Frontotemporal dementia (FTD) is a heterogeneous clinical, genetic, and neuropathological disorder. Clinical diagnosis and prediction of neuropathological substrates are hampered by heterogeneous pictures. Diagnostic markers are key in clinical trials to differentiate FTD from other neurodegenerative dementias. In the same view, identifying the neuropathological hallmarks of the disease is key in light of future disease-modifying treatments. The aim of the present review is to unravel the progress in biomarker discovery, discussing the potential applications of available biological, imaging, and neurophysiological markers.

Summary: Pisa syndrome and camptocormia in patients with Parkinson's disease are associated with neuropsychological functioning. Pisa syndrome patients showed worse visuospatial performances and subjective visual vertical perception, while camptocormia patients did not exhibit significant differences. Furthermore, more than 60% of patients with these postural abnormalities had reduced awareness of their condition.

Summary: In this study, genetic and proteomic data were integrated to prioritize drug targets for neurodegenerative diseases. The analysis identified several proteins as potential targets for different diseases, such as BIN1, GRN, RET, CD38, DGKQ, GPNMB, FCRL3, LMAN2, MAPK3, DHRS11, FAM120B, SHMT1, PSMB3, SARM1, and SCFD1.

Brain-derived tau: a novel blood-based biomarker for Alzheimer's disease-type neurodegeneration

Summary: Blood-based biomarkers for amyloid beta and phosphorylated tau show good diagnostic accuracies and agreements with their corresponding CSF and neuroimaging biomarkers, while the blood-based neurodegeneration marker neurofilament light is not specific to Alzheimer's disease. A newly developed blood-based assay for brain-derived tau has shown equivalent diagnostic performance as CSF total-tau, accurately distinguishing Alzheimer's disease from other neurodegenerative diseases.

The researchers examined brain structure and function in young adult mutation carriers and found differences between preclinical mutation carriers and familial non-carriers at a mean age of 26 years. Unravelling neurotransmitters impairment in primary progressive aphasias

Summary: Primary progressive aphasias (PPAs) are a group of neurodegenerative diseases characterized by language impairment and possible presence of other symptoms. In this study, researchers used the JuSpace toolbox to analyze the correlation between brain changes in PPA patients and various neurotransmitter systems. The results showed significant associations between voxel-based brain changes and the distribution of serotonin, dopamine, and glutamatergic pathways. Additionally, disease severity and behavioral symptoms were negatively correlated with specific neurotransmitter systems. This pilot study highlights the potential of JuSpace in assessing neurotransmitter deficits in neurodegenerative dementias and gaining insight into disease mechanisms and associated clinical features.

The Early Diagnosis of Alzheimer's Disease: A Patient-Centred Conversation with the Care Team

Summary: Alzheimer's disease (AD), the leading cause of dementia, affects around 10 million people in Europe. Early diagnosis of AD can be achieved through neuroimaging techniques and biomarkers, but challenges such as low sensitivity, high cost, and limited availability need to be addressed. Plasma biomarkers offer a low-cost and non-invasive alternative for AD screening and diagnosis. In addition to technical challenges, organizational and ethical concerns must also be taken into account for effective diagnosis and patient care.

Summary: This study used a rigorous approach to estimate the Levodopa equivalent dose (LED) of safinamide 50 mg and 100 mg, which were found to be equivalent to 100 mg LED and rasagiline 1 mg. Large prospective pragmatic trials are needed to replicate these findings.

Summary: ALCAR is a donor of acetyl groups and can increase the levels of carnitine in cells, which helps transport fatty acids across mitochondrial membranes. In vivo studies have shown that ALCAR can reduce oxidative stress and inflammation. This observational study aimed to provide additional data on the effects of ALCAR in ALS patients in Italy.

Summary: Ataxia-Telangiectasia Mutated (ATM) is a protein kinase involved in DNA repair processes. Mutations in the Atm gene cause Ataxia-Telangiectasia (AT), a disorder characterized by ataxic movements, immune alterations, genomic instability, and cancer predisposition. Recent studies have shown unexpected roles of ATM in cognition and synaptic function, suggesting the importance of understanding ATM-dependent mechanisms in neurodevelopmental and degenerative disorders.
