De novo acute heart failure: Clinical features and one-year mortality in the Spanish nationwide Registry of Acute Heart Failure

Introduction and objectives: Acute heart failure (AHF), can occur as decompensated chronic heart failure (HF) or as a first episode, new onset. The aim of this study was to analyse the clinical characteristics and prognosis at one-year in a cohort of patients with new onset AHF. Methods: Prospective observational study of 3,550 patients with AHF. We compared patients with new onset HF with the others. Restricting the analysis to new onset AHF patients, we analysed the clinical characteristics, readmissions, mortality and impact of left ventricular ejection fraction on the prognosis. Results: A total of 1,105 (31%) patients fulfil the criteria for new onset AHF. These patients versus the rest, were younger, had a higher aetiology of hypertension and preserved left ventricular ejection fraction, less global comorbidity and better baseline overall functional status. After one year, mortality in new onset HF was less than chronic decompensated HF (15 vs. 27%; p<.001; respectively). Multivariate analysis showed a correlation between mortality and higher global comorbidity (hazard ratio. -HR- 1.11), renal failure (HR 1.73), higher prescription of antialdosteronics and antiaggregant (HR 2.13; 1.8; respectively). Left ventricular ejection fraction was unrelated to mortality. Conclusions: New onset AHF shows a clinical profile and prognosis different to that of chronic decompensated HF. Higher comorbidity, renal function and treatment post-discharge predict a higher risk of mortality. (C) 2018 Elsevier Espafia, S.L.U. All rights reserved.

Summary: This study compared the effectiveness of methylprednisolone 250 mg/d for 3 days with dexamethasone 6 mg/d for 10 days in severe COVID-19 patients. The results showed that there was no significant difference in mortality or intubation between methylprednisolone and dexamethasone, and hyperglycemia was more common in the methylprednisolone group.

Summary: This study aims to evaluate whether adding hydrochlorothiazide (HCTZ) to intravenous furosemide is a safe and effective strategy for improving diuretic response in acute heart failure (AHF). The study found that adding HCTZ to loop diuretic therapy can improve diuretic response in patients with AHF, but may also lead to renal function impairment.

Summary: This study aimed to validate the RIM Score-COVID in the SEMI-COVID-19 Registry and found that it showed good predictive ability for in-hospital COVID-19 mortality. The RIM Score-COVID is a simple and easy-to-use nomogram that uses clinical and analytical parameters to predict the risk of severe disease and death. The validation was done using data from consecutive patients hospitalized with confirmed COVID-19 in Spain.

Summary: This study analyzed the persistence, efficacy, and impact on quality of life of PCSK9 inhibitors (PCSK9is) in familial hypercholesterolaemia (FH) patients in clinical practice setting. The results showed that long-term persistence with PCSK9i in FH patients was very high, and it significantly reduced LDL-C levels and achieved treatment goals.

Usefulness of urinary potassium to creatinine ratio to predict diuretic response in patients with acute heart failure and preserved ejection fraction

Summary: This study aimed to evaluate the role of urinary potassium to creatinine ratio (K/Cr) in predicting diuretic and natriuretic response in patients with acute heart failure and preserved ejection fraction (AHF-pEF). The results showed that chlorthalidone had a better diuretic and natriuretic effect compared to spironolactone in patients with a high K/Cr ratio. Therefore, the K/Cr ratio does not have a guiding significance in the choice of diuretics for AHF-pEF patients.

Peritoneal ultrafiltration in older adult patients with advanced heart failure

Summary: Advanced heart failure patients with refractory congestive symptoms may benefit from peritoneal ultrafiltration (PUF) treatment, as it reduces hospital admissions and the use of intravenous diuretic rescue treatment without major complications. In a study of 18 older adult patients with comorbidity and advanced HF, PUF resulted in a significant decrease in hospital admissions (4 vs 20, p = 0.01) and intravenous diuretic use (4 vs 118, p < 0.001). The survival rate at one year was 72%.

Atrial fibrillation as a new prognosis factor in chronic patients after hospitalization: the CHRONIBERIA index

Summary: This collaborative project aimed to identify variables that affect mortality after discharge and develop a prognostic model suitable for chronic patients in an internal medicine ward. Logistic regression and Cox proportional hazard models were used to analyze the influence of these variables on one-year mortality. A model named CHRONIBERIA index was created, incorporating age, gender, physical dependence, neoplasia, and atrial fibrillation. The external validation of the index showed a successful and reliable prediction of mortality risk.

Quality of Life and Kidney Function in Older Adults: Prospective Data of the SCOPE Study

Summary: This study investigated the longitudinal alteration in health-related quality of life (HRQoL) over a two-year period among older adults with early-stage chronic kidney disease (CKD) and its association with CKD progression. The results showed that 41% of the participants experienced a decline in HRQoL, and 16.3% showed kidney function decline. Participants with HRQoL decline had higher levels of depression and greater decline in physical performance. The decline in kidney function did not contribute to the decline in HRQoL, but higher levels of depression and better physical performance were associated with less HRQoL decline.

Current heart failure disease management and treatment in accredited units from cardiology and internal medicine in Spain

Summary: This study investigated the organizational model and adherence to scientific recommendations in HF units in Spain. The results showed that HF units are mainly composed of specialists from cardiology, internal medicine, and specialized nurse practitioners. Most units use a hybrid approach for patient follow-up and natriuretic peptides are the most commonly used biomarkers. However, there is room for improvement in coordinating with primary care.
