Summary: This study provides new insights into the abundance, expression patterns, and interactomes of endothelial cell (EC) subtypes in COVID-19 and idiopathic pulmonary fibrosis (IPF). The findings suggest the importance of ECs in the pathophysiology of both conditions and may contribute to future investigations into the progression and treatment of COVID-19 and IPF.

Biomarkers for Chronic Lung Allograft Dysfunction: Ready for Prime Time?

Summary: Chronic lung allograft dysfunction (CLAD) is a major obstacle in lung transplant outcomes. The search for biomarkers to assist in earlier detection, differential diagnosis, and prognosis of CLAD phenotypes is increasing. This article discusses the current status and state-of-the-art of biomarker research in CLAD, with a focus on radiological biomarkers and those found in peripheral tissue, bronchoalveolar lavage, and circulating blood. The future of biomarker research lies in combining clinical information with findings in tissue, bronchoalveolar lavage, or blood to achieve the ultimate goal of earlier CLAD identification.

Disruption of GCN2 Pathway Aggravates Vascular and Parenchymal Remodeling during Pulmonary Fibrosis

Summary: This study examined the involvement of the GCN2/eIF2 alpha pathway in the development of both pulmonary fibrosis (PF) and pulmonary hypertension (PH). The researchers found that GCN2 expression was reduced in lung tissue from PF and PH patients, as well as in endothelial cells. Animal experiments showed that GCN2 mutations led to increased fibrosis and vascular remodeling, as well as elevated right ventricular systolic pressure. These findings suggest a potential role for GCN2 dysregulation in the development of PF and PH.

Summary: In this multicentre study, researchers performed a comprehensive analysis of heart samples from autopsies of COVID-19 patients and discovered that cardiac involvement in COVID-19 is a macrophage-driven inflammatory process that is distinct from the typical anti-viral inflammatory responses. The study also found the presence of intussusceptive angiogenesis in the affected hearts, which is a key characteristic of vascular remodeling in COVID-19 pneumonia.

Summary: This study systematically analyzed the distribution and metabolism of histidine-containing dipeptides (HCDs) in mouse, rat, and human tissues using a sensitive UHPLC-MS/MS approach. The results show that tissue HCD levels are uniquely produced by carnosine synthase (CARNS1) in fast-twitch skeletal muscle fibers and brain oligodendrocytes. Additionally, N-acetylcarnosine is a stable plasma HCD and is enriched in human skeletal muscles.

The hemodynamic interplay between pulmonary ischemia-reperfusion injury and right ventricular function in lung transplantation: a translational porcine model

Summary: Lung transplantation is a challenging procedure and can result in severe damage to the transplanted pulmonary graft. The interaction between right ventricular function and lung injury during the transplantation process is poorly understood. Through a translational porcine model, it was found that right ventricular failure can occur as a result of forcing the entire cardiac output through a lung suffering from ischemia-reperfusion injury. The study also revealed a delicate balance between the development of pulmonary ischemia-reperfusion injury and right ventricular function, highlighting the potential benefits of extracorporeal life support technology in these cases.

Aspergillus-Specific IgG Antibodies are Associated With Fungal-Related Complications and Chronic Lung Allograft Dysfunction After Lung Transplantation

Summary: Fungal exposure and sensitization negatively impact respiratory diseases, including lung transplant recipients. This study examines the correlation between fungal-specific antibodies, fungal isolation, chronic lung allograft dysfunction (CLAD), and overall survival in 311 lung transplant patients. The results show that elevated levels of Aspergillus fumigatus or Aspergillus flavus IgG are associated with more fungal isolation and CLAD. However, IgE levels and fungal isolation are not correlated with CLAD or death. Mold and Aspergillus species isolation are associated with CLAD occurrence, while Aspergillus species isolation is also linked to impaired survival. Fungus-specific IgG could serve as a non-invasive marker for fungal exposure and a diagnostic tool for identifying high-risk patients in long-term follow-up post-lung transplant.
