Red cell distribution width in adults with congenital heart disease: A worldwide available and low-cost predictor of cardiovascular events

Background: Red cell distribution width (RDW) is a standard component of the automated blood count, and is of prognostic value in heart failure and coronary heart disease. We investigated the association between RDW and cardiovascular events in patients with adult congenital heart disease (ACHD). Methods and results: In this prospective cohort study, 602 consecutive patients with ACHD who routinely visited the outpatient clinic were enrolled between 2011 and 2013. RDW was measured in fresh venous blood samples at inclusion in 592 patients (median age 33 [IQR 25-41] years, 58% male, 90% NYHA I) and at four annual follow-up visits. During 4.3 [IQR 3.8-4.7] years of follow-up, the primary endpoint (death, heart failure, hospitalization, arrhythmia, thromboembolic events, cardiac intervention) occurred in 196 patients (33%). Median RDW was 13.4 (12.8-14.1)% versus 12.9 (12.5-13.4)% in patients with and without the primary endpoint (P < 0.001). RDW was significantly associated with the endpoint when adjusted for age, sex, clinical risk factors, CRP, and NT-proBNP (HR 1.20; 95% CI 1.06-1.35; P=0.003). The C-index of the model including RDW was slightly, but significantly (P=0.005) higher than the model without (0.74, 95% CI 0.70-0.78 versus 0.73, 95% CI 0.69-0.78). Analysis of repeated RDWmeasurements (n=2449) did not show an increase in RDW prior to the occurrence of the endpoint. Conclusions: RDW is associated with cardiovascular events in patients with ACHD, independently of age, sex, clinical risk factors, CRP, and NT-proBNP. This readily available biomarker could therefore be considered as an additive biomarker for risk stratification in these patients. (c) 2018 The Authors. Published by Elsevier B.V.

Daily life and psychosocial functioning of adults with congenital heart disease: a 40-53 years after surgery follow-up study

Summary: Nowadays, a large number of adults with congenital heart disease (CHD) have attained adulthood, but there is limited knowledge about their psychosocial functioning. This longitudinal cohort study followed 204 patients who underwent surgery for CHD during childhood and found that despite lower education and employment rates, CHD patients had better health-related quality of life and emotional functioning compared to the general population.

Usefulness of insertable cardiac monitors for risk stratification: current indications and clinical evidence

Summary: The 2018 ESC Syncope guidelines expanded the indications for an insertable cardiac monitor (ICM) to include patients with unexplained syncope and primary cardiomyopathy or inheritable arrhythmogenic disorders. This review article discusses the clinical evidence for using an ICM for risk stratification in different patient populations, including various cardiac conditions. The experts suggest that while there is limited data on the usefulness of ICMs in these populations, most studies demonstrate its early detection of clinically relevant arrhythmias. However, the impact of ICMs on prognosis is difficult to demonstrate in populations with rare diseases.

Stable and durable factor IX levels in patients with hemophilia B over 3 years after etranacogene dezaparvovec gene therapy

Summary: This study reports the 3-year outcomes of using Etranacogene dezaparvovec in the treatment of adults with hemophilia B. The results show that Etranacogene dezaparvovec significantly increases FIX activity, reduces bleeding frequency, improves joint health, and has no major adverse events over the 3-year period.

Prevalence and development of aortic dilation and dissection in women with Turner syndrome: a systematic review and meta-analysis

Summary: The association between clinical bleeding phenotype and thrombin generation and plasmin generation profiles in patients with hemophilia was investigated. The study found that thrombin generation and plasmin generation parameters differed between patients with hemophilia and healthy individuals, and a decreased thrombin generation profile was associated with a severe clinical bleeding phenotype.

Elective Ascending Aortic Aneurysm Surgery in the Elderly

Summary: This study aims to evaluate patient and procedural characteristics and compare the outcomes and mortality after surgery between elderly and non-elderly patients with ascending aortic aneurysm. The study found that elderly patients had larger aortic diameters and more cardiovascular risk factors. Short-term mortality was comparable between elderly and non-elderly patients, but the five-year survival rate was lower in elderly patients.

Reproductive health in adults with congenital heart disease: a review on fertility, sexual health, assisted reproductive technology and contraception

Summary: Due to improved survival rates in individuals with CHD, considering their reproductive health is important. This paper discusses fertility, sexuality, assisted reproductive technology (ART), and contraception in adults with CHD. The expert opinion suggests that timely counseling is necessary, and ART decisions are typically based on expert opinion with follow-up in an expert center recommended. Further research is needed to fill gaps in knowledge on the risks and frequency of complications of ART in adults with CHD, as well as differentiate relative risks in different types of CHD.

Summary: Cardiovascular disease is becoming more common in patients with hemophilia, and managing the delicate balance between thrombosis and hemostasis in these patients requires careful consideration. Specific recommendations are lacking for patients with very low clotting factor levels, and treatment strategies are mainly based on anecdotal evidence. For patients with higher clotting factor levels, antithrombotic therapy can be used with careful monitoring.

Pregnancy in women with congenital heart disease: a focus on management and preventing the risk of complications

Summary: This article provides an overview of common scenarios and management strategies for congenital heart disease (CHD) during pregnancy, including preconception counseling, antenatal management, and strategies to prevent maternal and fetal complications. Preconception counseling should start early and be offered to both men and women, addressing contraception choices, lifestyle modifications, cardiac optimization, chances of inherited cardiac lesions in the child, risks to the mother, and long-term prognosis. Pregnancy induces cardiovascular changes that may result in cardiac complications, and risk stratification is based on the underlying cardiac disease and data from studies. Women with lower-risk CHDs can be managed in secondary care, while complex CHD cases require expert counseling in tertiary centers. Those with high-risk conditions should avoid pregnancy, use effective contraception, and seek tertiary center management if pregnancy does occur.

Summary: The prevalence of anti-von Willebrand factor (VWF) antibodies was assessed in patients with type 3 von Willebrand disease (VWD). The study found that 8.4% of the subjects tested positive for anti-VWF antibodies, while 6% had neutralizing VWF inhibitors. Summary: This study aimed to evaluate the efficacy of FIX-FIAV in plasma from HA patients. The results showed that FIX-FIAV could increase the FVIII-equivalent activity and coagulation activity in HA patient plasma, mitigating the HA phenotype. Therefore, FIX-FIAV could serve as a potential treatment for HA patients.
