The International Society for the Study of Women's Sexual Health process of care (POC) for management of hypoactive sexual desire disorder (HSDD) algorithm was developed to provide evidence-based guidelines for diagnosis and treatment of HSDD in women by health care professionals. Affecting 10% of adult females, HSDD is associated with negative emotional and psychological states and medical conditions including depression. The algorithm was developed using a modified Delphi method to reach consensus among the 17 international panelists representing multiple disciplines. The POC starts with the health care professional asking about sexual concerns, focusing on issues related to low sexual desire/interest. Diagnosis includes distinguishing between generalized acquired HSDD and other forms of low sexual interest. Biopsychosocial assessment of potentially modifiable factors facilitates initiation of treatment with education, modification of potentially modifiable factors, and, if needed, additional therapeutic intervention: sex therapy, central nervous system agents, and hormonal therapy, guided in part by menopausal status. Sex therapy includes behavior therapy, cognitive behavior therapy, and mindfulness. The only central nervous system agent currently approved by the US Food and Drug Administration (FDA) for HSDD is flibanserin in premenopausal women; use of flibanserin in postmenopausal women with HSDD is supported by data but is not FDA approved. Hormonal therapy includes off-label use of testosterone in postmenopausal women with HSDD, which is supported by data but not FDA approved. The POC incorporates monitoring the progress of therapy. In conclusion, the International Society for the Study of Women's Sexual Health POC for the management of women with HSDD provides a rational, evidence-based guideline for health care professionals to manage patients with appropriate assessments and individualized treatments. The association between sex hormones and the change in brain-predicted age difference in older women

Jo Wrigglesworth, Ian H. Harding, Rakibul M. Islam, Phillip G. D. Ward, Robyn L. Woods, Robin J. Bell, John J. McNeil, Elsdon Storey, Gary Egan, Anne M. Murray, Ruth E. Trevaks, Stephanie A. Ward, Susan R. Davis, Joanne Ryan

Summary: This study investigated the relationship between circulating sex hormones and structural brain ageing in older women. The findings showed that women with higher levels of DHEA had higher baseline brain ageing, but this association was not significant after controlling for age and other potential confounding factors. Estrone, testosterone, and sex-hormone binding globulin were not associated with brain ageing. Further studies on circulating sex hormones and brain health in postmenopausal women are needed.

Hyperprolactinemia and male sexual function: focus on erectile dysfunction and sexual desire

G. Corona, G. Rastrelli, N. Bianchi, C. Sparano, A. Sforza, L. Vignozzi, M. Maggi

Summary: The present paper aims to analyze and discuss the relationship between male sexual function and elevated prolactin (PRL) levels (HPRL) using clinical data and meta-analytic approach. 4.2% of patients seeking medical care for sexual dysfunction showed PRL levels above the normal range. HPRL was found to have a negative influence on male sexual desire. Treating HPRL can improve libido, but its effect on erection is limited. The role of HPRL in erectile dysfunction remains inconclusive. Overall, the paper provides important evidence on the relationship between PRL levels and male sexual function.

Cryopreservation of Human Spermatozoa: Functional, Molecular and Clinical Aspects

Lara Tamburrino, Giulia Traini, Arianna Marcellini, Linda Vignozzi, Elisabetta Baldi, Sara Marchiani

Summary: Cryopreservation is an expanding strategy for fertility preservation, animal breeding, and endangered species preservation. Despite improvements in semen cryopreservation techniques, damage to spermatozoa and its impairment of function remain unsolved problems. Active research is needed to optimize the cryopreservation process and prevent sperm damage. This review discusses the evidence of structural, molecular, and functional damage in cryopreserved human spermatozoa and strategies to prevent it, as well as the outcomes of assisted reproduction techniques using cryopreserved spermatozoa.

Testicular and Haematological Cancer Induce Very High Levels of Sperm Oxidative Stress

Costanza Calamai, Oumaima Ammar, Viktoria Rosta, Ginevra Farnetani, Salvatore Zimmitti, Lisa Giovannelli, Linda Vignozzi, Csilla Krausz, Monica Muratori

Summary: Cancer significantly increases sperm DNA fragmentation (sDF) and oxidative stress levels. The induction of viable oxidative stress is even higher in cancer patients compared to normozoospermic subfertile patients and healthy donors. There is a correlation between sDF and viable oxidative stress in all subjects, but not in cancer patients alone. Detecting oxidative stress could help manage the reproductive issues of cancer patients.

Rodgers, Annie Aime, Jacinthe Dion, Giada Pietrabissa, Gianluca Lo Coco, Marie Caltabiano, Esben Strodl, Catherine Begin, Marie-Eve Blackburn, Gianluca Castelnuovo, Antonio Granero-Gallegos, Salvatore Gullo, Naomi Hayami-Chisuwa, Qiqiang He, Christoph Maiano, Gian Mauro Manzoni, David Mellor, Michel Probst, Matthew Fuller-Tyszkiewicz

Summary: This study aimed to examine the usefulness of a biopsychosocial model in explaining higher body mass index (BMI) and disordered eating. The study analyzed predictors of BMI and various measures of disordered eating in young adults from seven countries, grouped into four regions. Results showed unique patterns of association between predictors and BMI as well as different measures of disordered eating in the four geographical regions. The findings highlight the specific biopsychosocial factors that predict higher BMI and different aspects of disordered eating, emphasizing the need for caution in generalizing findings across countries.

Testosterone and the heart: friend or foe?

S. R. Davis

Summary: Cardiovascular disease (CVD) is the leading cause of death in older women, and sex hormones, especially estrogens in women, play a crucial role in its development. Low testosterone levels in women may also have detrimental effects on cardiovascular function. Testosterone has been shown to have favorable effects on blood vessel function and blood pressure in postmenopausal women. Animal studies have demonstrated advantageous effects of testosterone on myocardial function and cardiac electrical signaling. However, there is limited human data, mainly consisting of observational and mechanistic studies, on the cardiovascular effects of testosterone in women.

Summary: Sperm cryopreservation is recommended for male fertility preservation in cancer patients or other medical conditions. The recovery rates of motility and viability vary depending on the medical condition. Our study showed that lower sperm quality led to lower recovery rates, and donor samples had the highest recovery rates. Cut-off values for motility recovery were established based on initial semen quality. Overall, our results emphasize the importance of considering intrinsic sperm characteristics in cryopreservation.
