Alzheimer's disease is the most common type of dementia and one of the leading sources of disability and dependency in the elderly. Given the limited treatment options, understanding the role of modifiable risk factors implied in the disease pathogenesis is a worthwhile endeavor to limit its global burden. Recently, the variability of blood pressure has been suggested to be a significant determinant of brain alterations and a potential therapeutic target. The aim of this article is to review the clinical evidence on the association between visit-to-visit blood pressure variability and Alzheimer's disease, highlight the underlying mechanisms, and suggest future implications.

Summary: Distinct clinical phenotypes have been identified within the ESUS population, which are associated with different risk factors and vascular involvement; this may provide clues to the underlying pathogenic mechanisms of ESUS.

Predictors of Pharmaco-Resistance in Patients with Post-Stroke Epilepsy

Simona Lattanzi, Claudia Rinaldi, Claudia Cagnetti, Nicoletta Foschi, Davide Norata, Serena Broggi, Chiara Rocchi, Mauro Silvestrini

Summary: The study found that approximately 20% of patients with post-stroke epilepsy develop pharmaco-resistance, which is associated with younger age at stroke onset, stroke type and severity, occurrence of status epilepticus, and seizure types.

Systemic Inflammatory Response Index and Futile Recanalization in Patients with Ischemic Stroke Undergoing Endovascular Treatment

Simona Lattanzi, Davide Norata, Afshin A. Divani, Mario Di Napoli, Serena Broggi, Chiara Rocchi, Santiago Ortega-Gutierrez, Gelsomina Mansueto, Mauro Silvestrini

Summary: This study aimed to evaluate the relationship between systemic inflammatory response index (SIRI) and futile recanalization in patients with AIS. The study found that older age, higher admission National Institutes of Health stroke scale score, and higher admission SIRI increased the risk of poor outcomes at 3 months despite complete or near-complete recanalization.

Summary: PFO is a common postnatal defect of cardiac atrial septation with potential familial aggregation. However, no genetic variants associated with PFO have been identified so far. Despite the need for further research, existing evidence from animal and other studies suggest a strong genetic background for PFO.

Neutrophil-to-Lymphocyte Ratio Predicts Early Neurological Deterioration after Endovascular Treatment in Patients with Ischemic Stroke

Summary: The worsening of neurological status shortly after acute ischemic stroke (AIS) is a serious problem, and inflammation plays a key role in stroke pathology. A study found that the neutrophil-to-lymphocyte ratio (NLR) at admission in AIS patients undergoing endovascular treatment (EVT) is associated with early neurological deterioration (END). Higher NLR values predicted a higher risk of END.

Summary: This retrospective study examined the 12-month effectiveness and tolerability of Cenobamate (CNB) in patients with Lennox-Gastaut syndrome (LGS) in real-world practice. The results showed a reduction in seizure frequency and minimal side effects after CNB treatment. These findings suggest that CNB may be a promising therapeutic option for drug-resistant epilepsy associated with LGS.

Summary: Factor VII activating protease (FSAP) was first isolated from human plasma less than 30 years ago, and its biological properties and role in hemostasis have been extensively studied. The structure of FSAP and its interactions with other proteins and compounds that modulate its activity have been elucidated. This review summarizes the current knowledge on FSAP, including its structure, the processes regulating its activity, and its role in hemostasis and cardiovascular diseases.

Summary: This study aimed to develop a model and nomogram to predict drug resistance in patients with post-stroke epilepsy (PSE). The study identified age, intracerebral hemorrhage, stroke severity, latency of PSE, and status epilepticus as independent predictors of drug resistance. The nomogram showed good predictive accuracy and may serve as a practical tool for personalized prediction of drug-resistant PSE.

Perampanel in Brain Tumor-Related Epilepsy: A Systematic Review

Summary: Brain tumor-related epilepsy (BTRE) is a common comorbidity in patients with brain neoplasms. Glutamate receptors antagonist Perampanel (PER) may have therapeutic potential in reducing tumor growth and controlling epileptiform activity. Both pre-clinical and clinical studies suggest that PER can be an effective and well-tolerated option for patients with BTRE, but more research is needed for further understanding of its effects on tumor progression.
