Summary: This retrospective cohort study at an academic medical center from 2014 to 2018 examined consecutive patients undergoing colectomy or proctectomy, revealing that one third of emergency department visits after colorectal surgery are potentially preventable, with a particular focus needed on patients who do not speak English as their primary language.

Summary: The study examined the risk factors for 30-day readmission and emergency department visits after minimally invasive pelvic organ prolapse surgery. It found that patients undergoing concomitant hysterectomy had a slightly higher risk of 30-day readmission compared to those who did not have a hysterectomy.

Summary: This study aims to evaluate the impact of mental health disorders on emergency department utilization following bariatric surgery. The study found that bariatric patients with schizophrenia or bipolar disorder have higher baseline emergency department usage and experience a greater increase in usage after surgery. These patients would benefit from intensive outpatient follow-up to limit emergency department visits.

Summary: Although post-esophagectomy emergency department utilization is high, the majority of visits do not convert to inpatient admission. Anastomotic leak is independently associated with higher odds of conversion to admission. Patients living in regions further away tend to utilize closer local hospitals, resulting in lower conversion rates to admission at the index hospital.

Summary: Discharge against medical advice (DAMA) and readmissions are important issues worldwide, with key factors including medical aid, foreign worker insurance, and readmission by car. Healthcare workers need to treat these patients more carefully and preventively to mitigate potential clinical, legal, and economic impacts of DAMA.

Summary: As physicians aim to reduce ionizing radiation by utilizing Magnetic Resonance Imaging (MRI), it is important to be aware of the specific risks associated with MRI, such as thermal burns. Obese patients are particularly susceptible to MRI-related thermal burns, as demonstrated by a case of a 45-year-old female who experienced a delayed, occult full-thickness burn after a diagnostic MRI. Emergency physicians should be knowledgeable about the risks of MRI and able to properly counsel patients before and after the procedure.

Summary: This study aims to determine the incidence, reasons, and predictors of emergency department (ED) treat-and-release encounters after gastrointestinal cancer operations. The study found that 7.9% of patients had an ED treat-and-release encounter, while 10.8% had an ED encounter with readmission. The most common reasons for ED treat-and-release encounters were pain, device/ostomy complaints, and wound complaints. Factors predicting ED treat-and-release encounters included non-Hispanic Black race/ethnicity and Medicare or Medicaid coverage.

Summary: This study aimed to examine patient comprehension of diabetes self-management instructions provided at hospital discharge to assess the risk of readmission. Results showed similar patient comprehension scores between those with and without readmission, despite the need for clarification of discharge information in many patients. Glycemic variability was associated with comprehension scores, but not with readmission risk. The Diabetes Early Readmission Risk Indicator (DERRI) was validated as a predictor for readmission at 90 days.

The results showed that postoperative pain was the most common reason for ED visits within the first 7 days, while hypertension, diabetes, insurance type, and Medicaid were independent predictors for ED visits within 30 days postoperatively.

Summary: This study analyzed a national US database and found that treatment at ECMO centers did not offer a survival advantage in ARDS patients receiving mechanical ventilation. It suggests that early transfer of all ARDS patients to ECMO centers may not be necessary. Further evaluation of ECMO center volume and illness severity is needed.

Summary: This study investigates gender disparities in surgical care in Malawi, specifically focusing on health-seeking behavior and attitudes of community members. The results show that women are more likely to be fearful of surgery and report barriers to seeking surgical care. Men, on the other hand, express preferences for male medical treatment and greater decision-making power in household matters. Few participants believe that gender equality has been achieved.

Summary: The study found that about half of opioid naive patients undergoing minimally invasive, benign foregut operations filled an opioid prescription postoperatively, and the risk of progressing to persistent postoperative opioid use was less than 1%, with low clinical significance. The risk of persistent postoperative opioid use seemed to be influenced by Charlson comorbidity index scores, while patients with a history of mental illness or substance use disorder had a relatively lower risk.
