Introduction: HPV-driven oropharyngeal cancer (OPC) patients have a better prognosis than their HPV-negative counterparts but several studies have suggested that among HPV-positive patients those with a smoking history had worse oncological outcomes. The aim of our study is to characterize the interplay between tobacco consumption, patient and disease characteristics, and disease control. Materials and methods: All patients diagnosed with HPV-driven OPC and treated with curative intent between 2007 and 2009 and 2011-2016 at Gustave Roussy cancer center were included (n = 282). Demographic, clinical, morphological and tobacco consumption were correlated with oncologic outcomes. Results: 157 (56%) patients had a positive smoking history, including 23.8% who were smoking at the time of diagnosis and 37.6% who had a tobacco consumption exceeding 20 pack-years. In multivariate analysis, the strongest prognostic factor for survival was smoking status at cancer diagnosis, with a hazard ratio (HR) for nonsmokers compared to smokers of 0.25 ([0.12, 0.50], p = 0.0001). Smoking history, either more than 20 pack-years or smoking at diagnosis, was associated with local relapse and distant relapse. There was no difference in terms of comorbidity (p = 0.32) and radiotherapy duration (p = 0.93) according to tobacco consumption. Discussion: Smoking is frequent among patients with HPV-driven OPC and increases the risk of death and oncologic failure.

Summary: The study revealed that P16 tumor status is the main predictive factor of synchronous primary neoplasia (SPN) in OPSCC patients. Patients with SPN were more likely to receive non-curative treatment, while there was no impact of SPN on treatment strategies for patients treated with curative intent. There were no overall survival differences between patients with or without SPN.

Anaplastic Thyroid Carcinoma: An Update

Arnaud Jannin, Alexandre Escande, Abir Al Ghuzlan, Pierre Blanchard, Dana Hartl, Benjamin Chevalier, Frederic Deschamps, Livia Lamartina, Ludovic Lacroix, Corinne Dupuy, Eric Baudin, Christine Do Cao, Julien Hadoux

Summary: Anaplastic thyroid carcinoma (ATC) is a rare and aggressive form of thyroid cancer with a poor prognosis. Improving the management of this deadly disease involves the use of molecular tests for targeted therapies, fast-track dedicated care pathways, and prompt initiation of treatment. Adjuvant chemo-radiotherapy is recommended for localized disease after surgery, while chemotherapy or targeted therapy combined with decompressive cervical radiotherapy is recommended for locally advanced or metastatic disease.

Summary: In order to address the evaluation issue of radiotherapy innovations, the HERO project of ESTRO has initiated the development of a radiotherapy-specific value-based framework. A systematic literature search revealed no widely accepted definition or classification system for radiotherapy innovations. However, the unique properties of radiotherapy interventions can be used to categorize innovations in radiation oncology, but clear terminology denoting radiotherapy-specific characteristics is still needed.

Comprehensive clinical, genome and transcriptomic analysis of primary ghost cell odontogenic carcinoma

Lirui Zhang, Qiaoshi Xu, Zhien Feng

Summary: This study presents a comprehensive clinical, genomic, and transcriptomic analysis of primary ghost cell odontogenic carcinoma (GCOC). The results provide the first comprehensive molecular atlas for primary GCOC, including previously identified CTNNB1 mutation and novel alterations in MAP3K, EP300, and the 22q11.21 region. The transcriptome analysis reveals significant involvement of cytokine-cytokine receptor interaction and the PI3K-Akt signaling pathway. More GCOC cases should be compared to validate these findings for accurate clinical guidance.

Summary: This study compares the outcomes of transoral resections using the da Vinci SP and da Vinci Xi systems for oropharyngeal squamous cell carcinoma (OPSCC). The results indicate that the da Vinci SP system has advantages in terms of reduced docking and console times, as well as improved rates of wide negative resection margins. However, there were no significant differences in complication rates or postoperative functional outcomes between the two systems.
