The aim of this study was to prove whether Ginkgo biloba food supplements on the European market comply with pharmaceutical quality, and whether their composition satisfies the European Pharmacopoeia criteria. Medicinal products containing a standardised Ginkgo leaf extract are used for the improvement of cognitive impairment and quality of life in mild dementia. Further, Ginkgonis folium is used for the treatment of peripheral circulation disorders. Pharmacopoeial Ginkgo dry extract contains 22.0-27.0% flavonoids and 5.4-6.6% terpene lactones (ginkgolides, bilobalide). In addition to its widespread use as an herbal medicine (herbal medicinal product), the same extract can be an ingredient in food supplements. The content of active secondary metabolites was quantified in a number of European food supplements containing Ginkgo dry extract or Ginkgo leaf. Flavonoids were quantified using a modified pharmacopoeial HPLC-UV method, and terpene lactones (ginkgolides A, B, C, and bilobalide) using LC-MS/MS. Some Ginkgo leaf supplement samples were also analysed by microscopy. The quality of food supplements on the European market is dubious. In this paper, we present selected examples of several methods of adulteration and falsification, including higher/lower doses of Ginkgo dry extract or Ginkgo leaf than declared and the addition of undeclared extraneous materials. These examples reveal several patterns in the manufacturing of adulterated products.

Trends in opioid utilization in Hungary, 2006-2020: A nationwide retrospective study with multiple metrics

Zsofia Engi, Ria Benko, Gyongyver Soos, Delia Szok, Melinda Csenki, Emese Csullog, Attila Balog, Dezso Csupor, Reka Viola, Peter Doro, Maria Matuz

Summary: This study analyzed the utilization of opioids in ambulatory care in Hungary from 2006 to 2020. The study found that the overall utilization of opioids increased during this period, driven by an increase in both weak and strong opioids. Tramadol was the most commonly used opioid, and oral administration was the dominant route of administration. The study also revealed an increase in opioid use for musculoskeletal pain and a significant decline in opioid use for cancer pain.

Summary: Based on a meta-analysis of available clinical trials, we evaluated the efficacy of ivermectin in terms of time to viral clearance. The results showed that ivermectin treatment significantly reduced the time to viral clearance in mild to moderate COVID-19 cases compared to control groups. However, more studies are needed to improve the evidence of ivermectin use in COVID-19.

Neuropharmacological Potential of Diterpenoid Alkaloids

Arash Salehi, Mustafa Ghanadian, Behzad Zolfaghari, Amir Reza Jassbi, Maryam Fattahian, Parham Reisi, Dezso Csupor, Ikhlas A. Khan, Zulfiqar Ali

Summary: This study provides a narrative review of diterpenoid alkaloids (DAs), a family of extremely important natural products found predominantly in some species of Aconitum and Delphinium (Ranunculaceae). DAs have long been a focus of research attention due to their numerous intricate structures and diverse biological activities, especially in the central nervous system (CNS). Although the role of tertiary nitrogen in ring A and the polycyclic complex structure are of great importance in drug-receptor affinity, recent advances in developing new drugs from DAs were insignificant due to their neurotoxicity.
