Long-Term Clinical and Immunological Profile of Kidney Transplant Patients Given Mesenchymal Stromal Cell Immunotherapy

We report here the long-term clinical and immunological results of four living-donor kidney transplant patients given autologous bone marrow-derived mesenchymal stromal cells (MSCs) as part of a phase 1 study focused on the safety and feasibility of this cell therapy. According to study protocols implemented over time, based on initial early safety findings, the patients were given MSC at day 7 posttransplant (n = 2) or at day -1 pretransplant (n = 2) and received induction therapy with basiliximab and low-dose rabbit anti-thymocyte globulin (RATG) or RATG alone, and were maintained on low-dose ciclosporin (CsA)/mycophenolate mofetil (MMF). All MSC-treated patients had stable graft function during the 5-to 7-year follow-up, without increased susceptibility to infections or neoplasm. In three MSC recipients, but not historical control patients, circulating memory CD8(+) T cell percentages remained lower than basal, coupled with persistent reduction of ex vivo donor-specific cytotoxicity. Two patients showed a long-lasting increase in the regulatory T cell/memory CD8(+) T cell ratio, paralleled by high circulating levels of naive and transitional B cells. In one of these two patients, CsA was successfully discontinued, and currently the low-dose MMF monotherapy is on the tapering phase. The study shows that MSC therapy is safe in the long term and could promote a pro-tolerogenic environment in selected patients. Extensive immuno-monitoring of MSC-treated kidney transplant recipients could help selection of patients for safe withdrawal of maintenance immunosuppressive drugs (NCT00752479 and NCT02012153).

Summary: Assessing the burden of tuberculosis by sex is crucial. There are significant differences in mortality and incidence rates between males and females among HIV-negative individuals globally, with higher rates among males. Among HIV-positive individuals, females have a higher mortality and incidence rates. Controlling risk factors such as smoking, alcohol use, and diabetes is important in reducing tuberculosis mortality among males.

A comprehensive report of long-term stability data for a range ATMPs: A need to develop guidelines for safe and harmonized stability studies

Summary: This report aims to provide evidence-based information for stability studies on ATMPs and promote harmonization of practices among laboratories. Through the analysis of stability studies on 19 different cell-based experimental ATMPs, it was found that these cell products are highly stable when stored at low temperatures for an extended period and show no decreased viability or efficacy. The report suggests the development of new guidelines specific to ATMPs to reduce the costs of stability studies.

Summary: In this study, the center optimized the method of T cell expansion in G-Rex devices and demonstrated its efficiency and safety in expanding CIK and BET cells. The results showed that the expanded cells exhibited strong cytotoxicity against tumor cells and displayed therapeutic activity in vivo. This research provides a valuable approach for large-scale T cell expansion in clinical studies.

Summary: Glomerular hyperfiltration can be categorized as 'absolute' hyperfiltration and 'relative' hyperfiltration. 'Absolute' hyperfiltration occurs in the context of a normal number of functioning nephrons and can be seen in high protein intake, pregnancy, diabetes, obesity, or polycystic kidney disease. 'Relative' hyperfiltration occurs when the number of functioning nephrons is reduced and can be seen in patients with a congenitally reduced nephron number or acquired reduction in nephron mass due to surgery or kidney disease.

Home as the new frontier for the treatment of COVID-19: the case for anti-inflammatory agents

Summary: COVID-19 exhibits varying degrees of symptom severity, requiring different levels of care at different stages of the disease. Intervening early with anti-inflammatory drugs, especially NSAIDs, in the outpatient setting could prevent disease progression and long-term complications. Observational studies have shown promising results regarding the use of NSAIDs for early outpatient treatment of COVID-19.

Long-term adaptive response in COVID-19 vaccine recipients and the effect of a booster dose

Summary: We examined the immune response in individuals previously infected with SARS-CoV2 and those without prior infection after 9 months of primary vaccination and 3 months after receiving a booster dose. Previously infected individuals showed higher levels of residual antibodies and significant neutralizing activity against different variants compared to infection-naive individuals at 9 months after primary vaccination. The higher immune response was associated with higher levels of receptor binding domain (RBD)-specific IgG(+) and IgA(+) memory B cells. The booster dose did not increase neutralizing activity or the frequencies of B and T cells. Conversely, infection-naive individuals required the booster dose to achieve comparable levels of neutralizing antibodies observed in previously infected individuals after primary vaccination. The neutralizing titer correlated with IFN gamma producing T cells specific to the RBD, despite sustained B cell response. Importantly, pre-pandemic samples showed high cross-reactivity to Omicron. These data highlight the significance of the booster dose in reinforcing immunological memory and increasing circulating antibodies in infection-naive individuals.

Summary: Induced pluripotent stem cells (iPSC) have the potential for cell therapy due to their self-renewal and differentiation capabilities. However, the high production costs and limited use for acute conditions hinder their applications. An allogeneic iPSC-based strategy may overcome these issues, but immune rejection is a concern. In this study, CRISPR/Cas9 was used to create hypoimmunogenic iPSCs that can differentiate into liver cells, providing a cost-effective and off-the-shelf opportunity for liver disease cell therapy.

Summary: Immune dysregulation plays a key role in the pathogenesis of steroid-dependent/frequently relapsing nephrotic syndrome (SDNS/FRNS). However, no major B- or T-cell alterations have been described for adults, unlike in pediatric cases. This study found that patients with SDNS/FRNS have expansion of memory B cells and reduced memory Tregs. The levels of CD45RO(+) Tregs at baseline may help predict which patients will achieve sustained remission following rituximab infusion.
