What is dorso-lateral in the subthalamic Nucleus (STN)? - a topographic and anatomical consideration on the ambiguous description of today's primary target for deep brain stimulation (DBS) surgery

The most frequently used target for DBS in advanced Parkinson Disease (PD) is the sensorimotor subthalamic nucleus (STN), anatomically referred to as dorso-lateral STN [3]. Ambiguities arise, regarding the true meaning of this description in the STN. Does dorsal indicate posterior or superior? At its best, this definition assigns two directions in space to a three-dimensional structure. This paper evaluates the ambiguity and describes the sensorimotor part of the STN in stereotactic space.

Summary: SRS provides durable tumor control and acceptable rates of vision preservation in perioptic meningiomas. A prescription dose of >= 12 Gy is associated with improved tumor control, while a dose to the optic apparatus of >= 10 Gy and tumor progression are associated with post-SRS visual decline.

Summary: Trigeminal neuralgia in patients with multiple sclerosis is challenging to manage, often treated with Gamma Knife radiosurgery. This study evaluated the safety, efficacy, and durability of GKRS for TN in MS patients, finding short-term pain control was effective but long-term control was limited. Repeat GKRS was found to be safe and effective in prolonging pain relief.

Summary: The occurrence of symptomatic radiation necrosis (RN) in patients with renal cell carcinoma (RCC) brain metastases treated with immune checkpoint inhibitors (ICIs) and stereotactic radiosurgery (SRS) is relatively low, with most events being grade 1 to 3 and managed by medical means. Concurrent therapy with ICI/SRS does not appear to increase the risk of RN. Efforts to improve dose conformality may be the most successful strategy for mitigating RN in single-fraction SRS.

Summary: Gamma Knife radiosurgery (GKRS) can be used to treat drug-resistant cluster headache (CH). The outcomes of studies on GKRS for CH are inconsistent, and the ideal target and treatment parameters are unclear. This systematic review evaluates the safety and efficacy of GKRS for drug-resistant CH, showing high short-term pain reduction rates but controversial long-term results.

Summary: Stereotactic radiosurgery (SRS) is an effective treatment for pituitary adenomas, but the mechanism of delayed endocrine deficits is not well understood. This multicenter study aimed to evaluate radiation tolerance and identify predictors of delayed hypopituitarism after SRS for these tumors. The study found that tumor control was achieved in a high percentage of patients, but a significant number developed new loss of pituitary function, with certain factors such as tumor type, age, and radiation dose playing a role.
