Chronic heart failure (CHF) and obesity are two conditions frequently associated and which, despite all the advances made in their management in the recent years, their prevalence continues to rise. Obese patients present unique challenges in the diagnosis of CHF and also therapeutic particularities. The genetic differences may be a possible explanation for the fact that some people, irrespective of their lifestyle and common classical cardiovascular risk factors, are more susceptible to develop heart failure. Moreover, the adipose tissue, a huge endocrine organ which secretes adipokines, is also a well-established source of all renin-angiotensin-aldosterone system components, being strongly involved in the pathogenesis of CHF. That is why this review will explore the possible links between the RAS genetic polymorphisms and leptin secretion in obese HF patients, trying to bring a more precise understanding of this relationship, which will undoubtedly facilitate a more appropriate treatment of HF in obese patients. We also try to explain the possible incriminated mechanisms, and plausible biological explanations for the relationship between RAS genetic polymorphisms and adipokines secretion in obese heart failure patients.

Antioxidant and Anti-Inflammatory Actions of Polyphenols from Red and White Grape Pomace in Ischemic Heart Diseases

Ioana Corina Bocsan, Dan Claudiu Magureanu, Raluca Maria Pop, Antonia Mihaela Levai, Stefan Octavian Macovei, Ioana Maria Patrasca, Veronica Sanda Chedea, Anca Dana Buzoianu

Summary: Grape pomace, a byproduct of the wine industry, is a reliable source of polyphenols with antioxidant and anti-inflammatory activities. Differences in antioxidant and anti-inflammatory activities exist between red and white grape pomace, with more focus on red grape pomace and the need for further research on white grape pomace.

Therapeutic Efficacy and Outcomes of Remdesivir versus Remdesivir with Tocilizumab in Severe SARS-CoV-2 Infection

Damiana-Maria Vulturar, Maria Adriana Neag, Stefan Cristian Vesa, Anca-Diana Maierean, Diana Gherman, Anca Dana Buzoianu, Olga Hilda Orasan, Doina-Adina Todea

Summary: This retrospective cohort study investigated the clinical outcomes of severe COVID-19 pneumonia patients treated with remdesivir alone or a combination of remdesivir and tocilizumab. The combination therapy showed improved inflammatory markers and decreased oxygen requirement, although it had a higher rate of superinfection compared to remdesivir alone.

Short-Chain Fatty Acids as Bacterial Enterocytes and Therapeutic Target in Diabetes Mellitus Type 2

Maria-Adriana Neag, Anca-Elena Craciun, Andreea-Ioana Inceu, Diana-Elena Burlacu, Cristian-Ioan Craciun, Anca-Dana Buzoianu

Summary: Diabetes mellitus is a multifactorial disease with gastrointestinal symptoms caused by the enteric nervous system, which functions independently of the central nervous system. Short-chain fatty acids (SCFAs), bacterial metabolites of gut microbiota, modulate the enteric nervous system and have beneficial effects on diabetes. However, the specific nutraceuticals to increase SCFAs and achieve these effects are uncertain. Further studies are needed to understand the modulation of gut dysbiosis with SCFAs in diabetes, as well as the interplay between gut microbiota and the enteric nervous system.

Safety of Vaccination within First Year of Life-The Experience of One General Medicine Center

Claudia Felicia Pop, Petronela Coblisan, Ligia Capalna, Paraschiva Chereches Panta, Anca Dana Buzoianu, Ioana Corina Bocsan

Summary: This study analyzed the immediate adverse reactions induced by vaccines included in the national program of immunization for the first year of life. The results showed that the rate of adverse events was lower than 30% of the total number of doses, and most of them were mild. The incidence and severity of vaccine-induced adverse effects were correlated with prematurity. The vaccine hesitancy rate was lower than the national one, possibly due to a high level of education and good information provided by doctors that might have led to parents' concerns regarding vaccination being answered.

The Tumor Suppressor Functions of Ubiquitin Ligase KPC1: From Cell-Cycle Control to NF-KB Regulator

Diana Gulei, Rares Drula, Gabriel Ghiaur, Anca Dana Buzoianu, Yelena Kravtsova-Ivantsiv, Ciprian Tomuleasa, Aaron Ciechanover

Summary: The ubiquitin-proteasome system (UPS) is crucial for protein degradation, and its dysregulation is closely related to malignant pathologies. KPC1, an E3 ubiquitin ligase component, plays a key role in cancer by regulating p27 signaling and the NF-xB pathway. KPC1 maintains the ubiquitination of cytoplasmic p27, influencing cell-cycle progression, and also induces the ubiquitination of p105 to control NF-xB signaling. Therapeutic reinforcement of the KPC1-p50 axis shows promising tumor suppressor activity in multiple malignancies.

Gut Molecules in Cardiometabolic Diseases: The Mechanisms behind the Story

Andreea-Ioana Inceu, Maria-Adriana Neag, Anca-Elena Craciun, Anca-Dana Buzoianu

Summary: Atherosclerotic cardiovascular disease is a leading cause of illness and death worldwide. Diabetes mellitus increases the risk of cardiovascular issues. Heart failure and atrial fibrillation are related comorbidities that have similar cardiovascular risk factors. The use of incretin-based therapies has shown potential in reducing atherosclerosis and heart failure risk through alternative signaling pathways. Gut-derived molecules, gut hormones, and gut microbiota metabolites have both positive and negative effects on cardiometabolic disorders. Inflammation plays a crucial role, but other intracellular signaling pathways may also contribute to the observed effects. Understanding the molecular mechanisms involved could lead to new therapeutic strategies and improved knowledge of the gut-metabolic syndrome-cardiovascular disease relationship.

The Effect of Vitamin D Supplementation on the Length of Hospitalisation, Intensive Care Unit Admission, and Mortality in COVID-19-A Systematic Review and Meta-Analysis

Alexandru Constantin Sirbu, Octavia Sabin, Ioana Corina Bocsan, Stefan Cristian Vesa, Anca Dana Buzoianu

Summary: The COVID-19 pandemic has become a global health crisis, and researchers have been exploring treatments to improve patient outcomes. Vitamin D, known for its role in the immune system, has been suggested as a potential treatment for COVID-19. A systematic review and meta-analysis of 13 randomized controlled trials were conducted, indicating that high-dose vitamin D supplementation may reduce hospital stay and ICU admission rates for COVID-19 patients. However, the overall effect on mortality was not statistically significant. Further large-scale trials are needed to provide more robust evidence.

Summary: This study examined the effects of pruritus intensity on quality of life and depression in psoriasis patients, as well as the relationships between anger, self-esteem, and depression. The study also compared the differences between early and late onset psoriasis patients. The results showed that psoriasis patients had lower self-esteem and higher levels of anger and depression. Patients with early onset had higher depression scores and lower quality of life. Self-esteem was negatively correlated with depression, anger, disability severity, and disease duration.
