Percutaneous coronary intervention in left main coronary artery disease: the 13th consensus document from the European Bifurcation Club

The 2017 European Bifurcation Club (EBC) meeting was held in Porto (Portugal) and allowed a multidisciplinary international faculty to review and discuss the latest data collected in the field of coronary bifurcation interventions. In particular, the topic of percutaneous coronary intervention (PCI) on left main coronary artery (LM) disease was highlighted as a contemporary priority. Herein, we summarise the key LM anatomy features, the diagnostic modalities and available data that are relevant for a patient's procedural management. Since the clinical outcomes of patients undergoing PCI on LM disease may depend on both PCI team organisation and PCI performance, the optimal catheterisation laboratory set-up and the rationales for device and technique selection are critically reviewed. The best lesion preparation modalities, the different DES implantation technique choices and the strategies to be considered during PCI on unprotected LM for optimal PCI results are reviewed step by step.

Summary: This study aimed to analyze the impact of technical variables on procedural outcomes and periprocedural myocardial infarction following left main bifurcation intervention. The results showed that the use of jailed wires was associated with successful rewiring of jailed vessels, and the use of jailed wires was not associated with increased risk of periprocedural myocardial infarction. Side vessel preparation in provisional cases was linked to increased side vessel dissection requiring stenting. The use of noncompliant balloons for proximal optimization technique was associated with a reduction in the need for additional side vessel intervention and periprocedural myocardial infarction.

Summary: This study evaluated the long-term valve function and incidence of severe structural valve deterioration (SVD) among patients >=10 years post-TAVI and with at least 5 years of echocardiographic follow-up. The study found that the incidence of severe SVD was 5.9% in patients with echocardiographic follow-up exceeding 10 years. The incidence of severe SVD was higher in patients with balloon-expandable valves compared to self-expanding valves.

Management of failed stenting of the unprotected left main coronary artery

Summary: Percutaneous coronary intervention (PCI) is increasingly used to treat unprotected left main coronary artery (ULMCA) disease in patients who cannot undergo cardiac surgery. However, treating failed stents in ULMCA is complex and poses unique challenges. Intracoronary imaging has provided new insights into mechanisms of stent failure and treatment options have significantly improved. This study provides an overview of ULMCA stent failure and proposes a tailored algorithm for best management.

Cardiac hypertrophy simulations using parametric and echocardiography-based left ventricle model with shell finite elements

Summary: In this study, we simulated cardiac hypertrophy using shell elements in parametric and echocardiography-based left ventricle (LV) models. The effects of hypertrophy on wall thickness, displacement field and overall heart function were evaluated. Both eccentric and concentric hypertrophy were computed, and changes in ventricle shape and wall thickness were tracked. Our model provides insight into hypertrophy development in realistic heart geometries and can be used to test medical hypotheses regarding hypertrophy evolution.

Dynamic assessment of the left main-left circumflex bending angle: Implications for ostial left circumflex artery in-stent restenosis after successful two-stent PCI

Summary: This study found that two-stent techniques for left main bifurcation lesions are associated with an increased risk of in-stent restenosis at the ostium of the left circumflex artery. The study suggests that the cyclic change of the left main-left circumflex bending angle may be associated with the risk of ostial left circumflex in-stent restenosis.

Summary: This study assessed the association between periprocedural major adverse events (MAE) and subsequent mortality after percutaneous coronary intervention (PCI) and coronary artery bypass grafting (CABG) for left main coronary artery revascularization. The study found that nonfatal periprocedural MAE strongly predicted both early and late mortality after both PCI and CABG.

Summary: Coronary stent underexpansion is associated with restenosis and stent thrombosis. High wall shear stress (WSS) has been linked to prothrombotic pathways activation, matrix metalloproteinases upregulation, and future myocardial infarction in atherosclerosis studies. This study aimed to investigate the relationship between stent underexpansion and WSS. The results showed that frames with stent underexpansion were more likely to exhibit high WSS, while no significant association was found between underexpansion and low WSS or between underexpansion and stent platform.

Summary: In large non-left main true bifurcation lesions, the use of a systematic culotte strategy showed no benefit over provisional stenting for the composite outcome of all-cause mortality, myocardial infarction, or target vessel revascularisation at 5 years. The stepwise provisional approach may be considered preferable for the majority of true coronary bifurcation lesions.

Summary: This study evaluated the use of 2-stent techniques in coronary bifurcation PCI and analyzed the procedural factors associated with suboptimal results. The results showed that commonly used 2-stent techniques may result in imperfect stent configurations. More frequent use of optimization techniques and adherence to expert recommendations may reduce the occurrence of suboptimal stent configurations after PCI.
