Summary: Patients with refractory proctitis face challenges in finding effective therapies. This study reviewed various therapeutic strategies and their clinical efficacy and safety. Systemic therapies showed mixed remission rates but raised safety concerns, while topically applied tacrolimus demonstrated favorable clinical remission rates and safety. There is a lack of prospective clinical trials in this area, highlighting the need for further research.

Summary: Radiation-induced colitis and proctopathy (RICAP) is a recognized adverse effect of radiation therapy (RT) that can adversely affect cancer patients' quality of life. Our study found that acute RICAP (ARICAP) has non-bloody diarrhea as the predominant symptom, whereas chronic RICAP (CRICAP) has mostly bloody diarrhea. ARICAP patients more often received medical management, whereas CRICAP patients with bleeding more often received argon plasma coagulation (APC).

Summary: The serum level of leucine-rich alpha 2 glycoprotein (LRG) can serve as a biomarker for active ulcerative colitis (UC). In this study, it was found that LRG values were significantly elevated in patients with left-sided colitis and pancolitis, and could be used to predict patients with active UC, but not active proctitis.

Summary: This study evaluated the clinical characteristics and risk factors of chronic radiation proctitis (RP). The results showed that previous abdominopelvic surgery and total radiation dose were significant risk factors related to chronic RP. Total radiation dose was also related to chronic hemorrhagic RP. Argon plasma coagulation (APC) was effective and safe for the treatment of chronic hemorrhagic RP.

Summary: Radiation proctitis is a collateral damage to healthy cells during radiation treatment of pelvic malignancies. The study showed that colon organoid-based regeneration could successfully treat this condition by engrafting onto the damaged rectal mucosa of irradiated mice, with Lgr5+ stem cells being pivotal to the process. These findings provide insights into the potential use of colon organoid-based regenerative therapy for radiation proctitis.
