Summary: Vital pulp therapy (VPT) has been proposed as an alternative approach for symptomatic mature permanent teeth with deep caries lesions, aiming to maintain pulp vitality and avoid non-surgical root canal therapy. Pain assessment plays a key role in the success of VPT, and proper assessment of pulp inflammation and choice of appropriate materials are important factors for enhancing VPT success.

Summary: This study aimed to investigate whether a full pulpotomy (FP) performed by a general practice dentist (GPD) in a public dental care clinic (PDCC) is suitable for patients who cannot afford root canal treatment (RCT) and to assess its success rate and patient satisfaction. The results of the study showed excellent success rate and patient satisfaction with FP performed by a GPD in a PDCC, making it a viable routine treatment option for symptomatic irreversible pulpitis.

Summary: The promotion of minimally invasive treatments focused on maintaining pulp vitality is a priority in endodontics. This review discusses the outcomes of partial and full pulpotomy procedures and analyzes various factors that influence the results. Current evidence suggests that age, gender, tooth type, and intraoperative pulpal hemorrhage do not significantly impact the outcome, while factors such as caries depth, pulp inflammation, capping materials, and restoration integrity may affect the success. Further experimental research is needed to definitively determine the success of pulpotomy procedures.

Summary: This study aimed to compare the outcomes of complete pulpotomy (CP) and partial pulpotomy (PP) techniques for the management of mature permanent teeth with carious pulpal exposure and symptomatic irreversible pulpitis (SIP). The results showed that both CP and PP resulted in favorable outcomes for treating this condition. CP may be a more conservative approach and can be considered as the first choice for treatment.

Summary: Teeth with symptomatic irreversible pulpitis showed severe acute inflammation, necrosis, and bacterial infection in the pulp chamber. Bacterial invasion and colonization were observed in all teeth with caries exposure, suggesting a significant role in bacterial spread through the pulp tissue.

Summary: The aim of this study was to compare and evaluate the clinical and radiographic performance, post-operative pain, and anti-inflammatory intake after partial pulpotomy with different pulp capping agents. The results showed that there was a significant difference in post-operative pain and anti-inflammatory medication intake after partial pulpotomy with Emdogain compared to other three capping agents. However, there was no difference in both clinical and radiographic performances among the four capping agents.

Summary: This study compared the coronal discoloration at 6 and 12 months after full pulpotomy in mature permanent molars using MTA, Biodentine, and TotalFill, and found that Biodentine and TotalFill resulted in less discoloration compared to MTA. Additionally, the remaining buccal wall thickness had a significant effect on the degree of discoloration.

Summary: This study compared the outcomes, quality of life, and patients' satisfaction after full pulpotomy and root canal therapy (RCT) in mature teeth with irreversible pulpitis. The results showed that full pulpotomy could be an alternative treatment to RCT with similar success rates, lower pain levels, and shorter pain relief time. Additionally, patients were more satisfied with pulpotomy in terms of time, intraoperative pain, pleasantness, and cost.

Summary: This study investigated the factors that affect clinicians' decisions in direct pulp capping (DPC) cases, the preferred method of caries removal, and the preferred capping material. The results showed that carious-exposed pulp was the most important factor in clinical decisions, while the number of exposures had the least impact. Overall, complete caries removal was preferred over selective caries removal, and calcium silicate-based materials were preferred over calcium hydroxide-based materials.
