing the IAC policy index score is linked to a decrease in drinking frequency and volume. Furthermore, women, individuals with lower education levels, and young adults are more responsive to these policies. This study highlights the importance of implementing alcohol policies globally.

Untargeted Metabolome Analysis of Alcohol-Exposed Pregnancies Reveals Metabolite Differences That Are Associated with Infant Birth Outcomes

Summary: Prenatal alcohol exposure can lead to offspring growth deficits and neurodevelopmental disabilities. Using untargeted metabolomics, this study found that alcohol consumption was associated with reduced levels of sphingomyelins, cholesterol, pregnenolones, and increased levels of fatty acids, acyl and amino acyl carnitines, and androsterones. Additionally, certain metabolites, including monoacylglycerols and glycerol-3-phosphate, were associated with negative outcomes for infant length, weight, and head circumference, implicating their role in fetal development.
