A systematic review of health economic models and utility estimation methods in schizophrenia

Introduction: There is a growing need for economic evaluations describing the disease course, as well as the costs and clinical outcomes related to the treatment of schizophrenia.Areas covered: A systematic review on studies describing health economic models in schizophrenia and a targeted literature review on utility mapping algorithms in schizophrenia were carried out. Models found in the review were collated and assessed in detail according to their type and various other attributes. Fifty-nine studies were included in the review. Modeling techniques varied from simple decision trees to complex simulation models. The models used various clinical endpoints as value drivers, 47% of the models used quality-adjusted life years, and eight percent used disability-adjusted life years to measure benefits, while others applied various clinical outcomes. Most models considered patients switching between therapies, and therapeutic adherence, compliance or persistence. The targeted literature review identified four main approaches to map PANSS scores to utility values.Expert commentary: Health economic models developed for schizophrenia showed great variability, with simulation models becoming more frequently used in the last decade. Using PANSS scores as the basis of utility estimations is justifiable.

Summary: The burden of obesity includes significant economic, humanistic, and clinical impacts in Egypt, leading to the development of various serious diseases such as diabetes, hypertension, sleep apnea, and fatty liver. Obesity contributes to around 115 thousand deaths annually, with about 4 million DALYs and an economic burden of approximately 62 billion Egyptian pounds per year.

A mixed methods approach was employed, and the main barriers were summarized in five groups: technical, regulatory, clinical, scientific, and perceptional. Further research should focus on developing consensus-based guidelines to improve the harmonization and standardization of real-world evidence.

Summary: The aim of this study was to review economic evaluations of health technologies in multiple myeloma (MM) and provide recommendations for future analyses. A systemic literature review was conducted and it was found that most economic analyses were from high-income countries, while evaluations from middle-income countries were rare. Diagnostic technologies and integrated care were rarely modelled, and there was a lack of evaluations regarding minimal residual disease (MRD).

The results indicate a significant burden of atopic dermatitis on the economy and quality of life in Saudi Arabia.

Summary: This study explores the impact of adopting Health Technology Assessment (HTA) on middle-income countries (MICs). The results show that the positive impacts of HTA implementation outweigh the negative impacts. The main benefit of HTA implementation is improving the transparency and accountability of healthcare decisions. Therefore, HTA implementation plays an important role in the healthcare systems of MICs.

Summary: This study estimated the economic and humanistic burden of atopic dermatitis in adults and adolescents in seven countries in the Middle East and Africa. The results showed that atopic dermatitis significantly affects the quality of life of patients. The disease consumes a certain proportion of healthcare expenditure in each country, and the indirect cost represents a certain percentage of the gross domestic product. Controlling atopic dermatitis can reduce costs, especially indirect costs, and significantly alleviate the disease burden.
