Summary: This study compared the extent of weight recurrence between patients who received Sleeve Gastrectomy (SG) and Roux-en-Y gastric bypass (RYGB) after adequate weight loss at 1-year follow-up. The results showed that patients undergoing SG had a higher likelihood of weight recurrence up to 5 years and were less likely to achieve remission of comorbidities such as type 2 diabetes, hypertension, dyslipidemia, gastroesophageal reflux, and obstructive sleep apnea syndrome.

Summary: Morbid obesity is a chronic disease with a rising incidence, and surgical treatment has shown superior outcomes compared to conventional weight loss measures. This study evaluated the long-term weight loss outcomes, comorbidity reduction, and adverse effects of Roux-en-Y Gastric Bypass (RYGB), Biliopancreatic Diversion (BPD), and Sleeve Gastrectomy (SG) at 10 years. RYGB and BPD resulted in significant weight loss, with no observed differences in diabetes resolution and adverse outcomes. Longer follow-up reports are important for comparing the outcomes of different bariatric operations.

Summary: Background: The rates of secondary bariatric surgery have increased, with sleeve gastrectomy to gastric bypass conversion being the most common. This study evaluates the outcomes of this conversion compared to primary Roux-en-Y gastric bypass (RYGB) surgery using MBSAQIP data. Methods: The study analyzed the data of patients who underwent sleeve gastrectomy to RYGB conversion and primary laparoscopic RYGB, matching them based on preoperative characteristics. Results: The analysis showed that the conversion from sleeve gastrectomy to RYGB was associated with higher readmissions, interventions, conversion to open surgery, longer hospital stay, and longer operative time compared to primary RYGB. However, there were no significant differences in mortality or rates of bariatric complications. Conclusion: The study concludes that conversion from sleeve gastrectomy to RYGB is a safe and feasible procedure with reasonable outcomes compared to primary RYGB.

Summary: This study evaluated hospital outcomes of cirrhosis patients with nonvariceal upper gastrointestinal bleeding (NVUGIB) using the 2014 Nationwide Readmission Database. The results showed that patients with decompensated cirrhosis had higher readmission rates, mortality rates, and healthcare utilization compared to those with compensated cirrhosis and no cirrhosis. Early esophagogastroduodenoscopy was a modifiable factor associated with reduced readmission rates. Early identification of high-risk patients and adherence to guidelines may improve clinical outcomes.

Summary: Although bariatric surgery is the most effective treatment for severe obesity, a proportion of patients experience weight regain after surgery. The causes of weight regain are not well-defined, but dysregulated/maladaptive eating behaviors, lifestyle factors, and physiological compensatory mechanisms are believed to contribute. Current interventions, including dietary, supportive, behavioral, and exercise approaches, have not shown significant impact on weight regain. Future studies should aim to better understand the etiologies of weight regain and develop non-surgical interventions with demonstrated efficacy in rigorous trials.

Summary: The sensitivity of ultrasound-based surveillance for early detection of hepatocellular carcinoma (HCC) in patients with cirrhosis is suboptimal. This study compared the performance of a novel multitarget HCC blood test (Mt-HBT) with ultrasound with or without alpha-fetoprotein (AFP) for early HCC detection. The results showed that Mt-HBT had higher sensitivity than ultrasound alone and similar sensitivity to ultrasound with AFP, suggesting that Mt-HBT could be a comparable alternative for HCC surveillance in at-risk patients.
