Summary: This study found that Crooke's hyaline change (CC) of nontumorous corticotrophs was observed in 83% of patients with Cushing disease (CD), with most patients experiencing remission. Absence of CC in nontumorous corticotrophs may serve as a predictor of reduced remission in patients with CD.

Summary: This study analyzed the diagnostic accuracy of BIPSS in pediatric patients with CD, showing that BIPSS has high accuracy in identifying the underlying cause and guiding surgical intervention, but limited value in tumor localization.

Summary: A novel regulatory region near the intron 2/exon 3 junction in the human POMC gene functions as a second promoter and enhancer, regulated by methylation status in ACTH-secreting pituitary tumors, and potentially influencing the clinical phenotype of Cushing disease.

Summary: We identified a novel regulatory region near the intron 2/exon 3 junction in the POMC gene, serving as a second promoter and enhancer regulated by methylation status, with demethylation patterns correlating with Cushing disease clinical phenotypes.

Summary: Seliciclib may directly target pituitary corticotrophs in patients with Cushing's disease and reverse hypercortisolism. However, further research is needed regarding the liver toxicity associated with seliciclib.

Summary: Crooke's cell adenoma is a tumor subtype with obvious clinical aggressive behavior. Male patients are more common and often present with elevated adrenocorticotropic hormone levels. The tumor diameter is larger and invasive. Female patients dominate the reported cases and exhibit symptoms such as Cushing's syndrome and hormonal dysfunction. Tumors are more commonly macroadenomas and grow in the suprasellar region, with a high recurrence rate and a significant proportion progressing to pituitary carcinomas.

Summary: Molecular pathology has advanced our understanding of tumors, particularly in pituitary and craniopharyngioma where genetic mutations have been identified. However, these mutations have limited impact on treatment strategies. Epigenetic changes play a crucial role in the pathogenesis of sporadic pituitary neoplasms.

Summary: This study is the first to report a high prevalence of radiologic VFs in recently diagnosed CD patients. VFs may represent an early manifestation of CD and may be related to cortisol levels. Therefore, VF assessment should be included in the workup during CD diagnosis.

Summary: This study investigated whether advances in imaging improved outcomes for Cushing's disease. By analyzing 166 non-controlled single-arm studies, it was found that despite improvements in radiological detection rates over four decades, there were no changes in clinical outcomes. Therefore, advances in imaging alone may be insufficient to improve surgical outcomes for Cushing's disease.

Summary: In this study, clinical, biochemical, and molecular markers were evaluated to predict the long-term clinical outcome and remission in ACTHomas patients. The results showed that certain clinical variables and molecular markers were associated with tumor remission and relapsed disease. An integrative model combining selected clinical variables and molecular markers accurately predicted the clinical evolution and remission of patients with ACTHomas. These findings highlight the importance of using a combination of clinical and molecular biomarkers in the evaluation and follow-up of ACTHomas patients.
