Viruses are the most common causes of respiratory infection. The imaging findings of viral pneumonia are diverse and overlap with those of other nonviral infectious and inflammatory conditions. However, identification of the underlying viral pathogens may not always be easy. There are a number of indicators for identifying viral pathogens on the basis of imaging patterns, which are associated with the pathogenesis of viral infections. Viruses in the same viral family share a similar pathogenesis of pneumonia, and the imaging patterns have distinguishable characteristics. Although not all cases manifest with typical patterns, most typical imaging patterns of viral pneumonia can be classified according to viral families. Although a definite diagnosis cannot be achieved on the basis of imaging features alone, recognition of viral pneumonia patterns may aid in differentiating viral pathogens, thus reducing the use of antibiotics. Recently, new viruses associated with recent outbreaks including human metapneumovirus, severe acute respiratory syndrome coronavirus, and Middle East respiratory syndrome coronavirus have been discovered. The imaging findings of these emerging pathogens have been described in a few recent studies. This review focuses on the radiographic and computed tomographic patterns of viral pneumonia caused by different pathogens, including new pathogens. Clinical characteristics that could affect imaging, such as patient age and immune status, seasonal variation and community outbreaks, and pathogenesis, are also discussed. The first goal of this review is to indicate that there are imaging features that should raise the possibility of viral infections. Second, to help radiologists differentiate viral infections, viruses in the same viridae that have similar pathogenesis and can have similar imaging characteristics are shown. By considering both the clinical and radiologic characteristics, radiologists can suggest the diagnosis of viral pneumonia. (C) RSNA, 2018.

Short versus prolonged courses of antimicrobial therapy for patients with uncomplicated Pseudomonas aeruginosa bloodstream infection: a retrospective study

Moonsuk Bae, Yunseo Jeong, Seongman Bae, Min Jae Kim, Yong Pil Chong, Sung-Han Kim, Sang-Oh Lee, Sang-Ho Choi, Yang Soo Kim, Jiwon Jung

Summary: The study found that in patients with uncomplicated Pseudomonas aeruginosa bloodstream infection, there was no significant difference in recurrence rates or 30-day mortality between short-course and prolonged-course antimicrobial therapy. Prolonged therapy did not significantly reduce the risk of P. aeruginosa infection recurrence within 180 days compared to short-course therapy.

Efficacy of Doxycycline for Mild-to-moderate Community-acquired Pneumonia in Adults: A Systematic Review and Meta-analysis of Randomized Controlled Trials

Sang Ho Choi, Antoni Cesar, Timothy Arthur Chandos Snow, Naveed Saleem, Nishkantha Arulkumaran, Mervyn Singer

Summary: This study found that the efficacy of doxycycline is comparable to macrolides or fluoroquinolones in adult patients with mild-to-moderate community-acquired pneumonia (CAP), suggesting that it is a viable treatment option. However, larger trials are needed to confirm these findings due to the lack of recent studies.

Summary: The clinical significance of Clostridium tertium bacteremia remains uncertain. This study evaluated the incidence, clinical characteristics, and outcomes of C. tertium bacteremia in neutropenic and non-neutropenic patients. It was found that 10% of patients with Clostridium species bacteremia had C. tertium bacteremia, with a higher prevalence in patients with recent chemotherapy and neutropenia or hematologic malignancy.

Summary: This study evaluated the rate and composition of bacterial co-infection in COVID-19 patients and assessed the association between the use of empirical antibacterials and mortality. The results showed that bacterial co-infection was infrequent in the patients, and the co-infected organisms varied depending on the previous use of antibacterial drugs.

Summary: This study retrospectively analyzed the clinical characteristics and outcomes of invasive and non-invasive fusariosis in South Korea. Invasive fusariosis mainly affected immunocompromised patients and was associated with a poor prognosis. Non-invasive fusariosis was common in patients with diabetes and solid cancer.

Moonsuk Bae, Joon Seon Song, Ji Yeun Kim, Seongman Bae, Jiwon Jung, Min Jae Kim, Yong Pil Chong, Sang-Oh Lee, Sang-Ho Choi, Yang Soo Kim, Sung-Han Kim

Summary: This study investigated the molecular evidence of invasive mould pneumonia in lung tissues of patients diagnosed with organising pneumonia (OP) and suspected invasive mould pneumonia. The results showed that 6 out of 15 lung tissue samples (40%) exhibited molecular evidence of invasive mould infection.
