Summary: In this study, patients with heart failure with preserved ejection fraction (HFpEF) were stratified based on left ventricular ejection fraction (LVEF), resulting in distinct morphologic and pathophysiologic subphenotypes. Patients with LVEF ranging from 50% to 60% demonstrated reduced contractility, impaired ventriculo-arterial coupling, and higher extracellular volume fraction, while patients with LVEF >60% exhibited a hypercontractile state with excessive left ventricular afterload and diminished preload reserve.

Summary: This study aimed to investigate the predictive roles of pre-operative left ventricular size and ejection fraction in ejection fraction improvement and outcome following revascularization in patients with coronary artery disease and left ventricular dysfunction. The results showed that pre-operative left ventricular end-systolic diameter and ejection fraction were predictive of ejection fraction improvement after revascularization, and smaller left ventricular end-systolic diameter and lower ejection fraction were associated with better outcomes.

Summary: This study investigated the frequency and prognostic implications of changes in left ventricular ejection fraction (LVEF) in patients with heart failure (HF). The results showed that changes in LVEF are strongly correlated with patient outcomes, especially in those with significant changes. Therefore, the trajectories of LVEF and types of HF changes are recommended as useful tools for prognostication.

Summary: Recent data suggest that guideline-directed medical therapy may improve clinical outcomes in heart failure patients with reduced ejection fraction (HFrEF) up to a certain range, while patients with higher ejection fraction do not seem to benefit. Patients with supranormal ejection fraction may be a distinctive group of patients, characterized by higher risk of non-cardiovascular death and lower levels of natriuretic peptides.

Summary: The measurement of left ventricular ejection fraction (LVEF) has been a key component in imaging studies for cardiac patients, but its utility as a prognostic indicator has diminished in certain populations such as heart failure patients with preserved ejection fraction (HF-PEF) and those with valvular heart disease. Assessing myocardial strain, particularly global longitudinal strain (GLS), holds promise in improving risk stratification and therapy selection for these patients. Additional studies are needed to fully integrate GLS into routine clinical practice and improve patient outcomes.

Summary: By analyzing LVEF data in HF patients, we found that there is a threshold between 40% and 50% LVEF, below which the pattern of patient characteristics changes, and event rates begin to increase. This provides support for the current prognosis-based definition of HF with mildly reduced ejection fraction based on upper LVEF thresholds.

Summary: This study demonstrates that a strategy of rapid uptitration of oral treatment for heart failure and close follow-up, compared with usual care, can significantly improve the prognosis of patients with acute heart failure, regardless of left ventricular ejection fraction. This approach reduces the risk of HF rehospitalization or all-cause death within 180 days after AHF hospitalization, independently of LVEF.

Summary: Vitamin D plays an important role as a prognostic biomarker in heart failure, although its therapeutic implications are still debated. This study found a close correlation between 25(OH)D and LVEF, as well as a significant positive correlation between 25(OH)D, serum uric acid, and albumin levels in HF patients. Furthermore, 25(OH)D concentration independently affected LVEF, supporting the need for establishing proper vitamin D supplementation schemes and dietary interventions in HF.

Summary: This study aimed to validate the RIM Score-COVID in the SEMI-COVID-19 Registry and found that it showed good predictive ability for in-hospital COVID-19 mortality. The RIM Score-COVID is a simple and easy-to-use nomogram that uses clinical and analytical parameters to predict the risk of severe disease and death. The validation was done using data from consecutive patients hospitalized with confirmed COVID-19 in Spain.

Summary: This study analyzed the persistence, efficacy, and impact on quality of life of PCSK9 inhibitors (PCSK9is) in familial hypercholesterolaemia (FH) patients in clinical practice setting. The results showed that long-term persistence with PCSK9i in FH patients was very high, and it significantly reduced LDL-C levels and achieved treatment goals.

Summary: This study aimed to evaluate the role of urinary potassium to creatinine ratio (K/Cr) in predicting diuretic and natriuretic response in patients with acute heart failure and preserved ejection fraction (AHF-pEF). The results showed that chlorthalidone had a better diuretic and natriuretic effect compared to spironolactone in patients with a high K/Cr ratio. Therefore, the K/Cr ratio does not have a guiding significance in the choice of diuretics for AHF-pEF patients.

Summary: Advanced heart failure patients with refractory congestive symptoms may benefit from peritoneal ultrafiltration (PUF) treatment, as it reduces hospital admissions and the use of intravenous diuretic rescue treatment without major complications. In a study of 18 older adult patients with comorbidity and advanced HF, PUF resulted in a significant decrease in hospital admissions (4 vs 20, p = 0.01) and intravenous diuretic use (4 vs 118, p < 0.001). The survival rate at one year was 72%.

Summary: This collaborative project aimed to identify variables that affect mortality after discharge and develop a prognostic model suitable for chronic patients in an internal medicine ward. Logistic regression and Cox proportional hazard models were used to analyze the influence of these variables on one-year mortality. A model named CHRONIBERIA index was created, incorporating age, gender, physical dependence, neoplasia, and atrial fibrillation. The external validation of the index showed a successful and reliable prediction of mortality risk.

Summary: This study investigated the longitudinal alteration in health-related quality of life (HRQoL) over a two-year period among older adults with early-stage chronic kidney disease (CKD) and its association with CKD progression. The results showed that 41% of the participants experienced a decline in HRQoL, and 16.3% showed kidney function decline. Participants with HRQoL decline had higher levels of depression and greater decline in physical performance. The decline in kidney function did not contribute to the decline in HRQoL, but higher levels of depression and better physical performance were associated with less HRQoL decline.

Summary: This study investigated the organizational model and adherence to scientific recommendations in HF units in Spain. The results showed that HF units are mainly composed of specialists from cardiology, internal medicine, and specialized nurse practitioners. Most units use a hybrid approach for patient follow-up and natriuretic peptides are the most commonly used biomarkers. However, there is room for improvement in coordinating with primary care.
