Summary: This study aimed to identify barriers to, and facilitators of, the implementation of the REACH-HF programme and develop and refine the REACH-HF Service Delivery Guide. The results showed that factors such as lack of resources, interest in heart failure, and close collaboration with the clinical heart failure team can impact the implementation process. The main output of the study, a refined version of the REACH-HF Service Delivery Guide, can guide the implementation process and provide practical solutions to common implementation barriers.

Summary: Heart transplantation is the preferred treatment for end-stage chronic heart failure. Although many patients experience improved functional ability after transplantation, they still face the challenges of acute rejection and other comorbidities. Exercise training, recommended before and after transplantation, can improve exercise capacity and skeletal muscle strength without altering immunosuppression.

Summary: This study aimed to determine the association of sex and race/ethnicity with acute hospital readmissions in traumatic brain injury patients during inpatient rehabilitation. The study found that the readmission rates were similar between female and male patients, but certain factors differed. The study also revealed significant differences in readmission rates among different race/ethnic groups, as well as the impact of factors such as length of stay and cognitive/motor function on readmissions. Therefore, this study is important for improving care planning and quality improvement efforts for traumatic brain injury patients.

Important research priorities include including diverse populations, enhancing equitable delivery, identifying populations that benefit most from virtual and remote CR, and comparing outcomes between traditional and virtual CR.

Summary: The purpose of this research was to determine if early participation in cardiac rehabilitation could reduce readmission rates for heart failure patients. The study found that patients who attended cardiac rehabilitation within 6 weeks of discharge had lower readmission rates compared to those who did not attend. Specifically, early participation was associated with reduced all-cause readmissions at 30 days and 6 weeks.

Summary: This study aims to identify causes and risk factors for potentially preventable readmissions after discharge from an inpatient rehabilitation facility. The study found that patients with more comorbidities, initial admission with a spinal cord injury, or lower Functional Independence Measure motor scores upon admission or discharge from the facility were more likely to experience a potentially preventable readmission. The most common diagnoses for potentially preventable readmissions were sepsis, renal failure, respiratory problems, and urinary tract infection.

Summary: The study aimed to assess the provision of cardiac rehabilitation to Danish survivors of cardiac arrest and found that, compared with patients after acute myocardial infarction, survivors of cardiac arrest received lower levels of certain core components in hospitals. Municipalities showed no difference in provision of core components. Overall, organizational factors did not appear to influence the provision of cardiac rehabilitation to survivors of cardiac arrest in Denmark, suggesting the need for further research to ensure equal access to all components of cardiac rehabilitation.
