Neutrophil to lymphocyte ratio and early clinical outcomes in patients with acute ischemic stroke

Background: The neutrophil to lymphocyte ratio (NLR) is closely linked to mortality in patients with cardiovascular disease. We investigated whether NLR is associated with early clinical outcomes in patients with acute ischemic stroke. Methods: We collated data from a tertiary hospital's stroke registry including admitted patients with a first-ever acute ischemic stroke within 72 h of onset. White blood cell counts and peripheral differential counts were measured on admission. Early clinical outcomes were in-hospital mortality and disability at discharge assessed by the modified Rankin scale (mRS). Results: Among 1131 stroke patients, 454 patients were included and classified into tertile groups based on NLR on admission. Patients in higher tertiles of NLR were likely to have severe neurologic deficit at discharge. Higher NLR tertiles were associated with an unfavourable shift of mRS score (p <.0001). This association remained significant after adjustment for clinical and laboratory variables including age, sex, hypertension, hypercholesterolemia, atrial fibrillation, stroke severity, and glucose level (p =.032 for trend). However, risk of death or major disability (score of 3-6 on mRS) and in-hospital mortality were not significantly different across NLR tertile groups. Conclusions: In patients with acute ischemic stroke, NLR was predictive of short-term functional outcome.

Summary: This study aimed to investigate the effect of diabetes status at admission on long-term survival in patients with first-ever stroke. The findings suggest that stroke patients with diabetes exhibited a higher risk of all-cause mortality than those without diabetes, particularly in the overall stroke and ischemic stroke populations. Additionally, in stroke populations after 28 days of onset, high risk of long-term mortality was stated in stroke patients with poor HbA1c control.

Summary: This study investigated the impact of TTF-1 immunostaining on the therapeutic effect of chemotherapy in Japanese patients with nonsq NSCLC. The results showed that patients who were TTF-1-positive had significantly longer overall survival under the pemetrexed regimen compared to regimens without pemetrexed. Therefore, TTF-1 immunostaining plays an important role in predicting the prognosis of nonsq NSCLC patients.

A Randomized Placebo-Controlled Study of a Transcranial Photobiomodulation Helmet in Parkinson's Disease: Post-Hoc Analysis of Motor Outcomes

Summary: Emerging evidence supports the use of transcranial photobiomodulation (tPBM) for improving symptoms of neurodegenerative diseases, particularly Parkinson's disease (PD). A triple blind, randomized placebo-controlled trial was conducted to analyze the safety and efficacy of tPBM for PD motor symptoms. The results showed that tPBM is a safe treatment and improved several PD motor symptoms in patients that responded to treatment, making it an attractive non-pharmaceutical adjunct therapy.

Summary: In this study, we characterized different populations of extracellular vesicles (EVs) derived from pleural mesothelioma cell lines and identified unique cancer-specific proteomes and oncogenic cargo in each subtype. We also established a comprehensive proteomic database of cancer-associated proteins in EVs, which could serve as new targets for future biomarker studies in pleural mesothelioma.

Anti-PD-1 Monoclonal Antibodies (mAbs) Are Superior to Anti-PD-L1 mAbs When Combined with Chemotherapy in First-Line Treatment for Metastatic Non-Small Cell Lung Cancer (mNSCLC): A Network Meta-Analysis

Summary: Platinum-based chemotherapy combined with anti-PD-1 or PD-L1 monoclonal antibodies is the standard first-line therapy for mNSCLC patients without sensitizing driver mutations. Using network meta-analysis, we compared the efficacy of these two antibodies indirectly due to the absence of head-to-head randomized control trials. The results showed that anti-PD-1 mAb + chemotherapy had superior overall survival, progression-free survival, and objective response rate compared to anti-PD-L1 mAb + chemotherapy, with similar rates of grade 3 and higher toxicities. However, head-to-head RCTs are still needed in the future.

The results showed a significant association between serum triglyceride concentrations and incident CKD in Japanese men, but not in women. This study provides important insights for predicting and preventing CKD.

Through a Delphi consensus study, 23 specific indicators were identified, covering aspects such as diagnosis, staging, and treatment of HCC. Adherence to these indicators can improve the quality of care and patient outcomes, including survival.
