The Relevance of Vitamin and Iron Deficiency in Patients with Inflammatory Bowel Diseases in Patients of the Swiss IBD Cohort

Background and Aims: Vitamin and iron deficiencies are common in patients with inflammatory bowel disease (IBD) as a result of chronic intestinal inflammation, increase in demand, or dietary restrictions. Here, we assessed the frequency of complications in relation to deficiency of iron, folate acid, and vitamin B12 in patients enrolled in the nationwide Swiss Inflammatory Bowel Disease Cohort Study (SIBDCS). Methods: A total of 2666 patients were included in the study, 1558 with Crohn's disease (CD) and 1108 with ulcerative colitis (UC). Results: Iron deficiency anemia was detected in 19.6% of CD patients and 21.6% of UC patients. In CD patients low BMI and nonsmoker status were positively associated with anemia. In both CD and UC, malabsorption syndrome, defined as failure of the GI tract to absorb 1 or more substances from the diet, was found to be significantly associated with anemia (6.2% and 3.8%, respectively) and current steroid use (40% CD, 52.7% UC). In CD patients with ileal (31.7% vs 20%) and colonic (29.9% vs 25%) disease location folate deficiency was significantly higher than in patients with ileocolonic CD or upper GI involvement. In CD patients, vitamin B12 deficiency was associated with the onset of stenosis and intestinal surgery (42.9% vs 32.8% and 46% vs 33% for patients with versus without B12 deficiency). Conclusion: Our data indicate that due to frequent occurrence of deficiency states, regular monitoring and substitution of vitamins and iron are mandatory and may prevent long-term intestinal and extraintestinal complications in IBD patients.

Alcohol and cannabis consumption in patients with inflammatory bowel disease: prevalence, pattern of consumption and impact on the disease

Summary: This study evaluated the long-term persistence of resistance-associated substitutions (RASs) in HCV patients treated with DAAs. Low-to medium-level RASs were found to persist over time, while high-level resistant RASs gradually disappeared. Different patterns of RAS persistence according to HCV subtype could have implications for retreatment with first-generation DAAs and for global HCV elimination goals.

DNA Methylation of α-Synuclein Intron 1 Is Significantly Decreased in the Frontal Cortex of Parkinson's Individuals with GBA1 Mutations

Summary: Parkinson's disease (PD) is a common movement disorder, and mutations in the GBA1 gene are the most common genetic risk factor for PD. PD-GBA1 is distinct from idiopathic PD in terms of age of onset, neuropsychiatric symptoms, and cognitive impairment. This study found differences in DNA methylation levels of the SNCA gene between PD-GBA1 and idiopathic PD, suggesting the existence of different genetic subtypes within PD.

Sex-Specific Microglial Responses to Glucocerebrosidase Inhibition: Relevance to GBA1-Linked Parkinson's Disease

Summary: Microglia are heterogenous cells in the nervous system that have distinct populations contributing to specific processes, including neuroprotection. Our study found that male microglia tend to have a more pro-inflammatory phenotype, while female microglia are more sensitive to glucocerebrosidase inhibition. Furthermore, glucocerebrosidase inhibition impaired the ability of female microglia to enhance the Nrf2-dependent detoxification pathway in neurons.

Lifestyle factors associated with inflammatory bowel disease: data from the Swiss IBD cohort study

Summary: This study aimed to identify lifestyle factors that affect the onset of Crohn's disease and ulcerative colitis. The results showed that patients with inflammatory bowel disease were less likely to engage in regular physical activity during childhood and early adulthood compared to the control group. There was no significant association between intake of refined sugar and the onset of inflammatory bowel disease. Patients with Crohn's disease had a higher rate of overweight during childhood compared to ulcerative colitis and the control group. The study highlights the relevance of environmental factors in the development of inflammatory bowel disease and suggests a protective effect of physical activity on its onset.

Performance of two-dimensional shear wave elastography and transient elastography compared to liver biopsy for staging of liver fibrosis

Summary: This study evaluated the diagnostic accuracy of 2D-SWE assessed by the Canon Aplio i800 ultrasound system in liver fibrosis and compared it with traditional liver histology and transient elastography (TE). The results showed that 2D-SWE had good to excellent performance in diagnosing significant fibrosis, severe fibrosis, and cirrhosis, and its accuracy was comparable to TE. Therefore, the 2D-SWE system has a promising potential in the diagnostic workup of chronic liver disease.

Benchmarking microbial DNA enrichment protocols from human intestinal biopsies

Summary: Microbiome DNA enrichment is an effective method to reduce host DNA contamination and improve the sensitivity of metagenomic sequencing studies. This study evaluated different enrichment methods and found that the NEBNext and QIAamp kits were effective in reducing host DNA contamination.

Summary: This study found that AXL is expressed on tissue macrophages in patients with cirrhosis, but not on tissue-infiltrating liver macrophages, hepatocytes, and sinusoidal endothelial cells. The expression of AXL decreases significantly with cirrhosis progression and is negatively correlated with liver function and C-reactive protein. AXL-expressing liver macrophages show high expression of CD68, HLA-DR, CD16, and CD206. Additionally, AXL expression decreases on gut and peritoneal macrophages in cirrhotic patients but increases in regional lymph nodes. These findings suggest an important role for AXL in the regulation of hepatic immune homeostasis.
