Use of Interleukin-1 Blockers in Pericardial and Cardiovascular Diseases

Purpose of Review This review aims to summarize the role of the interleukin-1 (IL-1) blocking agents in cardiovascular diseases, briefly describing the pathogenetic rationale and the most relevant clinical studies. Recent Findings IL-1 is a pivotal cytokine of the innate immune system. Anti-IL-1 agents are currently used for the treatment of several autoimmune and autoinflammatory conditions. Recently, the role of IL-1 has also emerged in cardiovascular diseases. Indeed, two recent randomized controlled trials have shown that the IL-1 receptor antagonist anakinra is effective for the treatment of idiopathic recurrent pericarditis and the IL-1 beta blocking agent canakinumab is effective in reducing myocardial infarction in people at risk. Interestingly, interfering with IL-1 has proved to be also effective in other cardiovascular manifestations, such as myocarditis, arrhythmias, and heart failure. Summary Blocking the IL-1 pathway is a possible new therapeutic strategy, potentially leading to innovative therapies in many acute and chronic cardiovascular diseases.

Pediatric Scleritis: An Update

Summary: Posterior idiopathic scleritis is the most common type of scleritis observed in childhood, but anterior and necrotizing inflammatory scleritis can also occur. While less common than in adults, systemic disorders can be associated with scleral inflammation in children and should be promptly recognized and treated to prevent complications. A multidisciplinary diagnostic work-up is necessary to rule out infectious and autoimmune causes and tailor treatment to the specific patient.

Summary: Autoimmune rheumatic diseases (ARD) pose challenges to reproductive health. Preconception counseling should consider factors such as fertility, disease impact on pregnancy outcomes, disease activity during pregnancy, medication compatibility, and biomarkers predictive of adverse pregnancy outcomes (APO). Diseases like preeclampsia and pregnancy-induced hypertension are more likely to occur in women with ARD. Placental insufficiency can result in restricted fetal growth. Treatment with compatible drugs is essential for maternal and fetal wellbeing. While limited data is available on medication usage during pregnancy, TNF inhibitors are recommended. Non-medical issues like sexual dysfunction and long-term outcomes of children also require attention in this review. Collaborative research and pregnancy registries will contribute to future insights.

The findings revealed a high prevalence of inappropriate use of sulfonylureas in the prescription of anti-diabetes drugs.

Summary: The potential role of Streptococcus salivarius K12 (SSK12) in controlling febrile flares in patients with Periodic Fever, Aphthous stomatitis, Pharyngitis, and cervical Adenitis (PFAPA) syndrome was evaluated. The results showed that SSK12 significantly reduced the number and duration of febrile flares, lowered body temperature, provided a steroid-sparing effect, and reduced accompanying symptoms related to the syndrome.

Summary: This study assessed the efficacy of belimumab in treating joint and skin manifestations in a nationwide cohort of SLE patients. The results showed that belimumab significantly improved joint and skin symptoms and reduced the use of glucocorticoids. A significant proportion of patients who did not achieve complete remission initially achieved remission during follow-up.

Summary: This cohort study compares a systemic phenotype of pericarditis with other forms of pericarditis. Through the analysis of 412 patients with idiopathic recurrent pericarditis enrolled in a registry from 2019 to 2022, it was found that 211 patients presented the systemic subset. The systemic subset was characterized by higher age, CRP values, pericardiocentesis rate, leukocyte count, neutrophil count, and lower lymphocyte count.

Patients with skin disease at the onset are likely to have persistent cutaneous involvement thereafter; mucocutaneous lesions observed at the onset, especially PF, could be a warning sign for future persistent skin involvement requiring closer dermatological care.

Color Doppler Eye Ultrasonography in giant cell arteritis: differential diagnosis between arteritic and non-arteritic sudden blindness

Summary: The aim of this study was to assess the role of Color Doppler Eye Ultrasound (CDEUS) in the differential diagnosis between arteritic and non-arteritic blindness. The results showed that CDEUS displayed good reliability in distinguishing between arteritic and non-arteritic central retinal artery occlusion (CRAO), but no significant difference was found in arteritic and non-arteritic anterior ischemic optic neuropathy (AION). Therefore, CDEUS is not a reliable procedure in the diagnostic work-up of giant cell arteritis (GCA) and should be restricted to the exclusion of thrombo-embolic occlusions within the territory of central retinal artery.

Summary: The objective of this study was to validate the PEDiatric Behcet's Disease classification criteria (PEDBD) using an evidence-based approach. The results showed that the PEDBD criteria were highly specific, while the ICBD criteria were more sensitive. Further research is needed to evaluate the performance of the criteria.
