Purpose: To examine the circadian intraocular pressure (IOP) patterns in healthy subjects, in primary open angle and normal tension glaucoma (POAG; NTG) using a contact lens sensor (CLS; Sensimed Triggerfish, Lausanne, Switzerland). Methods: This was an observational, nonrandomized study. Ten healthy subjects (Group 1, 10 eyes) and 20 glaucomatous patients [20 eyes, 10 with POAG (Group 2) and 10 with NTG (Group 3)] were enrolled. All patients were controlled with prostaglandin analogues. The 24-hr IOP pattern was the main outcome. The morning (6AM-11AM), afternoon/evening (noon-11PM) and night (midnight-5AM) subperiod patterns, peaks and prolonged peaks (>1 hr) were secondary outcomes. Results: Mean 24-hr IOP pattern showed a nocturnal acrophase in all groups. Patterns were significantly different among groups (p = 0.02), with highest nocturnal IOP values in POAG. Prolonged peaks were more common in patients with glaucoma (70%) than in healthy subjects (33.3%) (p < 0.001). Significant differences were found for Groups 2 and 3 in the morning versus afternoon/evening (p = 0.019 and p = 0.035, Bonferroni correction), morning versus night (p = 0.005 and p < 0.0001) and afternoon/evening versus night periods comparisons (p < 0.0001 for both groups). In Group 1, patterns significantly differed in the morning versus night and afternoon/evening versus night period comparisons (p < 0.0001). Conclusions: Continuous 24-hr IOP monitoring with the CLS revealed a nocturnal acrophase in healthy subjects and, more markedly, in glaucoma. Because the diurnal IOP profile seems not to predict the nocturnal rhythm, the circadian IOP pattern should be evaluated in clinical practice. These findings may be worthwhile for the management of glaucoma.

Levetiracetam Prophylaxis Therapy for Brain Tumor-Related Epilepsy (BTRE) Is Associated With a Higher Psychiatric Burden

Fedele Dono, Stefano Consoli, Giacomo Evangelista, Annalisa Ricci, Mirella Russo, Claudia Carrarini, Angelo Di Iorio, Laura Bonanni, Francesca Anzellotti, Marco Onofrj, Stefano L. Sensi

Summary: This study evaluated the neuropsychiatric side effects of prophylactic use of levetiracetam (LEV), a third-generation anti-seizure medication, in patients with brain tumor-related epilepsy (BTRE). The results showed that compared to a group of BTRE patients, a group taking LEV as prophylaxis had higher severity of neuropsychiatric symptoms. The study emphasizes the importance of accurate epileptological evaluations in selecting patients who would benefit most from anti-seizure medication in brain tumor patients.

Summary: This study conducted a cross-sectional analysis on patients with UMNS spasticity and found that women may require more attention to the treatment of spasticity in adductor muscles, especially the adductor group. According to the pattern of disability, patients with hemiparesis more frequently needed treatment in the upper limb, while patients with diplegia/double-hemiparesis more frequently required treatment in the adductor and crux muscles.

Summary: The temporal lobe plays a central role in regulating the Central Autonomic Network and cardiovascular functions. This study suggests that Perampanel (PER) may increase vagal tone and have a cardioprotective effect by reducing the risk of cardiac arrhythmias.

Natural IgG antibodies to β amyloid are decreased in patients with Parkinson's disease

Roberto Paganelli, Alessia Paganelli, Graham Pawelec, Angelo Di Iorio

Summary: Natural antibodies against aggregation-prone proteins have been discovered in healthy individuals, and these proteins may play a pathogenic role in neurodegenerative diseases of ageing. The study examines the levels of natural antibodies to amyloid beta in Italian patients with Alzheimer's dementia, vascular dementia, non-demented Parkinson's disease patients, and healthy elderly controls. The results show that the levels of amyloid beta antibodies are similar in Alzheimer's patients compared to controls, but surprisingly reduced in Parkinson's disease patients, which may identify individuals more prone to amyloid aggregation.

Summary: Aging is characterized by chronic low-level inflammation and is associated with geriatric syndromes such as sarcopenia and frailty. Leukocyte-derived ratios, specifically neutrophil-to-lymphocyte ratio (NL-ratio) and monocyte-to-lymphocyte ratio (ML-ratio), may be implicated in age-related mechanisms that affect body composition and muscle strength. Further studies are needed to determine their usefulness for early detection of sarcopenia, myosteatosis, and frailty in older adults.

Summary: The aim of this study was to evaluate the changes in vitamins, hormones, free radicals, and antioxidant substances during the season and their association with performance improvement after training. The results showed that athletes had better performance after training, but no significant relationships were found between vitamin and hormone levels and the performance improvement. However, there was an indirect relationship between oxidative stress and performance improvement.

Muscle quality, physical performance, and comorbidity are predicted by circulating procollagen type III N-terminal peptide (P3NP): the InCHIANTI follow-up study

Summary: The study finds that the collagen turnover marker P3NP is associated with changes in muscle density and physical performance. Participants with lower P3NP levels perform better in terms of muscle density and physical performance and have a lower risk of developing chronic degenerative age-related diseases. The interaction between P3NP and factors such as leptin, muscle density, and fat mass affects muscle density and physical performance.
