Indoleamine 2,3-dioxygenase depletes tryptophan, activates general control non-derepressible 2 kinase and down-regulates key enzymes involved in fatty acid synthesis in primary human CD4+ T cells

Indoleamine 2,3-dioxygenase (IDO) is expressed in antigen-presenting cells and exerts immunosuppressive effects on CD4(+) T cells. One mechanism is through the inhibition of aerobic glycolysis. Another prerequisite for T-cell proliferation and differentiation into effector cells is increased fatty acid (FA) synthesis. The effect of IDO on enzymes involved in FA synthesis was evaluated in primary human cells both in mixed lymphocyte reactions in the presence or not of the IDO inhibitor 1-DL-methyl-tryptophan, and in stimulated CD4(+) T cells in the presence or not of the general control non-derepressible 2 (GCN2) kinase activator tryptophanol (TRP). IDO or TRP inhibited cell proliferation. By assessing the level of GCN2 kinase or mammalian target of rapamycin complex 1 substrates along with a kynurenine free system we showed that IDO exerts its effect mainly through activation of GCN2 kinase. IDO or TRP down-regulated ATP-citrate lyase and acetyl coenzyme A carboxylase 1, key enzymes involved in FA synthesis. Also, IDO or TRP altered the expression of enzymes that control the availability of carbon atoms for FA synthesis, such as lactate dehydrogenase-A, pyruvate dehydrogenase, glutaminase 1 and glutaminase 2, in a way that inhibits FA synthesis. In conclusion, IDO through GCN2 kinase activation inhibits CD4(+) T-cell proliferation and down-regulates key enzymes that directly or indirectly promote FA synthesis, a prerequisite for CD4(+) T-cell proliferation and differentiation into effector cell lineages.

Therapeutic Advances in Diabetic Kidney Disease

Summary: Although initially introduced as glucose-lowering medications, it was discovered that the most important treatment effect of SGLT-2 inhibitors is cardiorenal protection. The benefits of SGLT-2 inhibitors on kidney and cardiovascular outcomes in patients with chronic kidney disease (CKD), regardless of the presence or absence of Type 2 diabetes (T2D), have been consistently shown in landmark trials. Additionally, the nonsteroidal MRA finerenone has effectively improved cardiorenal outcomes in a large Phase 3 clinical trial program that included a wide spectrum of CKD patients with T2D.

Effects of Home-Based Exercise Training on Cardiac Autonomic Neuropathy and Metabolic Profile in Diabetic Hemodialysis Patients

Summary: This study aimed to investigate the effects of a home-based exercise training program on Cardiac Autonomic Neuropathy (CAN) and metabolic profile in Diabetic Kidney Disease (DKD) patients undergoing maintenance hemodialysis (HD). The results showed that after a 6-month home-based exercise training program, the EX group had significant improvements in HDL, VO(2)peak, SDNN, pNN50, TS index, HbA1c, and LF. Linear regression analysis showed that VO(2)peak was positively correlated with SDNN and HF. Multiple regression analysis indicated that the improvement of sympathovagal balance and aerobic capacity depended on patients' participation in exercise training.

Effects of a Combined Intradialytic Exercise Training Program on Functional Capacity and Body Composition in Kidney Transplant Candidates

Summary: This study aimed to investigate the effects of a 4-month intradialytic exercise program on the functional capacity and body composition of kidney transplant candidates. The results showed that the exercise program improved grip strength and walking distance, increased muscle mass and body water, and reduced body fat. This is important for improving the quality of life of patients and the success rate of kidney transplant surgery.

Routes of Albumin Overload Toxicity in Renal Tubular Epithelial Cells

Summary: This study evaluated the response of renal proximal tubular epithelial cells (RPTECs) to high albumin concentration, focusing on the unfolded protein response (UPR) and DNA damage response (DDR). The results showed that albumin led to reactive oxygen species (ROS) overproduction and protein modification, which activated both UPR and DDR. This resulted in cellular apoptosis, senescence, and epithelial-to-mesenchymal transition (EMT). While agents against endoplasmic reticulum stress (ERS) only partially alleviated the harmful effects, inhibiting ROS upregulation prevented both UPR and DDR. Therefore, suppressing ROS overproduction may be more effective in mitigating the deleterious effects of albumin overload.

Heart Failure in Patients with Chronic Kidney Disease

Summary: The function of the kidney is closely linked to the function of the heart. Dysfunction/disease of the kidney may initiate or worsen cardiac dysfunction/disease, and vice versa. Chronic kidney disease can affect cardiac function through altered hemodynamics and salt and water retention, leading to heart failure. However, the management of heart failure in the context of kidney disease is challenging due to complex interactions and effects on medication metabolism.

Summary: The study examined the association between polymorphisms of the interleukin-1 gene cluster and the risk of progressive IgAN. A meta-analysis of genetic association studies was conducted to further investigate the involvement of interleukin genes in IgAN. The results suggest that genetic variants and haplotypes in the IL1B, IL1RN, and IL10 genes may contribute to an increased risk for the development and progression of IgAN.
