Purpose: Optical coherence tomography (OCT) is restricted by its low scanning speed and limited resolution. High-definition raster-scanning OCT (HD-OCT) was used to evaluate changes in retinal microstructure in patients with acute central serous chorioretinopathy (CSCR) and to find new morphological features. Methods: Eighteen patients with subretinal fluid accumulation caused by acute CSCR were imaged in a cross-sectional study design. High-speed frequency-domain HD-OCT was used to image an area of 6 x 6 mm in the macular retina. Three-dimensional analyses were performed using en-face imaging and section analysis of single HD-OCT scans. Results: Detailed information about fluid accumulation can be obtained in all compartments. Discrete changes in reflectivity are visualized within the outer nuclear or plexiform layers in > 90% of patients. Subretinal fluid appears as a dome-shaped pool of fluid and is not associated with a loss of photoreceptor layer integrity. Deposits are demarcated beneath the outer cone segments. Multiple small pigment epithelial detachments are present in > 60% of patients. Conclusions: High-definition OCT provides extensive information regarding precise topographic and layer-specific localization of discrete morphological changes. Along with well-known changes in the retinal microstructure, hyper-reflectivity can be imaged in the outer nuclear or plexiform layers and may represent intraretinal changes indicating the presence of subretinal pathologies or retinal maladjustment caused by the underlying pathology.

Therapeutic response in the HAWK and HARRIER trials using deep learning in retinal fluid volume and compartment analysis

Ursula Schmidt-Erfurth, Zufar Mulyukov, Bianca S. Gerendas, Gregor S. Reiter, Daniel Lorand, Georges Weissgerber, Hrvoje Bogunovic

Summary: Deep learning methods allow accurate assessment of the therapeutic response to brolucizumab and aflibercept in neovascular age-related macular degeneration. Residual volumes of intraretinal fluid, subretinal fluid, and pigment epithelial detachment are associated with visual loss.

Markus Schranz, Reinhard Told, Valentin Hacker, Gregor S. Reiter, Adrian Reumueller, Wolf-Dieter Vogl, Hrvoje Bogunovic, Stefan Sacu, Ursula Schmidt-Erfurth, Philipp K. Roberts

Summary: The study found that macular neovascularization parameters play a minor role in the exudation process in nAMD, and none of the MNV parameters were correlated with the treatment response.

A systematic evaluation of human expert agreement on optical coherence tomography biomarkers using multiple devices

Martin Michl, Martina Neschi, Alexandra Kaider, Katja Hatz, Gabor Deak, Bianca S. S. Gerendas, Ursula Schmidt-Erfurth

Summary: The objective of this study was to assess agreement in evaluating OCT variables in leading macular diseases among OCT-certified graders. Even in optimized conditions, there is disease-dependent variability in biomarker evaluation, particularly for IRF in nAMD and DMO. Our findings highlight the variability in human expert OCT grading performance and the need for AI-based automated feature analyses.

Segmentation of Bruch's Membrane in Retinal OCT With AMD Using Anatomical Priors and Uncertainty Quantification

Botond Fazekas, Dmitrii Lachinov, Guilherme Aresta, Julia Mai, Ursula Schmidt-Erfurth, Hrvoje Bogunovic

Summary: Segmentation of Bruch's membrane (BM) on optical coherence tomography (OCT) is crucial for the diagnosis and follow-up of age-related macular degeneration (AMD), a leading cause of blindness. Existing automated methods lack anatomical coherence and confidence feedback, limiting their real-world applicability. To address this, we propose an end-to-end deep learning method that uses an Attention U-Net to output a probability density function and considers the natural curvature of the surface. Additionally, our method estimates uncertainty and interpolates A-scans with high uncertainty. Evaluation on internal and external datasets demonstrates superior performance and strong generalization ability.

Summary: This study aimed to assess disease activity and the effects of intravitreal pegcetacoplan treatment on the topographic progression of geographic atrophy (GA) secondary to age-related macular degeneration. The study used automated deep learning algorithms to analyze SD-OCT scans and found that pegcetacoplan-treated eyes showed significantly slower GA lesion progression compared with sham.

FACE-Q Patient Report-Assisted Subjective and Objective Evaluation of Blepharoplasty Outcomes Using Two Different Suturing Techniques: A Randomized and Patient-Blinded Pilot Study

Reinhard Told, Eva Placheta-Gyoeri, Birgit Lackner, Andreas Kuchar, Jonas Brugger, Ursula Schmidt-Erfurth, Roman Dunavoelgyi

Summary: This study compared two suturing techniques in upper eyelid blepharoplasty and assessed patient-reported outcomes using the FACE-Q(TM) Eye Module questionnaire and blinded Likert-scale gradings of two surgeons. The results showed no significant difference in patient satisfaction and appraisal of the upper eyelids between the two suturing techniques at both 7 days and 3 months after surgery. Dry eye symptoms increased postoperatively, regardless of the suturing technique, patient age, or sex.

OCTA Biomarker Search in Patients with nAMD: Influence of Retinal Fluid on Time-Dependent Biomarker Response

Reinhard Told, Adrian Reumueller, Markus Schranz, Jonas Brugger, Guenther Weigert, Gregor Sebastian Reiter, Stefan Sacu, Ursula Schmidt-Erfurth

Summary: This study aimed to evaluate the individual influence of retinal thickness, intra- and sub-retinal fluid on the treatment response in patients with neovascular AMD. The results showed that retinal thickness and the presence of intra- and sub-retinal fluid had an impact on the treatment response, while junction density and vessel density did not.

Summary: This study evaluated the quantitative impact of drusen and hyperreflective foci (HRF) volumes on mesopic retinal sensitivity in age-related macular degeneration (AMD) patients. The results showed a negative correlation between the volume of drusen and HRF and retinal sensitivity. The study highlights the use of AI-based methods to assess the impact of cellular changes on the development of AMD.

Summary: The purpose of this study was to evaluate the reliability of automated fluid detection in identifying retinal fluid activity in OCT scans of patients treated with anti-VEGF therapy for neovascular age-related macular degeneration. The results showed that the concordance between human expert grading and automated algorithm performance was high, indicating that deep learning-based segmentation of retinal fluid performs reliably on OCT images.

Summary: Optical Coherence Tomography Angiography (OCTA) has the potential to replace invasive fluorescein angiography (FA) in ophthalmology, but it still lacks the field of view compared to fluorescence fundus photography techniques. To address this issue, a custom developed high-speed swept-source OCT (SS-OCT) system is presented, which can capture ultra-wide fields of view up to 90 degrees with high resolution. Additionally, a three-dimensional deep learning based algorithm is developed for denoising volumetric OCTA data sets, enhancing the visual appearance of angiograms.

Impact of Choroidal Melanoma Characteristics on Progression-Free Survival in Patients Undergoing Hypofractionated Stereotactic Photon Radiotherapy

Reinhard Told, Judith Kreminger, Ursula Schmidt-Erfurth, Roman Dunavoelgyi, Adrian Reumueller

Summary: This study investigated the relationship between choroidal melanoma characteristics and progression-free survival (PFS) in patients who underwent linear accelerator-based hypofractionated stereotactic photon radiotherapy. The results showed that thickness and largest basal diameter (LBD) were significantly associated with PFS. For monitoring treated choroidal melanoma, LBD assessments could be used instead of ultrasonography-based thickness measurements.
