Objective - The primary aim of this study was to assess the effect of glucometabolic disorders on coronary collateral vessels in patients with occlusive coronary artery disease. Methods and results - Hundred and ninety-five consecutive patients with at least single-vessel occlusion were enrolled in this study prospectively. The standard oral glucose tolerance test was performed according to the criteria of the World Health Organization. Collateral circulation was graded according to the Rentrop classification. The mean Rentrop scores in normal glucose tolerance (NGT), impaired glucose tolerance (IGT), and diabetes were 1.40 +/- 1.02, 1.05 +/- 0.84, 1.00 +/- 0.98, respectively (P = 0.043). The percentage of patients without collateral circulation (Rentrop-0) was greatest in the diabetic group (44.4%), while the percentage was 21.8% in the IGT group and 22.0% in the NGT group. Ninety-five patients with at least one totally occluded coronary artery were analysed as a subgroup. In the totally occluded artery subgroup postprandial glycaemia was the only parameter that was associated with the Rentrop score in the univariate analysis (r = -0.34, P = 0.002) Conclusions - In conclusion, our study results, which are in agreement with previous results, indicate that not only diabetic glucose tolerance but also impaired glucose tolerance has an adverse impact on the development of coronary collaterals.

The fifth redo operation for mitral paravalvular leakage and free- floating closure device extraction: A case report

Yasin Ozden, Yavuz Sensoz, Mehmet Eren, Osman E. Karpuzoglu, Ilyas Kayacioglu

Summary: Paravalvular leakage is a serious complication of prosthetic valve surgery, and it can be treated through surgical or transcatheter methods. This article presents a case where a closure device freely passed through the paravalvular leakage from the ventricle to the atrium, following multiple surgical valve replacements and transcatheter device closures due to infective endocarditis.

Summary: The study did not find a significant correlation between epicardial fat thickness and post-procedural left bundle branch block, right bundle branch block, paravalvular aortic regurgitation and pacemaker implantation rates. Further studies with larger sample numbers are needed to explore the relationship.

Summary: Eight new compounds (7a-h) based on triazole compounds containing ester groups were synthesized with high yields. The structures of the synthesized compounds were determined using various spectroscopic methods. Antioxidant, anticancer, and alpha-amylase enzyme inhibition activities of the synthesized compounds were investigated. Compound 7g showed significant activity in both alpha-amylase inhibition and anticancer assays.

Carotid artery bypass versus endarterectomy as an alternative treatment of carotid artery stenosis: A propensity score matching analysis

Bulent Mert, Fatma Tugba Ilal Mert, Kamil Boyacioglu, Irfan Sahin, Berk Ozkaynak

Summary: This study aimed to evaluate the reliability and durability of saphenous vein bypass grafting (CBG) as an alternative treatment for carotid artery stenosis. The outcomes of 459 patients who underwent carotid endarterectomy (CEA) or CBG were analyzed. The results showed no significant differences in the survival, stroke-free survival, and restenosis-free survival distributions between the two groups. It is reasonable to consider CBG as an alternative surgical option for carotid artery stenosis when adequate disease-free inflow and outflow segments are present.

Summary: Eight new hybrid structures containing sulfonamide and 1,2,3-triazole units were designed and synthesized. The anticancer, antioxidant, and cholinesterase activities of these hybrid compounds were investigated. The compounds exhibited higher antioxidant activity and better cytotoxic effects compared to standard drugs, and also showed improved AChE inhibitory activity.

Summary: Aortic valve stenosis is the most common valvular disease, and surgical or transcatheter aortic valve replacement is the treatment options. In this study, the researchers found that the increase in MPV after TAVR may be an indicator of adverse outcomes within 1 month.

Summary: This study evaluated the predictive ability of the MAGGIC score for short and long-term mortality after coronary artery bypass grafting (CABG) compared to the EuroSCORE II and STS scoring systems. The results showed that the MAGGIC score had good accuracy in predicting 30-day, one-year, and 10-year mortality, and was an independent predictor associated with mortality in follow-up.

The association between hemogram parameters and the development of contrast-induced nephropathy in patients presenting with non-ST-elevation myocardial infarction

Summary: This study aimed to investigate the role of hemogram parameters in predicting the development of contrast-induced nephropathy (CIN) in patients with non-ST segment elevation myocardial infarction (non-STEMI) treated with percutaneous coronary intervention (PCI). The results showed that age, diabetes, congestive heart failure, volume of contrast medium, mean platelet volume (MPV), and neutrophil/lymphocyte ratio (NLr) were independent risk factors associated with CIN development. Additionally, MPV and NLr were demonstrated as predictors of CIN development in non-STEMI patients treated with PCI.

Predictive Value of CHA2DS2 -VASc Score in Patients with Contrast-Induced Nephropathy After Primary Percutaneous Coronary Intervention for ST-Elevated Myocardial Infarction

Summary: This study evaluated the CHA2DS2-VASc score as a surrogate for contrast-induced nephropathy and its relationship with major adverse cardiac events during hospitalization. The study found that age, diabetes, history of stroke, volume of contrast medium, left ventricular ejection fraction, and CHA2DS2-VASc score were associated with contrast-induced nephropathy development. A CHA2DS2-VASc score of >=2.5 was associated with high sensitivity and specificity in predicting contrast-induced nephropathy development.
